Abstract

Devices, systems, and methods for delivering fluid therapy to a patient are disclosed herein. An exemplary method can comprise obtaining a urine output rate from a patient; causing a diuretic to be provided to the patient at a dosage rate, and increasing the dosage rate of the diuretic from the initial dosage rate to additional dosage rates higher than the initial dosage rate over a period of time. In some embodiments, the additional dosage rates include a first dosage rate higher than the initial dosage rate, a second dosage rate higher than the first dosage rate, and a third dosage rate higher than the second dosage rate.

Claims (54)

1 . A fluid therapy system, comprising:

a urine measurement device configured to measure urine output from a patient;

a pump configured to provide a diuretic to the patient;

one or more processors; and

tangible, non-transitory computer-readable media having instructions that, when executed by the one or more processors, cause the fluid therapy system to perform operations comprising—

obtaining a urine output rate from the urine measurement device;

causing the diuretic to be provided, via the pump, to the patient at an initial dosage rate; and

increasing the dosage rate of the diuretic from the initial dosage rate to additional dosage rates higher than the initial dosage rate over a period of time of 120 minutes, the additional dosage rates including at least:

a first dosage rate higher than the initial dosage rate,

a second dosage rate higher than the first dosage rate, and

a third dosage rate higher than the second dosage rate.

2 . The fluid therapy system of claim 1 , the operations further comprising, after increasing the dosage rate, setting the dosage rate of the diuretic to be a nonzero predetermined percentage of a current dosage rate.

3 . The fluid therapy system of claim 1 , the operations further comprising:

after the period of time, setting the dosage rate of the diuretic to be a nonzero predetermined percentage of a total amount of the diuretic delivered at the time of determining the urine output rate is above the predetermined threshold.

4 . The fluid therapy system of claim 1 , the operations further comprising:

determining that an average urine output rate measured over a preset time period is above the predetermined threshold; and

in response to the determination, decreasing the dosage rate of the diuretic by a nonzero predetermined percentage.

5 . The fluid therapy system of claim 1 , wherein the predetermined threshold is a first predetermined threshold, the operations further comprising:

determining that one or more of the following set of conditions exists:

(i) an average urine output rate measured over a first preset time period is above the first predetermined threshold;

(ii) the urine output rate measured over a second preset time period is increasing at a rate above a predetermined rate of increase; or

(iii) the dosage rate is above a second predetermined threshold; and

in response to determining that one or more of the set of conditions exists, decreasing the dosage rate of the diuretic by a predetermined percentage.

6 . The fluid therapy system of claim 5 , wherein the first predetermined threshold is at least 500 mL/hour, the predetermined rate of increase is at least 30 ml/hour 2 , and/or the second predetermined threshold is at least 5 mg/hour.

7 . The fluid therapy system of claim 1 , wherein increasing the dosage rate comprises increasing the dosage rate by at least 200% over the period of time.

8 . The fluid therapy system of claim 1 , wherein the period of time is 60 minutes.

9 . The fluid therapy system of claim 1 , wherein, in operation, increasing the dosage rate occurs as the urine output rate increases.

10 . The fluid therapy system of claim 1 , wherein obtaining the urine output rate comprises obtaining the urine output every 10 minutes or less.

11 . The fluid therapy system of claim 1 , wherein a dose for the first dosage rate is immediately subsequent to a dose for the initial dosage rate.

12 . The fluid therapy system of claim 1 , wherein the first dosage rate occurs within 30 minutes of the initial dosage rate.

13 . A fluid therapy system, comprising:

a urine measurement device configured to measure urine output from a patient;

a pump configured to provide a diuretic to the patient;

one or more processors; and

tangible, non-transitory computer-readable media having instructions that, when executed by the one or more processors, cause the fluid therapy system to perform operations comprising—

obtaining a urine output rate;

causing the diuretic to be provided to the patient at an initial dosage rate; and

increasing the dosage rate of the diuretic from the initial dosage rate to three or more additional dosage rates higher than the initial dosage rate over a period of time within 120 minutes, wherein, in operation, an end of the period of time is based at least in part on the urine output rate being above a predetermined threshold.

14 . The fluid therapy system of claim 13 , the operations further comprising:

determining that an average urine output rate measured over a preset time period is below the predetermined threshold; and

in response to the determination, iteratively increasing the dosage rate of the diuretic in an exponential manner.

15 . The fluid therapy system of claim 13 , wherein increasing the dosage rate comprises iteratively increasing the dosage rate in an exponential manner.

16 . The fluid therapy system of claim 15 , wherein automatically increasing the dosage rate comprises iteratively increasing the dosage rate such that each dosage rate increase is greater than a previous dosage rate increase.

17 . A fluid therapy system, comprising:

a urine measurement device configured to measure urine output from a patient;

a pump configured to provide a diuretic to the patient;

one or more processors; and

tangible, non-transitory computer-readable media having instructions that, when executed by the one or more processors, cause the fluid therapy system to perform operations comprising—

obtaining a urine output rate from the urine measurement device;

providing an initial dosage rate to the pump; and

increasing the dosage rate provided to the pump by repeatedly increasing the dosage rate at least three times for each subsequent dose over a period of time of 120 minutes, until (i) the urine output rate is above a predetermined threshold, (ii) a predetermined amount of time has elapsed since providing the initial dosage rate, and/or (iii) the urine output rate is above a predetermined threshold and the predetermined amount of time has elapsed since providing the initial dosage rate.

18 . The fluid therapy system of claim 17 , wherein obtaining the urine output rate comprises obtaining the urine output rate every 10 minutes or less.

19 . The fluid therapy system of claim 17 , wherein the first of the three subsequent doses occurs within 30 minutes of the initial dosage rate.

20 . The fluid therapy system of claim 17 , wherein the first of the three subsequent doses is immediately subsequent to the initial dosage rate.
