Abstract

Systems and methods for the intravascular treatment of clot material within a blood vessel of a human patient are disclosed herein. A device in accordance with embodiments of the present technology can include, for example, a plurality of interconnected struts forming a unitary structure having a proximal portion and a distal portion. The struts can form a plurality of first cells in the proximal portion and a plurality of second cells, smaller than the first cells, in the distal portion. The device can be pulled against clot material within a blood vessel to engage, disrupt, and/or capture the clot material.

Claims (57)

1. A method of treating a pulmonary embolism within the vasculature of a patient, the method comprising:

advancing an aspiration catheter at least partially through the vasculature of the patient such that a distal portion of the aspiration catheter is positioned proximate to the pulmonary embolism, wherein the aspiration catheter defines an aspiration lumen;

generating vacuum pressure within a barrel of a syringe;

storing the vacuum pressure within the barrel;

applying the vacuum pressure to the aspiration lumen to aspirate at least a first portion of the pulmonary embolism into the aspiration lumen by moving a valve from a first position to a second position to permit fluid flow from the aspiration lumen to the barrel;

determining that a second portion of the pulmonary embolism remains within the vasculature;

advancing a clot treatment device having an expandable member through the aspiration lumen such that the clot treatment device is positioned at least partially distal to the second portion of the pulmonary embolism;

expanding the expandable member of the clot treatment device within the vasculature distal of the pulmonary embolism such that the expandable member has a proximal tapered region and a distal tapered region, wherein the expandable member includes a plurality of interconnected struts forming a unitary structure; and

proximally retracting the expandable member through the second portion of the pulmonary embolism to capture and/or disrupt the second portion of the pulmonary embolism.

2. The method of claim 1 wherein advancing the aspiration catheter comprises positioning the distal portion of the aspiration catheter proximate to the pulmonary embolism within a pulmonary artery of the vasculature of the patient.

3. The method of claim 1 wherein determining that the second portion of the pulmonary embolism remains within the vasculature includes determining that the pulmonary embolism comprises chronic clot material.

4. The method of claim 1 wherein determining that the second portion of the pulmonary embolism remains within the vasculature includes determining that the pulmonary embolism comprises organized clot material.

5. The method of claim 1 , further comprising, after proximally retracting the expandable member:

generating additional vacuum pressure within the barrel of the syringe; and

applying the additional vacuum pressure to the aspiration lumen to aspirate a third portion of the pulmonary embolism into the aspiration lumen.

6. The method of claim 1 wherein the valve is positioned between the barrel and the aspiration lumen.

7. The method of claim 1 wherein the valve inhibits fluid flow between the aspiration lumen and the barrel when the valve is in the first position.

8. The method of claim 7 wherein generating the vacuum pressure within the barrel of the syringe includes generating the vacuum pressure within the barrel while the valve is in the first position.

9. The method of claim 8 wherein the syringe is a vacuum-pressure locking syringe.

10. The method of claim 1 wherein advancing the clot treatment device through the aspiration lumen comprises advancing the clot treatment device through a hemostasis valve separate from the valve.

11. The method of claim 1 wherein the aspiration catheter has a size of 16 French or greater.

12. The method of claim 1 wherein the aspiration catheter has a size of 20 French or greater.

13. A method of treating a deep vein thrombosis within the vasculature of a patient, the method comprising:

advancing an aspiration catheter at least partially through the vasculature of the patient such that a distal portion of the aspiration catheter is positioned proximate to the deep vein thrombosis, wherein the aspiration catheter defines an aspiration lumen;

generating vacuum pressure within a barrel of a syringe;

storing the vacuum pressure within the barrel;

applying the vacuum pressure to the aspiration lumen to aspirate at least a first portion of the deep vein thrombosis into the aspiration lumen by moving a valve from a first position to a second position to permit fluid flow from the aspiration lumen to the barrel;

determining that a second portion of the deep vein thrombosis remains within the vasculature;

advancing a clot treatment device having an expandable member through the aspiration lumen such that the clot treatment device is positioned at least partially distal to the second portion of the deep vein thrombosis;

expanding the expandable member of the clot treatment device within the vasculature distal of the deep vein thrombosis such that the expandable member has a proximal tapered region and a distal tapered region, wherein the expandable member includes a plurality of interconnected struts forming a unitary structure; and

proximally retracting the expandable member through the second portion of the deep vein thrombosis to capture and/or disrupt the second portion of the deep vein thrombosis.

14. The method of claim 13 wherein advancing the aspiration catheter comprises positioning the distal portion of the aspiration catheter proximate to the deep vein thrombosis within a peripheral portion of the vasculature of the patient.

15. The method of claim 13 wherein determining that the second portion of the deep vein thrombosis remains within the vasculature includes determining that the deep vein thrombosis comprises chronic clot material.

16. The method of claim 13 wherein determining that the second portion of the deep vein thrombosis remains within the vasculature includes determining that the deep vein thrombosis comprises organized clot material.

17. The method of claim 13 , further comprising, after proximally retracting the expandable member:

generating additional vacuum pressure within the barrel of the syringe; and

applying the additional vacuum pressure to the aspiration lumen to aspirate a third portion of the deep vein thrombosis into the aspiration lumen.

18. The method of claim 13 wherein the valve is positioned between the barrel and the aspiration lumen.

19. The method of claim 13 wherein the valve inhibits fluid flow between the aspiration lumen and the barrel when the valve is in the first position.

20. The method of claim 19 wherein generating the vacuum pressure within the barrel of the syringe includes generating the vacuum pressure within the barrel while the valve is in the first position.

21. The method of claim 20 wherein the syringe is a vacuum-pressure locking syringe.

22. The method of claim 13 wherein advancing the clot treatment device through the aspiration lumen comprises advancing the clot treatment device through a hemostasis valve separate from the valve.

23. The method of claim 13 wherein the aspiration catheter has a size of 16 French or greater.

24. The method of claim 13 wherein the aspiration catheter has a size of 20 French or greater.

25. A method of treating clot material within the vasculature of a patient, the method comprising:

advancing a catheter at least partially through the vasculature of the patient such that a distal portion of the catheter is positioned proximate to the clot material;

advancing a clot treatment system through the catheter to proximate the clot material,

wherein the clot treatment system includes a delivery catheter, a clot treatment device, and a distal tip, and

wherein, during advancement of the clot treatment system, the clot treatment device is constrained within the delivery catheter and the distal tip abuts a distal end portion of the delivery catheter;

deploying the clot treatment device from the delivery catheter within the vasculature at least partially distal to the clot material;

expanding the clot treatment device within the vasculature distal of the clot material such that the clot treatment device has a proximal tapered region and a distal tapered region, wherein the clot treatment device includes a plurality of interconnected struts forming a unitary structure;

generating suction at the distal portion of the catheter; and

proximally retracting the clot treatment device through the clot material, wherein generating the suction at the distal portion of the catheter comprises generating the suction to aspirate a first portion of the clot material into the catheter before proximally retracting the clot treatment device.

26. The method of claim 25 wherein advancing the clot treatment system through the catheter includes advancing the clot treatment system over a guidewire extending through the catheter, the delivery catheter, and the distal tip.

27. The method of claim 25 wherein proximally retracting the clot treatment device comprises proximally retracting the clot treatment device through a second portion of the clot material remaining in the vasculature to capture the second portion of the clot material.

28. The method of claim 25 wherein the clot material comprises a pulmonary embolism, and wherein advancing the catheter comprises positioning the distal portion of the catheter proximate to the pulmonary embolism within a pulmonary artery of the vasculature of the patient.

29. The method of claim 25 wherein the clot material comprises a deep vein thrombosis, and wherein advancing the catheter comprises positioning the distal portion of the catheter proximate to the deep vein thrombosis within a peripheral portion of the vasculature of the patient.
