Systems and methods for evaluating the integrity of a uterine cavity
A system for accessing a patient's uterine cavity and detecting perforations in the uterus includes an elongated probe having a flow channel extending to a terminal outlet in a distal region of the probe. A fluid source is coupled to the flow channel, and a seal on the probe is positionable in an endocervical canal. The probe may be trans-cervically inserted into the uterine cavity, and a fluid may be introduced through the channel to flow outwardly from the terminal outlet into the uterine cavity. A parameter of said fluid flow is monitored to detect a perforation in the uterus.
1. A system for endometrial ablation comprising:
an elongated probe configured for trans-cervical introduction to a uterine cavity;
an expandable-collapsible energy applicator for ablating uterine tissue in a uterus, the expandable-collapsible energy applicator positioned at a distal end of the elongated probe, the expandable-collapsible energy applicator including a thin-walled flexible polymeric membrane defining an interior chamber; and
a flow channel extending along an exterior of the thin-walled flexible polymeric membrane, the flow channel including a plurality of outlets along the length of the flow channel and opening to the exterior of the thin-walled flexible polymeric membrane;
a pressurized fluid source adapted to deliver a fluid flow through the flow channel to exit the plurality of outlets when the expandable-collapsible energy applicator is positioned in the uterine cavity; and
a controller configured to monitor at least one fluid flow parameter of the fluid flow from a monitoring mechanism selected from the group of a flow rate meter, a pressure sensor and a flow volume meter.
2. The system of claim 1 , wherein the controller is configured to control a flow rate of the fluid flow at a rate of between 0.01 slpm and 1.0 slpm.
3. The system of claim 2 , wherein the controller is configured to determine if the flow rate of the fluid flow drops below a predetermined minimum threshold level over a predetermined time interval after initiation of the fluid flow.
4. The system of claim 3 , wherein the controller is configured to provide a notification that the uterus is not perforated if the flow rate of the fluid flow drops below the predetermined minimum threshold level over the predetermined time interval after initiation of the fluid flow.
5. The system of claim 4 , wherein the predetermined minimum threshold level is between 0.01 slpm and 0.05 slpm.
6. The system of claim 4 , wherein the predetermined time interval is between 1 second and 20 seconds.

The system of claim 4 , wherein the controller is configured to provide a notification that the uterus is perforated to thereby characterize if the flow rate of the fluid flow does not drop below the predetermined minimum threshold level over the predetermined time interval after initiation of the fluid flow.
8. The system of claim 7 , wherein the controller includes a lock-out to prevent activation of the expandable-collapsible energy applicator in conjunction with providing the notification that the uterus is perforated.
9. The system of claim 1 , wherein the controller is configured to provide a notification that the uterus is perforated if a pressure of the fluid flow drops below a predetermined minimum threshold level over a predetermined time interval after initiation of the fluid flow.
10. The system of claim 9 , wherein the controller is configured to provide a notification that the uterus is not perforated if a pressure of the fluid flow remains above a predetermined minimum threshold level over a predetermined time interval after initiation of the fluid flow.
11. The system of claim 9 , wherein the controller includes a lock-out to prevent activation of the expandable-collapsible energy applicator in conjunction with providing the notification that the uterus is perforated.
12. The system of claim 1 , wherein the controller is configured to provide a notification that the uterus is not perforated if a pressure of the fluid flow remains above a predetermined minimum threshold level over a predetermined time interval after initiation of the fluid flow.
13. The system of claim 1 , wherein the controller includes a lock-out to prevent activation of the expandable-collapsible energy applicator.
14. The system of claim 1 , wherein the expandable-collapsible energy applicator is configured to ablate uterine tissue with heat.
15. The system of claim 1 , wherein the fluid flow comprises CO 2 .
16. The system of claim 1 , wherein the thin-walled flexible polymeric membrane is expandable to a triangular shape.
17. The system of claim 16 , wherein the expandable-collapsible energy applicator has a first flow channel extending along one outside wall surface of the thin-walled flexible polymeric membrane and a second flow channel extending along an opposite outside wall surface of the thin-walled flexible polymeric membrane.
18. The system of claim 17 , wherein the first flow channel extends to a distal outlet located at a first distal tip of the thin-walled flexible polymeric membrane.
19. The system of claim 17 , wherein the second flow channel extends to a distal outlet located at a second distal tip of the thin-walled flexible polymeric membrane.
20. The system of claim 1 , wherein the elongate probe includes a cervical seal configured to prevent fluid leakage outwardly from the uterine cavity.
