Abstract

Devices and methods establish and facilitate retrograde or reverse flow blood circulation in the region of the carotid artery bifurcation in order to limit or prevent the release of emboli into the cerebral vasculature such as into the internal carotid artery. The methods are particularly useful for interventional procedures performed through a transcarotid approach or transfemoral into the common carotid artery.

Claims (48)

21 . A system for accessing and treating a carotid artery, the system comprising:

an arterial access sheath;

a shunt coupled to the arterial access sheath;

a venous return catheter coupled to the shunt; and

a flow control assembly disposed along the shunt, the flow control assembly comprising:

a housing,

a first fluid pathway disposed within the housing,

a second fluid pathway disposed within the housing, and

an actuator configured to shift flow through the housing between the first fluid pathway and the second fluid pathway.

22 . The system of claim 21 , wherein the first fluid pathway is a high flow pathway.

23 . The system of claim 21 , wherein the second fluid pathway is a low flow pathway.

24 . The system of claim 21 , wherein the actuator includes a valve that closes the first fluid pathway and directs fluid toward the second fluid pathway.

25 . The system of claim 21 , further comprising a second actuator.

26 . The system of claim 25 , wherein the second actuator comprises a shut-off valve.

27 . The system of claim 21 , wherein the actuator includes a depressible button disposed along an exterior of the housing.

28 . The system of claim 21 , wherein the first fluid pathway is substantially linear.

29 . The system of claim 21 , wherein the second fluid pathway includes one or more curved regions.

30 . The system of claim 21 , wherein the first fluid pathway is shorter than the second fluid pathway.

31 . The system of claim 21 , further comprising a filter disposed within the housing.

32 . The system of claim 31 , wherein the filter is removable from the housing.

33 . The system of claim 21 , further comprising a check valve disposed within the housing.

34 . A system for accessing and treating a carotid artery, the system comprising:

an arterial access sheath;

a shunt coupled to the arterial access sheath;

a venous return catheter coupled to the shunt; and

a flow control assembly disposed along the shunt, the flow control assembly comprising:

a housing,

a main lumen extending through the housing,

a secondary lumen extending through the housing,

an actuator configured to shift flow through the housing between a high flow state and a low flow state,

wherein the high flow state allows flow through the main lumen, and

wherein the low flow state allows flow through the secondary lumen.

35 . The system of claim 34 , wherein the actuator includes a valve that closes the main lumen and directs flow toward the secondary lumen.

36 . The system of claim 34 , further comprising a second actuator.

37 . The system of claim 36 , wherein the second actuator comprises a shut-off valve.

38 . The system of claim 34 , wherein the main lumen is substantially linear.

39 . The system of claim 34 , wherein the secondary lumen includes one or more curved regions.

40 . A system for accessing and treating a carotid artery, the system comprising:

an arterial access sheath;

a shunt coupled to the arterial access sheath; and

a flow control assembly disposed along the shunt, the flow control assembly comprising:

a housing,

a main lumen extending through the housing,

a secondary lumen extending through the housing,

an actuator coupled to the housing, the actuator including a valve that is configured to shift flow through the housing between a high flow state and a low flow state,

wherein the high flow state allows flow through the main lumen, and

wherein the low flow state allows flow through the secondary lumen.
