Abstract

A method for treating a nasal airway to ameliorate one or more symptoms of rhinitis in a patient may involve activating a radiofrequency console attached to a stylus, bending a shaft of the stylus in at least one location to a desired angle, advancing a distal tip of the radiofrequency stylus into a nostril of the patient, applying pressure against nasal mucosa lining the nasal airway with a treatment surface of the distal tip, and delivering radiofrequency energy from one set of bipolar electrodes on the treatment surface of the distal tip to a second set of bipolar electrodes on the treatment surface, to treat tissue underlying the nasal mucosa, including at least one nasal nerve. The method may also involve contacting the distal tip with an additional tissue at another location and delivering radiofrequency energy to the additional tissue.

27 . A device for treating nasal airway tissue, the device comprising:

a handle;

a shaft extending from a distal end of the handle;

a distal tip extending from a distal end of the shaft, the distal tip comprising:

a treatment surface;

a plurality of rows of bipolar radiofrequency electrodes on the treatment surface configured to contact and treat a first target tissue; and

a temperature sensing member on the treatment surface; and

an expandable wire electrode component having a plurality of bipolar electrode pairs located at various locations along its length, the expandable wire electrode component being configured to be movable between a first position in which a distal end of the expandable wire electrode component is positioned proximal to a distal end of the shaft and a second position in which the distal end of the expandable wire electrode component is advanced out of a distal end of the shaft and expands to contact and treat a second target tissue.

28 . The device of claim 27 , wherein the handle comprises a slider configured to be advanced to move the expandable wire electrode component between the first position and the second position.

29 . The device of claim 27 , wherein the first target tissue is one of a posterior nasal nerve tissue, turbinates, and nasal swell bodies.

30 . The device of claim 27 , wherein the first target tissue is one or more nasal turbinates in areas anterior to a posterior nasal nerve.

31 . The device of claim 30 , where in the second target tissue is the posterior nasal nerve.

32 . The device of claim 27 , wherein the expandable wire electrode component includes one or more thermocouples configured to measure a temperature of nasal mucosa.

33 . The device of claim 27 , wherein the expandable wire electrode component includes one or more nerve sensors configured to sense a location of nerve tissue under nasal mucosa.

34 . The device of claim 27 , wherein the expandable wire electrode component is configured to activate bipolar electrode pairs of the expandable wire electrode component located directly over those sensed nerves.

35 . The device of claim 27 , wherein all electrode pairs of the expandable wire electrode component are configured to be activated simultaneously.

36 . The device of claim 27 , wherein only one or more selected electrode pairs of the expandable wire electrode component are configured to be activated.

37 . The device of claim 27 , wherein the expandable wire electrode component is configured to extend through the shaft and has a cylindrical shape such that the expandable wire electrode component forms a loop around the distal tip.

38 . A device for treating nasal airway tissue, the device comprising:

a handle;

a shaft extending from a distal end of the handle;

a distal tip extending from a distal end of the shaft, the distal tip comprising:

a treatment surface;

two rows of bipolar radiofrequency electrodes on the treatment surface; and

a temperature sensing member on the treatment surface; and

an expandable wire electrode component configured to be advanced out of a distal end of the shaft, wherein the expandable wire electrode component comprises at least one pair of bipolar radiofrequency electrodes.

39 . The device of claim 38 , wherein the expandable wire electrode component is configured to extend through the shaft and has a cylindrical shape such that the expandable wire electrode component forms a loop around the distal tip.

40 . The device of claim 38 , wherein the expandable wire electrode component includes one or more thermocouples configured to measure a temperature of nasal mucosa.

41 . The device of claim 38 , wherein the expandable wire electrode component includes one or more nerve sensors configured to sense a location of nerve tissue under nasal mucosa.

42 . The device of claim 41 , wherein the expandable wire electrode component is configured to activate bipolar electrode pairs of the expandable wire electrode component located directly over those sensed nerves.

43 . The device of claim 38 , wherein the treatment surface of the distal tip is positioned in a nasal cavity to treat a first target tissue, and the expandable wire electrode component is positioned in the nasal cavity to treat a second target tissue.

44 . A method for treating a nasal airway, the method comprising:

advancing a distal tip of a stylus into a nostril of a patient, the stylus comprising:

a shaft having a distal tip extending therefrom, the distal tip comprising a treatment surface and a plurality of rows of bipolar radiofrequency electrodes on the treatment surface configured to contact and treat a first target tissue, and an expandable wire electrode component having a plurality of bipolar electrode pairs located at various locations along its length, the expandable wire electrode component being configured to be movable between a first position in which a distal end of the expandable wire electrode component is positioned proximal to a distal end of the shaft and a second positioned in which the distal end of the expandable wire electrode component is advanced out of a distal end of the shaft and expands to contact and treat a second target tissue;

applying pressure against nasal mucosa lining the nasal airway with the treatment surface of the distal tip;

delivering radiofrequency energy from the bipolar radiofrequency electrodes on the treatment surface of the distal tip to treat the first target tissue;

delivering radiofrequency energy from the expandable wire electrode component to treat the second target tissue; and

removing the distal tip of the stylus from the nostril.

45 . The method of claim 44 , further comprising sensing a temperature of the nasal mucosa with a temperature sensing member located on the treatment surface of the distal tip.

46 . The method of claim 45 , further comprising automatically shutting off delivery of radiofrequency energy from a console to the stylus if the temperature of the nasal mucosa is above a predefined acceptable maximum temperature.
