Abstract

It has been discovered that pain felt in a given region of the body can be treated, not by motor point stimulation of muscle in the local region where pain is felt, but by stimulating muscle spaced from a “nerve of passage” in a region that is superior (i.e., cranial or upstream toward the spinal column) to the region where pain is felt. Spinal nerves such as the intercostal nerves or nerves passing through a nerve plexus, which comprise trunks that divide by divisions and/or cords into branches, comprise “nerves of passage.”

Claims (38)

1 . A method for treating pain, the method comprising the steps of:

providing an introducer needle preloaded with a coiled lead having an insulated portion and an electrode formed from a deinsulated portion;

percutaneously inserting the introducer needle into a body at an angle at a puncture site;

depositing the electrode outside a spinal column in a region having a spinal nerve that is between a painful region and a center of the body of a patient,

electrically coupling the coiled lead to a pulse generator; and

stimulating the spinal nerve to recruit target fibers in the spinal nerve while minimizing recruitment of non-target fibers in the spinal nerve, wherein the electrode is within electrical proximity to and physically spaced from the spinal nerve.

2 . The method of claim 1 further comprising applying a test stimulation to the spinal nerve to recruit target fibers in the spinal nerve while minimizing recruitment of non-target fibers in the spinal nerve.

3 . The method of claim 1 , wherein the stimulating of the spinal nerve comprises application of electrical stimulation having stimulation parameters of 4 mA, 100 μs, 100 Hz, and an on-off duty cycle of 0.25 sec.

4 . The method of claim 1 further comprising advancing the introducer needle until a patient reports an evoked sensation in the painful region.

5 . The method of claim 4 further comprising increasing a stimulus amplitude until a stimulation-evoked tingling sensation expands to overlay the painful region.

6 . A system for reducing and/or relieving pain in a painful region comprising:

an introducer needle preloaded with a coiled lead having an insulated portion and an electrode formed from a deinsulated portion, wherein the introducer needle is configured to be percutaneously inserted into a body at an angle at a puncture site;

wherein the electrode is configured to be outside of a spinal column in a region having a spinal nerve that is between a painful region and a center of the body of a patient; and

a pulse generator electrically coupled the coiled lead, wherein the pulse generator is configured to stimulate the spinal nerve to recruit target fibers in the spinal nerve while minimizing recruitment of non-target fibers in the spinal nerve, wherein the electrode is within electrical proximity to and physically spaced from the spinal nerve.

7 . A system for reducing and/or relieving pain in a painful region comprising:

a coiled lead comprising a monopolar electrode; and

an external stimulating device operatively coupled with the coiled lead to apply stimulation to a spinal nerve innervating a painful region perceived by a patient with the monopolar electrode inserted into muscle tissue that is: i) distant to the painful region, ii) spaced from the spinal nerve to recruit target fibers in the spinal nerve while minimizing recruitment of non-target fibers in the spinal nerve, and iii) between the painful region and a center of a body of a patient.

8 . The system of claim 7 , wherein the coiled lead is configured to withstand mechanical forces and resist migration.

9 . The system of claim 7 , wherein the coiled lead comprises an insulation positioned on an exterior of the coiled lead.

10 . The system of claim 9 , wherein the insulation comprises a biocompatible polymer film.

11 . The system of claim 10 , wherein the biocompatible polymer film comprises at least one of polyfluorocarbon, polymide and parylene.

12 . The system of claim 9 , wherein the coiled lead comprises an anchoring element.

13 . The system of claim 9 , wherein the monopolar electrode is formed from a deinsulated portion of the coiled lead.

14 . The system of claim 13 , wherein the monopolar electrode is formed differently from a remaining portion of the coiled lead.

15 . The system of claim 13 , wherein the monopolar electrode is wound with a different pitch than a remaining portion of the coiled lead.

16 . The system of claim 13 , wherein the monopolar electrode is wound such that the electrode has a different diameter than a remaining portion of the coiled lead.

17 . The system of claim 13 , wherein the monopolar electrode comprises a different material than a lead material of the coiled lead.

18 . A system to alleviate pain perceived by a patient in a painful region, the system comprising:

a lead comprising an electrode, wherein the lead is configured to withstand mechanical forces in flexible peripheral regions of a body of a patient, and the electrode is configured to be placed outside of a spinal column and within a peripheral tissue region that comprises skeletal muscle and a spinal nerve, wherein the tissue region is between the painful region and a center of the body of the patient, the tissue region is distant to the painful region and wherein the electrode is spaced from the spinal nerve, and

an external electrical pulse generator operatively coupled with the lead applying stimulation to the electrode according to predefined therapeutic stimulation parameters to provide therapeutic nerve stimulation to recruit target fibers in the spinal nerve while minimizing recruitment of non-target fibers in the spinal nerve to alleviate pain in the painful region without functional nerve stimulation at a motor point.

19 . The system of claim 18 , wherein the electrode is formed from a deinsulated portion of the lead.

20 . The system of claim 19 , wherein the electrode is formed differently from a remaining portion of the lead.

21 . The system of claim 19 , wherein the electrode is wound with a different pitch than a remaining portion of the lead.

22 . The system of claim 19 , wherein the electrode is wound such that the electrode has a different diameter than a remaining portion of the lead.

23 . The system of claim 19 , wherein the electrode comprises a different material than a lead material of the lead.

24 . A system for reducing and/or relieving pain in a painful region comprising:

a lead comprising an electrode, wherein the electrode is formed from a deinsulated portion of the lead and wherein the electrode is formed differently from a remaining portion of the lead; and

a stimulating device operatively coupled with the lead to apply stimulation to a spinal nerve innervating a painful region perceived by a patient with the electrode inserted into muscle tissue that is: i) distant to the painful region, ii) spaced from the spinal nerve to recruit target fibers in the spinal nerve while minimizing recruitment of non-target fibers in the spinal nerve, and iii) between the painful region and a center of a body of a patient.
