Abstract

A pivotal bone anchor assembly includes a receiver having a first channel for receiving a rod and a central bore with a seating surface adjacent a bottom opening with a first cut-out portion. The assembly also includes a bone anchor having an upper end portion pivotably engageable with the seating surface, an anchor portion opposite the upper end portion, and a neck extending between the upper end and anchor portions. The assembly further includes an insert positionable in the central bore with a second channel alignable with the first channel, a concave lower surface engageable with the upper end portion of the bone anchor, and a second cut-out portion alignable with the first cut-out portion. The neck and at least a portion of the upper end portion of the bone anchor are positionable into the first cut-out portion and the second cut-out portion, respectively, to provide for increased pivotal angulation of the bone anchor with respect to the receiver in the direction of the cut-out portions.

Claims (39)

1 . A pivotal bone anchor assembly intended for securing an elongate rod to a bone of a patient, the pivotal bone anchor assembly comprising:

a receiver comprising a base defining a lower portion of a central bore centered about a vertical centerline axis and communicating with a bottom surface of the base through a partially circular bottom opening, and an upper portion with opposed internal surfaces extending between a front face and a back face of the receiver to define a first channel configured to receive the elongate rod, the central bore extending upward from the bottom opening through the first channel to a top of the receiver and including a guide and advancement structure adjacent the top of the receiver configured for engagement with a closure top and a lower seating surface adjacent the bottom opening, at least a portion of the bottom surface of the base extending perpendicular to the vertical centerline axis and the bottom opening including a first cut-out portion extending radially outward through the bottom surface of the base;

a bone anchor comprising a longitudinal axis, a shank body with a threaded anchor portion configured for attachment to the bone, an upper end portion opposite the threaded anchor portion, and a neck extending between the shank body and the upper end portion, the upper end portion having a rounded shape including a partially-spherical upper outer surface at a top end of the upper end portion and a partially-spherical lower outer surface transitioning downwardly and inwardly toward the neck, the upper end portion of the bone anchor being positionable into the lower portion of the central bore with the bone anchor extending downward through the bottom opening and with the lower outer surface configured for pivotal engagement with the lower seating surface of the base in an unlocked configuration prior to the pivotal bone anchor assembly being locked by the closure top, so as to provide for a first range of pivotal articulation of the bone anchor with respect to the receiver in a plurality of directions radial to the vertical centerline axis of the receiver; and

a compression insert positionable in the central bore of the receiver, the compression insert comprising an insert body and a pair of upstanding insert arms defining a second channel configured to receive the elongate rod, the second channel being alignable with the first channel when the compression insert is positioned in the receiver, the insert body having a through bore and a downwardly-facing concave lower surface configured to engage the partially-spherical upper outer surface of the bone anchor without engaging the partially-spherical lower outer surface when the longitudinal axis of the bone anchor is aligned with the vertical centerline axis of the receiver, the concave lower surface including a second cut-out portion alignable with the first cut-out portion,

wherein the neck and at least a portion of the upper end portion of the bone anchor are positionable in the first cut-out portion and the second cut-out portion, respectively, to provide for a second range of pivotal angulation of the bone anchor with respect to the receiver in the single direction of the cut-out portions radial to the vertical centerline axis of the receiver, the second range of pivotal articulation being greater than the first range of pivotal articulation.

2 . The pivotal bone anchor assembly of claim 1 , wherein the neck of the bone anchor includes a cylindrical surface portion receivable within the first cut-out portion.

3 . The pivotal bone anchor assembly of claim 1 , wherein the upper portion of the receiver further comprises a pair of upright receiver arms extending upward from the base to define the first channel.

4 . The pivotal bone anchor assembly of claim 1 , wherein the compression insert is downloadable into the central bore of the receiver from the top of the receiver.

5 . The pivotal bone anchor assembly of claim 1 ,

wherein the central bore of the receiver includes a discontinuous recess between the guide and advancement structure and the lower seating surface and the compression insert includes opposite engagement structures extending outward from upper ends of the upstanding insert arms in a direction transverse to the second channel, and

wherein the compression insert is configured to be loaded into the central bore of the receiver with the second channel mal-aligned with the first channel and then rotated about the vertical centerline axis so as to rotate the second channel into alignment with the first channel as the opposite engagement structures of the upstanding insert arms rotate into the discontinuous recess of the central bore.

The pivotal bone anchor assembly of claim 1 , wherein the concave lower surface of the compression insert is configured to engage the upper end portion of the bone anchor with a friction fit that inhibits pivotal movement of the bone anchor with respect to the receiver in an unlocked configuration prior to the pivotal bone anchor assembly being locked by the closure top.

The pivotal bone anchor assembly of claim 1 , wherein the upper end portion of the bone anchor further comprises a capture portion formed integral with the shank body of the bone anchor and a retaining structure configured for positioning in the lower portion of the central bore prior to uploading the capture portion through the bottom opening, the retaining structure configured to capture and hold the capture portion of the shank body within the receiver.

8 . The pivotal bone anchor assembly of claim 7 , wherein the retaining structure includes an interior surface configured to engage the capture portion of the shank body above the lower outer surface.

The pivotal bone anchor assembly of claim 8 , wherein the retaining structure includes a partially-spherical outer surface configured to align with the lower outer surface of the capture portion of the shank body and for pivotal engagement with the lower seating surface of the base in the unlocked configuration prior to the pivotal bone anchor assembly being locked by the closure top.

The pivotal bone anchor assembly of claim 1 and further comprising the elongate rod and the closure top, wherein the closure top is configured for positioning within the central bore of the receiver above the elongate rod and in engagement with the guide and advancement structure to apply a downward pressure to a top of the elongate rod, so as to secure the elongate rod to the bone of the patient in a locked configuration.

11 . The pivotal bone anchor assembly of claim 1 , wherein the second range of pivotal articulation is at least twice the first range of pivotal articulation.

12 . The pivotal bone anchor assembly of claim 1 , wherein the second range of pivotal articulation is at least about forty-two degrees.

13 . The pivotal bone anchor assembly of claim 12 , wherein the first range of pivotal articulation is at least about eighteen degrees.

14 . A method of assembling a pivotal bone anchor assembly configured to secure an elongate rod to a bone of a patient, the method comprising:

positioning an upper end portion of a bone anchor and a compression insert into a central bore of a receiver,

the receiver comprising a base defining a lower portion of the central bore centered about a vertical centerline axis and communicating with a bottom surface of the base through a partially circular bottom opening, and an upper portion with opposed internal surfaces extending between a front face and a back face of the receiver to define a first channel configured to receive the elongate rod, the central bore extending upward from the bottom opening through the first channel to a top of the receiver and including a guide and advancement structure adjacent the top of the receiver configured for engagement with a closure top and a lower seating surface adjacent the bottom opening, at least a portion of the bottom surface of the base extending perpendicular to the vertical centerline axis, and the bottom opening including a first cut-out portion extending radially outward through the bottom surface of the base,

the bone anchor comprising a longitudinal axis, a shank body with a threaded anchor portion configured for attachment to the bone, the upper end portion opposite the threaded anchor portion, and a neck extending between the shank body and the upper end portion, the upper end portion having a rounded shape including a partially-spherical upper outer surface at a top end of the upper end portion and a partially-spherical lower outer surface transitioning downwardly and inwardly toward the neck, and

the compression insert comprising an insert body and a pair of upstanding insert arms defining a second channel configured to receive the elongate rod, the second channel being alignable with the first channel when the compression insert is positioned in the receiver, the insert body having a through bore and a downwardly-facing concave lower surface configured to engage the partially-spherical upper outer surface of the bone anchor without engaging the partially-spherical lower outer surface when the longitudinal axis of the bone anchor is aligned with the vertical centerline axis of the receiver, the concave lower surface including a second cut-out portion alignable with the first cut-out portion,

wherein the upper end portion of the bone anchor is positionable into the lower portion of the central bore with the bone anchor extending downward through the bottom opening and with the lower outer surface configured for pivotal engagement with the lower seating surface of the base in an unlocked configuration prior to the pivotal bone anchor assembly being locked by the closure top, so as to provide for a first range of pivotal articulation of the bone anchor with respect to the receiver in a plurality of directions radial to the vertical centerline axis of the receiver, and

wherein the neck and at least a portion of the upper end portion of the bone anchor are positionable in the first cut-out portion and the second cut-out portion, respectively, to provide for a second range of pivotal angulation of the bone anchor with respect to the receiver in the single direction of the cut-out portions radial to the vertical centerline axis of the receiver, the second range of motion being greater than the first range of motion.

15 . The method of claim 14 , wherein the neck of the bone anchor includes a cylindrical surface portion receivable within the first cut-out portion.

16 . The method of claim 14 , wherein positioning the compression insert into the central bore of the receiver further comprises downloading the compression insert into the central bore from the top of the receiver.

17 . The method of claim 14 ,

wherein the central bore of the receiver includes a discontinuous recess between the guide and advancement structure and the lower seating surface and the compression insert includes opposite engagement structures extending outward from upper ends of the upstanding insert arms in a direction transverse to the second channel, and

wherein positioning the compression insert into the central bore of the receiver further comprises loading the compression insert into the central bore with the second channel mal-aligned with the first channel, and then rotating the compression insert about the vertical centerline axis so as to rotate the second channel into alignment with the first channel as the opposite engagement structures of the upstanding insert arms rotate into the discontinuous recess of the central bore.

The method of claim 14 , wherein the concave lower surface of the compression insert is configured to engage the upper end portion of the bone anchor with a friction fit that inhibits pivotal movement of the bone anchor with respect to the receiver in an unlocked configuration prior to the pivotal bone anchor assembly being locked by the closure top.

The method of claim 14 , wherein the upper end portion of the bone anchor further comprises a capture portion integral the shank body of the bone anchor and a retaining structure configured for positioning in the lower portion of the central bore prior to uploading the capture portion through the bottom opening, the retaining structure configured to capture and hold the capture portion of the shank body within the receiver.

20 . The method of claim 19 , wherein the retaining structure includes an interior surface configured to engage the capture portion of the shank body above the lower outer surface.

The method of claim 20 , wherein the retaining structure includes a partially-spherical outer surface configured to align with the lower outer surface of the capture portion of the shank body and for pivotal engagement with the lower seating surface of the base in the unlocked configuration prior to the pivotal bone anchor assembly being locked by the closure top.

The method of claim 14 and further comprising the elongate rod and the closure top, wherein the closure top is configured for positioning within the central bore of the receiver above the elongate rod and in engagement with the guide and advancement structure to apply a downward pressure to a top of the elongate rod, so as to secure the elongate rod to the bone of the patient in a locked configuration.

23 . The method of claim 14 , wherein the second range of pivotal articulation is at least twice the first range of pivotal articulation.

24 . The method of claim 14 , wherein the second range of pivotal articulation is at least about forty-two degrees.

25 . The method of claim 24 , wherein the first range of pivotal articulation is at least about eighteen degrees.
