Abstract

A method of applying a neural stimulus with an implanted electrode array involves applying a sequence of stimuli configured to yield a therapeutic effect while suppressing psychophysical side effects. The stimuli sequence is configured such that a first stimulus recruits a portion of the fibre population, and a second stimulus is delivered within the refractory period following the first stimulus and the second stimulus being configured to recruit a further portion of the fibre population. Using an electrode array and suitable relative timing of the stimuli, ascending or descending volleys of evoked responses can be selectively synchronised or desynchronised to give directional control over responses evoked.

Claims (33)

1 . A method of controlling delivery of neural stimuli to a neural pathway of a patient, the method comprising repeatedly:

applying a neural stimulus from an electrode array comprising at least one stimulus electrode to the neural pathway of a patient, the neural stimulus being defined by a stimulus parameter, and the electrode array being subject to movement relative to the neural pathway;

measuring a neural response evoked by the neural stimulus, the neural response comprising an evoked compound action potential;

estimating a threshold, being a minimum stimulus at which a neural response arises, using the measured neural response; and

setting the stimulus parameter to the threshold multiplied by a scaling factor.

2 . The method of claim 1 , wherein estimating the threshold comprises:

applying a second neural stimulus, the second neural stimulus being defined by a second stimulus parameter;

measuring a second neural response evoked by the second neural stimulus, the second neural response comprising a evoked compound action potential; and

estimating the threshold from the stimulus parameter, the measured neural response, the second stimulus parameter, and the second measured neural response.

3 . The method of claim 2 , wherein estimating the threshold comprises:

fitting a line to the stimulus parameter, the measured neural response, the second stimulus parameter, and the second measured neural response; and

estimating the threshold as the intercept of the fitted line.

4 . The method of claim 1 , wherein the scaling factor is determined from a measurement in a different posture.

5 . The method of claim 1 , wherein the stimulus parameter is a stimulus current of the neural stimulus.

6 . The method of claim 1 , wherein the measured neural response comprises a peak-to-peak amplitude of the evoked compound action potential.

7 . An implantable device for controllably delivering neural stimuli, the device comprising:

a stimulus source configured to deliver neural stimuli via one or more stimulus electrodes of an electrode array to a neural pathway, the electrode array being subject to movement relative to the neural pathway;

measurement circuitry configured to measure a neural response sensed at one or more sense electrodes of the electrode array; and

a control unit configured to:

control the stimulus source to deliver a neural stimulus defined by at least one stimulus parameter;

control the measurement circuitry to measure a neural response evoked by the neural stimulus, the neural response comprising an evoked compound action potential;

estimate a threshold, being a minimum stimulus at which a neural response arises, using the measured neural response; and

set the stimulus parameter to the threshold multiplied by a scaling factor.

8 . The device of claim 7 , wherein the control unit is configured to estimate the threshold by:

applying a second neural stimulus, the second neural stimulus being defined by a second stimulus parameter;

measuring a second neural response evoked by the second neural stimulus, the second neural response comprising a evoked compound action potential; and

estimating the threshold from the stimulus parameter, the measured neural response, the second stimulus parameter, and the second measured neural response.

9 . The device of claim 8 , wherein the control unit is configured to estimate the threshold by:

fitting a line to the stimulus parameter, the measured neural response, the second stimulus parameter, and the second measured neural response; and

estimating the threshold as the intercept of the fitted line.

10 . The device of claim 7 , wherein the scaling factor is determined from a measurement in a different posture.

11 . The device of claim 7 , wherein the stimulus parameter is a stimulus current of the neural stimulus.

12 . The device of claim 7 , wherein the measured neural response comprises a peak-to-peak amplitude of the evoked compound action potential.
