Abstract

Some of the present methods include, and some embodiments of the present systems are configured for gaining access to a patient's vessel by way of the vessel (i.e. from the inside out). Some embodiments facilitate gaining access to an occluded vessel, where part of the access path is through the occlusion.

Claims (45)

1. A method for providing access to a central venous system of a patient comprising:

introducing a catheter and a projection angle catheter into a patient in an area remote from a predetermined exit area, wherein the projection angle catheter is configured to extend out a side aperture of a distal tip of the catheter at a projection angle, and a needle wire is configured to extend through the projection angle catheter, wherein the catheter has a distal end aperture at a distal end of the distal tip configured such that the needle wire can extend through the distal end aperture;

advancing the distal tip of the catheter into a target in the central venous system, the distal tip being configured to penetrate the target without deflection when advanced into the target;

advancing the needle wire through the projection angle catheter when the distal tip of the catheter is disposed in the target, such that a distal end of the needle wire advances at an angle relative to the catheter and penetrates the skin of the patient adjacent the predetermined exit area thereby providing the distal end of the

needle wire exterior to the skin, the needle wire having a bending stiffness sufficient to penetrate a muscle tissue and the skin of the patient without deflection.

2. The method of claim 1 , wherein the target to which the needle wire is advanced is located within a thrombus.

3. The method of claim 1 , wherein advancing the distal tip of the catheter into the target in the central nervous system comprises advancing the catheter over the needle wire into the target in the central nervous system.

4. The method of claim 1 , further comprising:

advancing the needle wire through the distal end aperture of the catheter to the target in the central venous system; and

retracting the needle wire from the target into the catheter.

5. The method of claim 1 , wherein the projection angle catheter comprises a preformed curved distal portion.

6. The method of claim 5 , further comprising:

advancing the needle wire through the distal end aperture of the catheter to the target in the central venous system; and

retracting the needle wire from beyond the distal end aperture into the catheter and into the projection angle catheter, wherein the distal end of the needle wire is spaced apart from a distal end of the projection angle catheter at least a distance such that the needle wire is not impeding the curvature of the preformed curved distal portion.

7. The method of claim 5 , further comprising:

advancing the needle wire through the distal end aperture of the catheter to the target in the central venous system; and

retracting the needle wire from beyond the distal end aperture into the catheter and into the projection angle catheter, wherein the distal end of the needle wire is disposed in a non-curved portion of the projection angle catheter.

8. The method of claim 1 , further comprising:

advancing the needle wire through the distal end aperture of the catheter to the target in the central venous system; and

retracting the needle wire from beyond the distal end aperture into the catheter and into the projection angle catheter, wherein the distal end of the needle wire is spaced apart from a distal end of the projection angle catheter at least a distance of 3 mm.

9. The method of claim 8 , further comprising:

advancing a guide wire into the dilation catheter and advancing a medical device over the guide wire along the dilated tissue track into the central venous system.

10. The method of claim 1 , further comprising:

attaching a dilation catheter to a portion the needle wire that is exterior to the skin and drawing the dilation catheter into the predetermined exit area by pulling on a proximal end of the needle wire thereby forming a dilated tissue track.

11. The method of claim 1 , wherein the target includes a thrombus and the distal tip of the catheter is configured to penetrate the thrombus without deflection.

12. The method of claim 1 , wherein the predetermined exit area is defined by a radiolucent area of a radiopaque target applied to the patient and having a radiopaque area and the radiolucent area.

13. A method for providing access to a central venous system of a patient comprising:

introducing a catheter into the patient, wherein the catheter has a projection angle catheter configured to extend out a side aperture of a distal tip of the catheter, and a needle wire configured to extend through the projection angle catheter, wherein the catheter has an aperture at a distal end of the distal tip;

advancing the catheter to position said distal tip in the vessel occlusion, the distal tip being configured to penetrate the vessel occlusion without deflection when the catheter is advanced to position the distal tip in the vessel occlusion;

advancing the projection angle catheter so that the catheter extends through the side aperture; and

advancing the needle wire through the projection angle catheter and through the skin of the patient, thereby providing a distal end of the needle wire exterior to the skin, the needle wire having a bending stiffness sufficient to penetrate a muscle tissue and skin of the patient without deflection.

14. The method of claim 13 , wherein the vessel occlusion is located within the central venous system.

15. The method of claim 13 , wherein the needle wire follows a substantially straight path when advanced from the catheter to the surface of the skin.

16. A catheter system for accessing a central venous system of a patient, the catheter system comprising:

a first catheter comprising a distal tip configured to penetrate a vessel occlusion without deflection and having a side aperture and a distal end aperture at a distal end of the first catheter;

a second catheter having a portion configured to extend through the first catheter and at least partially through the side aperture,

a needle wire configured to extend through the second catheter and to extend through the distal end aperture of the first catheter wherein the needle wire has a bending stiffness sufficient to penetrate a muscle tissue without deflection from a location within a vessel to a location exterior to a body of the patient.

17. The catheter system of claim 16 , wherein the distal tip further comprises a second side aperture and a radiopaque material defining the second side aperture.

18. The catheter system of claim 17 , wherein the distal tip further comprises a third side aperture located on an opposite side of the distal tip from the second side aperture, wherein the radiopaque material defines the third side aperture.

19. The catheter system of claim 16 , wherein a distal portion of the side aperture is defined by a surface that is a ramp configured to guide the second catheter through the side aperture.

20. The catheter system of claim 16 , wherein a distal portion of the side aperture is defined by a curved, sloped surface.

21. The catheter system of claim 16 , wherein the curved, sloping surface is a concave surface.

22. The catheter system of claim 16 , wherein the curved, surface is an inclined surface, wherein an incline angle of the inclined surface is at an angle relative to a longitudinal axis of the first catheter and sloped inward toward the distal end of the shaft.

23. The catheter system of claim 22 , wherein the distal tip comprises a sidewall body having an inner luminal surface, wherein a portion of the inner luminal surface that is proximal and opposite to side aperture is a sloped surface.

24. The catheter system of claim 16 , wherein the distal tip further comprises a second side aperture opposite the side aperture such that a line transverse to a longitudinal axis of the first catheter passes through the side aperture and the second side aperture.
