Therapeutic Regimens and Methods for Reducing Body Weight in a Subject with Fatty Liver Disease using a GLP-1R and GCGR Agonist
This disclosure relates to the once weekly dosing regimen of a dual GLP-1R and GCGR agonist, formulations, and methods of using the same for inducing weight loss in a human being with fatty liver disease, wherein the human may or may not have other comorbidities such as type 2 diabetes and/or cardiovascular disease/disorder. The dual GLP-1R and GCGR agonist includes the peptide product of SEQ ID NO. 1 (pemvidutide).
1 . A method of reducing body weight in a human being with fatty liver disease, the method comprising administering pemvidutide once weekly in an amount from at least about 1.2 to about 2.4 mg, to the human being in need thereof for at least about 12; wherein the fatty liver disease is non-alcoholic fatty liver disease (NAFLD) or non-alcoholic steatohepatitis (NASH).
2 . The method of claim 1 , wherein the body weight of the human being is reduced by at least 3% from baseline at week 12.
3 . The method of claim 1 , wherein the body weight of the human being is reduced by at least 4% from baseline at week 12.

6 . The method of claim 1 , wherein the pemvidutide is administered once weekly in an amount of 2.4 mg.
7 . The method of claim 1 , wherein a steady state dose is achieved after a dose escalating phase having a duration of about 2 weeks, about 3 weeks or about 4 weeks.
8 . The method of claim 1 , wherein the human being has a diagnosis of type 2 diabetes.
9 . The method of claim 1 , wherein the human being does not have a diagnosis of type 2 diabetes.
10 . The method of claim 1 , wherein the human being has a body mass index (BMI kg/m 2 ) of at least 28.
11 . The method of claim 1 , wherein the human being has a body mass index (BMI kg/m 2 ) of 30 or greater.
12 . The method of claim 1 , wherein the human being has a level of liver fat as measured by MRI-PDFF of 10% or greater.
13 . The method of claim 1 , wherein an absolute reduction in liver fat as measured by MRI-PDFF is a reduction of about 8% to about 15% after 12 weeks of the once weekly dosing.
14 . The method of claim 1 , wherein the relative reduction in liver fat as measured by MRI-PDFF compared to baseline is about 40% to about 70% after 12 weeks of the once weekly dosing.
15 . The method of claim 1 , wherein the relative reduction in liver fat as measured by MRI-PDFF compared to baseline is about 30% to about 50% after 24 weeks of the once weekly dosing, wherein the relative reduction is statistically significant defined as p<0.001 or p<0.0001.

17 . The method of claim 1 , wherein defatting of the liver as measured by MRI-PDFF compared to baseline is about 30% after 24 weeks of once weekly dosing of about 1.8 mg pemvidutide as compared to baseline in the human being.

19 . The method of claim 1 , wherein alanine aminotransferase (ALT) is reduced after 24 weeks of once weekly dosing as compared to baseline, wherein the human being has baseline ALT of >30 IU/L before dosing.

41 . A method of inducing weight loss in a human being with fatty liver disease, the method comprising administering pemvidutide once weekly for at least 12 continuous weeks in an amount from at least about 1.2 mg up to about 2.4 mg to the human being, wherein the human being:
has been diagnosed with fatty liver disease that is non-alcoholic fatty liver disease (NAFLD) or non-alcoholic steatohepatitis (NASH);
has a body mass index (BMI kg/m 2 ) of greater than about 28; and,

67 . The method of claim 1 , wherein the pemvidutide is administered from a liquid formulation comprising at least about 2.5 mg/ml pemvidutide.

73 . A liquid pharmaceutical formulation comprising: SEQ ID NO: 1 and about 0.020% (w/w) polysorbate 20, about 0.348% (w/w) arginine, and about 4.260% (w/w) mannitol in deionized water (pH 7.7±0.1).
74 . The formulation of claim 73 , comprising 1.8 mg of SEQ ID NO: 1 as a therapeutic dose.
75 . The formulation of claim 74 , wherein the therapeutic dose induces weight loss in a subject in need thereof.
