Abstract

The principles and embodiments of the present disclosure relate to methods for using terlipressin to treat a patient having impaired renal function associated with liver disease. A method of improving kidney function in an adult patient with hepatorenal syndrome with rapid reduction in kidney function may include determining the patient's acute-on-chronic liver failure (ACLF) grade and baseline serum creatinine level; obtaining a baseline oxygenation saturation (SpO 2 ) of the patient; administering a dose of terlipressin acetate to the patient by intravenous (IV) injection; and monitoring the patient's oxygenation saturation with pulse oximetry.

Claims (45)

1 . A method of improving kidney function in an adult patient with hepatorenal syndrome with rapid reduction in kidney function, the method comprising:

determining the patient's acute-on-chronic liver failure (ACLF) grade;

obtaining a baseline oxygenation saturation (SpO 2 ) of the patient;

administering a dose of terlipressin to the patient by intravenous (IV) injection; and

monitoring the patient's oxygenation saturation with pulse oximetry.

2 . The method of claim 1 , wherein the patient's oxygen saturation is measured continuously.

3 . The method of claim 1 , wherein the patient's oxygen saturation is measured at least 3 times a day.

4 . The method of claim 3 , wherein the patient's oxygen saturation is monitored more frequently if clinically indicated.

5 . The method of claim 1 , wherein if the patient is ACLF Grade 3, terlipressin is only administered when the anticipated benefit to the patient outweighs the potential risk of serious or fatal respiratory failure.

6 . The method of claim 1 , further comprising monitoring the patient with regular physical examinations.

7 . The method of claim 1 , wherein terlipressin administration is not initiated, increased, or continued if the patient is experiencing hypoxia.

8 . The method of claim 7 , wherein an SpO 2 value of <90% is indicative of some degree of hypoxia.

9 . The method of claim 7 , wherein terlipressin administration is initiated, increased, or continued when oxygenation levels improve.

10 . The method of claim 7 , wherein terlipressin administration is initiated, increased, or continued when the SpO 2 value is >90%.

11 . The method of claim 1 , wherein the dose administered is 1 mg terlipressin acetate having a formula of C 52 H 74 N 16 O 15 S 2 . (C 2 H 4 O 2 ) n , wherein n is 2.8.

12 . The method of claim 1 , wherein the dose administered is 0.85 mg terlipressin.

13 . The method of claim 12 , wherein the dose is administered every 6 hours by slow intravenous bolus injection over 2 minutes on days 1 through 3.

14 . The method of claim 13 , further comprising recording a baseline serum creatinine value comprising a last available serum creatinine value prior to initiating treatment.

15 . The method of claim 14 , further comprising measuring the patient's serum creatinine level at day 4, wherein the dose is adjusted based on changes in the serum creatinine level from the baseline serum creatinine to the serum creatinine level at day 4.

16 . The method of claim 15 , wherein:

if the serum creatinine level decreased by at least 30%, continue administering 0.85 mg terlipressin every 6 hours;

if the serum creatinine level has not decreased by 30%, increase the dose up to 1.7 mg of terlipressin every 6 hours; and

if the serum creatinine level is at or above the baseline serum creatinine level, discontinue administering terlipressin.

17 . The method of claim 14 , wherein the terlipressin is administered if the patient has a baseline serum creatinine level of less than 5 mg/dL.

18 . The method of claim 1 , wherein the patient has rapid reduction in kidney function prior to administration of terlipressin.

19 . The method of claim 18 , wherein the patient has improved kidney function after administration of terlipressin.

20 . A method of improving kidney function in an adult patient with hepatorenal syndrome comprising:

determining the patient's acute-on-chronic liver failure (ACLF) grade and volume status prior to administering terlipressin;

obtaining a baseline oxygenation saturation (SpO 2 ) of the patient;

administering a dose of terlipressin to the patient by intravenous (IV) injection; and

monitoring the patient's oxygenation saturation with pulse oximetry.

21 . A method of improving kidney function in an adult patient with hepatorenal syndrome comprising:

administering 0.85 mg terlipressin intravenously to the adult patient every 6 hours from days 1 to 3;

assessing adult patient's serum creatinine at day 4 relative to a patient baseline;

continuing administration of 0.85 mg terlipressin intravenously every 6 hours if serum creatinine has decreased by at least 30% relative to baseline.

22 . The method of claim 21 , wherein the serum creatinine patient baseline is a pre-determined mean patient baseline of 3.5 mg/mL.

23 . The method of claim 21 , wherein the serum creatinine patient baseline is a serum creatinine value of the adult patient recorded prior to administering terlipressin to the adult patient.

24 . The method of claim 21 , further comprising increasing terlipressin dose to 1.7 mg intravenously every 6 hours if SCr has decreased by less than 30% from baseline.

25 . The method of claim 21 , further comprising administering 1 mg terlipressin until 24 hours after two consecutive SCr≤1.5 mg/dL values at least 2 hours apart or a maximum of 14 days.

26 . A method of improving kidney function in an adult patient with hepatorenal syndrome comprising:

administering a starting dosage comprising 1 mg terlipressin acetate intravenously to the adult patient every 6 hours from days 1 to 3;

assessing adult patient's serum creatinine at day 4 relative to a patient baseline;

continuing administration of the terlipressin acetate until 24 hours after two consecutive SCr≤1.5 mg/dL values at least 2 hours apart or a maximum of 14 days,

wherein the terlipressin acetate has a formula of C 52 H 74 N 16 O 15 S 2 . (C 2 H 4 O 2 ) n , wherein n is 2.8.

27 . The method of claim 26 , further comprising increasing the terlipressin acetate dosage to 2 mg intravenously every 6 hours if SCr has decreased by less than 30% from baseline.
