Abstract

A method for forming and abrading an implant void in a sacroiliac joint (“SI Joint”) without the use of a rotary cutting instrument. The method incorporates a multimodal abrading device having abrading surfaces on opposing sides, an open tip having a leading edge defining an enclosed oblong perimeter, and a cutting edge disposed at the leading edge of the open tip. The method includes the step of using the abrading head to cut bone tissue from the SI Joint at an insertion point while simultaneously using the abrading surfaces to decorticate the cortical bone at the insertion point.

Claims (29)

1 . A surgical instrument for creating an implant-receiving void in a sacroiliac joint, comprising:

an elongated shaft having a distal end and a proximal end;

a handle disposed at the proximal end of the elongated shaft;

an abrading head disposed at the distal end of the elongated shaft, the abrading head comprising:

a pair of abrading surfaces disposed in opposing relation to one another, each abrading surface having teeth, barbs, blades, ridges, slots, or broaches configured to abrade sacrum and ilium bones within the sacroiliac joint;

a pair of lateral sides disposed in opposing relation to one another, wherein the lateral sides connect the pair of abrading surfaces, collectively enclosing an interior void, and wherein terminal edges of the abrading surfaces and the lateral sides collectively form a contiguous cutting edge having an oblong perimeter and configured to cut a cortical layer of the sacrum and ilium bones within the sacroiliac joint; and

a pair of openings disposed in each of the lateral sides providing ingress into and egress from the interior void, thereby enabling dislodged bone tissue to exit the interior void via the pair of openings.

2 . The surgical instrument of claim 1 , further comprising a protrusion disposed on the elongated shaft configured to operatively engage a complementary groove, recess, channel, or indent of a working channel providing access to the sacroiliac joint.

3 . The surgical instrument of claim 1 , further comprising a groove, recess, channel, or indent disposed on the elongated shaft configured to operatively engage a complementary protrusion disposed on a working channel providing access to the sacroiliac joint.

4 . The surgical instrument of claim 1 , wherein the oblong perimeter has rounded corners.

5 . The surgical instrument of claim 1 , wherein the proximal end of the shaft extends beyond the handle, thereby providing an impaction surface configured to be struck by a mallet.

6 . The surgical instrument of claim 1 , further comprising a stop configured to abut against a working channel, thereby preventing over penetration of the abrading head into the sacroiliac joint.

7 . The surgical instrument of claim 1 , wherein the handle is operatively connected to the shaft such that when the handle is moved relative to the shaft the handle causes a force to be exerted onto the abrading head to dislodge the abrading head from the sacroiliac joint.

8 . The surgical instrument of claim 7 , wherein the handle is configured to rotate relative to the shaft.

9 . The surgical instrument of claim 1 , wherein each of the pair of openings has an oblong shape.

10 . The surgical instrument of claim 9 , wherein the oblong shape has at least one rounded side.

11 . The surgical instrument of claim 1 , wherein at least some of the teeth or the barbs of the abrading surfaces have a square pyramidal shape.

12 . The surgical instrument of claim 1 , wherein the shaft is cylindrical.

13 . The surgical instrument of claim 1 , wherein the abrading surfaces are wider than the shaft.

14 . A method of using a surgical instrument to create an implant-receiving void within a sacroiliac joint, comprising the steps of:

receiving the surgical instrument comprising an elongated shaft having a distal end and a proximal end, wherein a handle is disposed at the proximal end of the elongated shaft and an abrading head is disposed at the distal end of the elongated shaft;

inserting the abrading head into the sacroiliac joint, wherein the abrading head comprises a pair of abrading surfaces disposed in opposing relation to one another and a pair of lateral sides disposed in opposing relation to one another, wherein the lateral sides connect the pair of abrading surfaces such that the lateral sides and the abrading surfaces collectively enclose an interior void, and wherein terminal edges of the abrading surfaces and the lateral sides collectively form a contiguous cutting edge having an oblong perimeter and configured to cut a cortical bone layer of sacrum and ilium bones within the sacroiliac joint, wherein each abrading surface has teeth, barbs, blades, ridges, slots, or broaches configured to abrade the sacrum and ilium bones, and wherein each of the pair of the lateral sides has an opening providing ingress into and egress from the interior void of the abrading head thereby enabling dislodged bone tissue to exit the interior void via the openings; and

impacting the proximal end of the shaft to advance the abrading head into the sacroiliac joint, wherein the contiguous cutting edge cuts into the cortical bone layer of the sacrum and ilium bones, and the teeth, barbs, blades, ridges, slots, or broaches abrade the sacrum and ilium bones, thereby creating the implant-receiving void.

15 . The method of claim 14 , wherein the proximal end of the shaft is impacted until a stop disposed on the shaft abuts against a working channel.

16 . The method of claim 14 , further comprising a step of moving the handle relative to the shaft to dislodge the abrading head from the sacroiliac joint.

17 . The method of claim 16 , wherein the step of moving the handle relative to the shaft comprises rotating the handle.

18 . The method of claim 14 , further comprising inserting a protrusion disposed on the shaft of the surgical instrument into a complementary groove, recess, channel, or indent of a working channel that provides access to the sacroiliac joint.

The method of claim 14 , further comprising a step of aligning a groove, recess, channel, or indent disposed on the shaft of the surgical instrument with a complementary a protrusion disposed on a working channel and advancing the shaft such that the protrusion enters into the groove, recess, channel, or indent, thereby preventing relative axial rotation of the surgical instrument relative to the working channel.

20 . The method of claim 14 , wherein the implant-receiving void is formed without a use of a rotary cutting instrument.
