Abstract

Methods and kits for predicting infusion reaction risk and antibody-mediated loss of response during intravenous PEGylated uricase therapy in gout patients is provided. Routine SUA monitoring can be used to identify patients receiving PEGylated uricase who may no longer benefit from treatment and who are at greater risk for infusion reactions.

Claims (18)

1. A method of treating a patient suffering from chronic gout refractory to conventional therapy (GRT), comprising:

administering to the patient 8 mg of a PEGylated uricase protein every 2 weeks by intravenous infusion;

measuring the serum uric acid (SUA) level of the patient after said administration, wherein the patient is a persistent responder; and

discontinuing administration of the PEGylated uricase once the patient is reclassified as a transient responder, wherein the patient is reclassified as a transient responder if the patient's SUA level has more than one measurement above 6 mg/dl.

2. The method of claim 1 , wherein a persistent responder maintains the SUA level at or below 6 mg/dl.

3. The method of claim 1 , wherein the patient is administered a prophylactic amount of fexofenadine and hydrocortisone before each infusion of the PEGylated uricase protein.

4. The method of claim 1 , wherein the PEGylated uricase protein is pegloticase.

5. A method of treating a patient suffering from chronic gout refractory to conventional therapy (GRT), comprising:

(i) administering to the patient 8 mg of PEGylated uricase protein every 2 weeks by intravenous infusion

(ii) measuring the serum uric acid (SUA) level of the patient 2 weeks after said administration;

(iii) classifying the patient as a persistent responder or a transient responder,

(iv) (a) continuing administering the PEGylated uricase protein by intravenous infusion to a persistent responder, or

(b) discontinuing administering the PEGylated uricase to a transient responder; and

(v) repeating the steps (i) to (iv).

6. The method of claim 5 , wherein the patient is classified as a persistent responder where the patient maintains the SUA level at or below 6 mg/dl.

7. The method of claim 5 , wherein the patient is classified as a transient responder if the patient's SUA level has more than one measurement above 6 mg/dl.

8. The method of claim 5 , wherein the patient is administered a prophylactic amount of fexofenadine and hydrocortisone before each infusion of the PEGylated uricase protein.

9. The method of claim 5 , wherein the PEGylated uricase protein is pegloticase.
