METHODS FOR ROUTING A GUIDEWIRE FROM A FIRST VESSEL AND THROUGH A SECOND VESSEL IN LOWER EXTREMITY VASCULATURE
A catheter system can include a tubular body, and at least one of a targeting system coupled to the tubular body, an expandable member, or a fluid injection port. A method of identifying a bifurcation can include inserting a catheter system into a first vessel, positioning the catheter system at a first location, expanding an expandable member to occlude the first vessel, delivering contrast material so the contrast material pooling proximate to the expandable member, and reviewing a shape of the contrast material in the first vessel under fluoroscopy.

2 . A system for routing a guidewire from a first blood vessel to a second blood vessel, the system comprising:
a catheter assembly configured to be inserted into the first blood vessel, wherein the catheter assembly comprises—
a sheath defining a lumen and having a distal portion and a proximal portion;
a shaft configured to extend at least partially through the lumen of the sheath; and
a capture structure coupled to the shaft, wherein the capture structure comprises a plurality of interconnected struts defining a plurality of cells, and wherein—
the catheter assembly is movable between a first position and a second position via axial movement of the sheath relative to the shaft and/or via axial movement of the shaft relative to the sheath,
in the first position, the distal portion of the sheath extends over the capture structure to radially constrain the capture structure in a constrained configuration, and
in the second position, the capture structure is unconstrained by the distal portion of the sheath and configured to self-expand to an expanded configuration within the first blood vessel;
wherein, in the expanded configuration, the capture structure is configured to receive a guidewire through at least one of the cells, the guidewire extending from the second blood vessel, through interstitial tissue between the first blood vessel and the second blood vessel, and into the first blood vessel;
wherein the catheter assembly is movable from the second position to the first position to move the capture structure from the expanded configuration to the constrained configuration to capture the guidewire with the capture structure; and
wherein the catheter assembly is retractable from the first blood vessel in the first position with the capture structure in the constrained configuration capturing the guidewire to withdraw the guidewire through and out of the first blood vessel such that the guidewire traverses a path from the second blood vessel through the first blood vessel.
3 . The system of claim 2 wherein the struts of the capture structure comprise nitinol.
4 . The system of claim 2 wherein the struts of the capture structure comprise a radiopaque material.
5 . The system of claim 2 wherein the capture structure comprises one or more radiopaque markers.
6 . The system of claim 2 wherein the catheter assembly is configured to be inserted into the first blood vessel in the first position.
7 . The system of claim 2 wherein the catheter assembly is movable from the first position to the second position via proximal retraction of the sheath relative to the shaft, and wherein the catheter assembly is movable from the second position to the first position to move the capture structure from the expanded configuration to the constrained configuration to capture the guidewire with the capture structure via distal advancement of the sheath relative to the shaft.
8 . The system of claim 2 wherein the first blood vessel is a vein, and wherein the second blood vessel is an artery.
9 . The system of claim 8 wherein the vein and the artery are positioned within a lower extremity of a patient.
10 . The system of claim 8 wherein the vein and the artery are positioned within a leg of a patient.
11 . The system of claim 2 wherein the proximal portion of the sheath is coupled to a hub, wherein the hub has a first port and a second port other than the first port, and wherein the second port is configured to axially receive the shaft.
12 . The system of claim 2 wherein the capture structure is configured to radially expand at least a portion of the first blood vessel in the expanded configuration.
13 . A system for routing a guidewire from a first blood vessel to a second blood vessel, the system comprising:
a first catheter assembly configured to be inserted into the first blood vessel, wherein the first catheter assembly comprises—
a sheath defining a lumen and having a distal portion and a proximal portion;
a shaft configured to extend at least partially through the lumen of the sheath; and
a capture structure coupled to the shaft, wherein the capture structure comprises a plurality of interconnected struts defining a plurality of cells, and wherein—
the first catheter assembly is movable between a first position and a second position via axial movement of the sheath relative to the shaft and/or via axial movement of the shaft relative to the sheath,
in the first position, the distal portion of the sheath extends over the capture structure to radially constrain the capture structure in a constrained configuration, and
in the second position, the capture structure is unconstrained by the distal portion of the sheath and configured to self-expand to an expanded configuration within the first blood vessel; and
a second catheter assembly configured to be inserted into the second blood vessel, wherein the second catheter assembly comprises a needle;
a guidewire configured to be advanced through the second blood vessel and through the needle;
wherein, with the capture structure in the expanded configuration—
the needle is configured to move from the retracted position to the extended position such that the needle extends from the second blood vessel, through interstitial tissue between the second blood vessel and the first blood vessel, into the first blood vessel, and through one of the cells of the capture structure; and
the guidewire is configured to be advanced out of the needle to within the capture structure;
wherein the catheter assembly is movable from the second position to the first position to move the capture structure from the expanded configuration to the constrained configuration to capture the guidewire with the capture structure; and
wherein the catheter assembly is retractable from the first blood vessel in the first position with the capture structure in the constrained configuration capturing the guidewire to withdraw the guidewire through and out of the first blood vessel such that the guidewire traverses a path from the second blood vessel through the first blood vessel.
13 . The system of claim 12 wherein the struts of the capture structure comprise nitinol.
15 . The system of claim 12 wherein the struts of the capture structure comprise a radiopaque material.
16 . The system of claim 12 wherein the capture structure comprises one or more radiopaque markers.
17 . The system of claim wherein the second catheter assembly comprises at least one radiopaque marker visible under fluoroscopy for aligning the needle with the capture structure.
18 . The system of claim 12 wherein the catheter assembly is configured to be inserted into the first blood vessel in the first position.
19 . The system of claim 12 wherein the catheter assembly is movable from the first position to the second position via proximal retraction of the sheath relative to the shaft, and wherein the catheter assembly is movable from the second position to the first position to move the capture structure from the expanded configuration to the constrained configuration to capture the guidewire with the capture structure via distal advancement of the sheath relative to the shaft.
20 . The system of claim 12 wherein the first blood vessel is a vein and wherein the second blood vessel is an artery, and wherein the vein and the artery are positioned within a lower extremity of a patient.
21 . The system of claim 12 wherein the proximal portion of the sheath is coupled to a hub, wherein the hub has a first port and a second port other than the first port, and wherein the second port is configured to axially receive the shaft.
