Abstract

A method of treating a subject having glaucoma comprises performing excimer laser trabeculostomy (ELT) on a subject having glaucoma and having previously undergone a failed treatment or a treatment that has been rendered ineffective by progression of the disease. In some examples, the failed treatment is a non-surgical treatment comprising administering medicated eye drops. In some examples, the failed treatment is a laser treatment or surgical treatment, such as a trabeculoplasty, iridotomy, iridectomy, trabeculectomy, trabeculotomy, goniotomy, surgical insertion of a shunt or implant, deep sclerectomy, viscocanalostomy, or a combination thereof.

Claims (31)

1 . A method of treating a subject having glaucoma comprising:

determining that medicated eye drops are no longer effective for treating the glaucoma in an eye of the subject, wherein the medicated eye drops were previously used to treat the glaucoma of the subject and wherein the medicated eye drops are no longer effective for treating the glaucoma of the subject where the medicated eye drops no longer control intra-ocular pressure (IOP) of the eye of the subject at a desired level; and

after the determining that the medicated eye drops are no longer effective for treating the glaucoma of the subject, causing an excimer laser trabeculostomy (ELT) to be performed on the subject having the glaucoma, wherein the ELT comprises:

making an incision in the eye of the subject,

inserting a fiber probe into the incision in the eye of the subject,

positioning a delivery tip of the fiber probe at a trabecular meshwork of the eye of the subject, and

applying a plurality of shots from an excimer laser to the trabecular meshwork of the eye of the subject.

2 . The method of claim 1 , wherein the medicated eye drops comprise alpha agonists, beta blockers, carbonic anhydrase inhibitors, cholinergic agonists, prostaglandin/prostamide analogues, or a combination thereof.

3 . The method of claim 1 , further comprising determining, prior to causing the ELT to be performed, that one or more failed treatments comprising a laser treatment has also failed to treat the glaucoma.

4 . The method of claim 3 , wherein the laser treatment comprises a non-invasive laser trabeculoplasty, an iridotomy, an iridectomy, or a combination thereof.

5 . The method of claim 4 , wherein the non-invasive laser trabeculoplasty comprises selective laser trabeculoplasty (SLT).

6 . The method of claim 1 , further comprising determining, prior to causing the ELT to be performed, that one or more failed treatments comprising a surgical treatment has also failed to treat the glaucoma.

7 . The method of claim 6 , wherein the surgical treatment comprises insertion of a shunt or implant, trabeculectomy, trabeculotomy, goniotomy, deep sclerectomy, viscocanalostomy, or a combination thereof.

8 . The method of claim 1 , wherein the excimer laser is a xenon chloride laser.

9 . The method of claim 1 , wherein performing the ELT further comprises:

adjusting placement of the fiber probe in the eye prior to applying the plurality of shots from the excimer laser.

10 . The method of claim 9 , wherein the placement of the fiber probe is in a position transverse to Schlemm's canal in the eye.

11 . The method of claim 10 , wherein a light source aids in adjusting the placement of the fiber probe.

12 . The method of claim 11 , wherein the light source is a Gonio lens, endoscope, or other illumination source.

13 . The method of claim 1 , further comprising administering anesthesia to the subject before inserting the fiber probe.

14 . The method of claim 1 , further comprising performing a post-operative analysis comprising visualizing drainage of aqueous humor and bloody reflux flowing from Schlemm's canal in the eye.

15 . A method of treating a subject having glaucoma comprising:

treating the glaucoma of the subject with medicated eye drops;

determining that the medicated eye drops are no longer effective for treating the glaucoma of the subject; and

after the determining that the medicated eye drops are no longer effective for treating the glaucoma of the subject, performing excimer laser trabeculostomy (ELT) on the subject, wherein the ELT comprises:

inserting a fiber probe into an eye of the subject, and

applying a plurality of shots from an excimer laser to a trabecular meshwork of the eye of the subject.

16 . The method of claim 15 , wherein the medicated eye drops comprise alpha agonists, beta blockers, carbonic anhydrase inhibitors, cholinergic agonists, prostaglandin/prostamide analogues, or a combination thereof.

17 . The method of claim 15 , wherein the medicated eye drops are no longer effective for treating the glaucoma of the subject where the medicated eye drops no longer control intra-ocular pressure (IOP) of the eye of the subject at a desired level.

18 . The method of claim 15 , wherein the excimer laser is a xenon chloride laser.

19 . The method of claim 15 , wherein the fiber probe is placed in a position transverse to Schlemm's canal in the eye during the ELT, and further wherein the plurality of shots creates perforations in the trabecular meshwork of the eye.
