Abstract

Systems and methods for cardiac pacing during a procedure are disclosed and may include an external pulse generator (EPG) for connecting to a lead. A remote-control module (RCM) wirelessly connected to the EPG may include user inputs to control the EPG. A central processing unit (CPU) with a memory unit for storing code and a processor for executing the code may be included where the CPU is connected to the EPG and RCM. The code may control the EPG in response to user input from the RCM. The CPU may be disposed in the EPG or the RCM, or an interface module (IM) configured to communicate between an otherwise conventional EPG and the RCM. The executable code may perform a continuity test (CT) routine, a capture check (CC) routine, rapid pacing (RP) routine, and/or a back-up pacing (BP) routine, in response to user input from the RCM.

Claims (57)

31 . A method for operating an external pacing device, the method comprising:

generating pace pulses having a first paced pulse rate (PPR) using a rapid pacing preset, wherein the pace pulses having the first PPR elevate a heart-rate (HR) of a patient to an elevated HR above an intrinsic HR;

subsequent to generating pace pulses having the first PPR, generating pace pulses having a second PPR, wherein the second PPR is lower than the intrinsic HR;

subsequent to generating pace pulses having the second PPR, sensing a HR of the patient;

determining that the sensed HR of the patient is greater than the second PPR; and

in response to determining that the sensed HR of the patient is greater than the second PPR, one of generating pace pulses having a third PPR using a pacing preset or ceasing generation of pace pulses.

32 . The method of claim 31 , wherein the pace pulses having the first PPR or the pace pulses having the second PPR are generated in response to a user input.

33 . The method of claim 31 , further comprising activating an indicator in response to generating pace pulses having the first PPR or the second PPR.

34 . The method of claim 31 , wherein the second PPR is determined using an inhibit pacing preset.

35 . The method of claim 31 , wherein generating pace pulses having the second PPR comprises ramping the first PPR down to the second PPR.

36 . The method of claim 31 , further comprising:

automatically maintaining pace pulses having the first PPR; and

performing a valve deployment cardiac intervention while maintaining pace pulses having the first PPR.

37 . The method of claim 36 , wherein the pace pulses having the second PPR are generated after performing the valve deployment cardiac intervention.

38 . The method of claim 31 , wherein sensing the HR of the patient comprises:

receiving an electrical signal over a guidewire; and

determining the HR of the patient based on the electrical signal.

39 . A method for operating an external pacing device, the method comprising:

determining that a current heart-rate (HR) of a patient is above an intrinsic HR;

subsequent to determining that the current heart-rate (HR) of the patient is above the intrinsic HR, generating pace pulses having a first paced pulse rate (PPR) using a backup pacing preset, wherein the pace pulses having the first PPR reduce the current HR of the patient to a reduced HR below the current HR;

subsequent to generating pace pulses having the first PPR, generating pace pulses having a second PPR, wherein the second PPR is lower than an intrinsic HR and the first PPR;

subsequent to generating pace pulses having the second PPR, sensing an updated HR of the patient;

determining that the updated HR of the patient is greater than the second PPR; and

in response to determining that the updated HR of the patient is greater than the second PPR, one of generating pace pulses having a third PPR using a pacing preset or ceasing generation of pace pulses.

40 . The method of claim 39 , wherein the pace pulses having the first PPR or the second PPR are generated in response to a user input.

41 . The method of claim 39 , further comprising activating an indicator in response to generating pace pulses having the first PPR or pace pulses having the second PPR.

42 . The method of claim 39 , further comprising:

prior to generating pace pulses having the first PPR, performing a valve deployment cardiac intervention.

43 . The method of claim 39 , wherein sensing the updated HR of the patient comprises:

receiving an electrical signal over a unipolar lead; and

determining the updated HR of the patient based on the electrical signal.

44 . A system for cardiac pacing, the system comprising:

a grounding pad;

a unipolar lead comprising a unipolar guidewire;

an external pulse generator (EPG) configured to electrically connect to the unipolar lead and the grounding pad, and further configured to output pace pulses;

a remote-control module (RCM) wirelessly connected to the EPG, wherein the RCM is configured to receive user inputs and to control the EPG; and

a central processing unit (CPU) operably connected to the EPG and RCM, the CPU including code having instructions, wherein the CPU is configured to execute the code to perform a routine in response to the user inputs received at the RCM, the routine comprising:

transmitting, via the unipolar lead, pace pulses having a first paced pulse rate (PPR) using a rapid pacing preset, wherein the pace pulses having the first PPR elevate a heart-rate (HR) of a patient to an elevated HR above an intrinsic HR;

subsequent to transmitting pace pulses having the first PPR, transmitting, via the unipolar lead, pace pulses having a second PPR, wherein the second PPR is lower than the intrinsic HR;

subsequent to generating pace pulses having the second PPR, sensing, via the unipolar lead, a HR of the patient;

determining that the sensed HR of the patient is greater than the second PPR; and

in response to determining that the sensed HR of the patient is greater than the second PPR, one of generating pace pulses having a third PPR using a pacing preset or ceasing generation of pace pulses.

45 . The system of claim 44 , wherein the routine further comprises:

receiving a first user input of the user inputs via the RCM;

transmitting the pace pulses having the first PPR in response to the first user input;

receiving a second user input of the user inputs via the RCM; and

transmitting the pace pulses having the second PPR in response to the second user input.

46 . The system of claim 44 , wherein the routine further comprises:

detecting a type of the unipolar lead; and

automatically selecting an operation mode of the EPG based on detecting the type of unipolar lead.

47 . The system of claim 44 , wherein the unipolar lead is an intracardiac lead.

48 . The system of claim 44 , wherein the routine further comprises:

determining that the unipolar lead is connected to the EPG; and

activating an indicator based on determining that the unipolar lead is connected to the EPG.

49 . The system of claim 44 further comprising a guidewire connector connected to the EPG, the guidewire connector being configured to penetrate a partial insulative outer portion of the unipolar lead to establish electrical connection with the unipolar guidewire.

50 . The system of claim 44 , wherein the CPU is disposed in the EPG or the RCM.
