Abstract

A self-shielded and computer controlled system for performing non-invasive stereotactic radiosurgery and precision radiotherapy using a linear accelerator mounted within a two degree-of-freedom radiation shield coupled to a three-degree of freedom patient table is provided. The radiation shield can include an axial shield rotatable about an axial axis and an oblique shield independently rotatable about an oblique axis, thereby providing improved range of trajectories of the therapeutic and diagnostic radiation beams. Such shields can be balanced about their respective axes of rotation and about a common support structure to facilitate ease of movement. Such systems can further include an imaging system to accurately deliver radiation to the treatment target and automatically make corrections needed to maintain the anatomical target at the system isocenter. Various subsystems to automate controlled and coordinated movement of the movable shield components and operation of the treatment related subsystems to optimize performance and ensure safety are also provided.

72 . A radiosurgical treatment system, the system comprising:

a radiation shield defining an interior treatment space, wherein the radiation shield comprises a first shield component and a second shield component, wherein each of the first and second shield components are movable, the first shield components being rotatable about a first axis that intersects at an isocenter and the second shield component being rotatable about a second axis that intersects at the isocenter;

a diagnostic radiation beam emitter mounted within the first shield component and configured to direct a diagnostic radiation beam through the target to a diagnostic radiation beam detector mounted within the second shield component opposite the diagnostic radiation beam emitter; and

a control unit operably and communicatively coupled with the first and second shield components and a therapy radiation emitter mounted within the second shield component, wherein the control unit is configured for patient alignment and tracking based on the one or more images obtained from the diagnostic radiation beam detector.

73 . The system of claim 72 , wherein the control unit is further configured to control delivery of the therapy radiation beam from the therapy radiation emitter based on a diagnostic imaging result generated from the one or more images.

74 . The system of claim 73 , wherein the control unit is further configured to adjust a movable patient table supported the patient so as to align the target with the isocenter based on the diagnostic imaging result.

75 . The system of claim 72 , wherein the diagnostic radiation beam emitter is the sole imaging source of the system and is configured for transmitting kV radiation of diagnostic intensity.

76 . The system of claim 75 , wherein the diagnostic radiation beam detector is the sole imaging detector of the system.

77 . The system of claim 73 , wherein the control unit is configured to deliver the treatment radiation beam when the diagnostic imaging result indicates alignment of the target with the isocenter.

78 . The system of claim 73 , wherein the control unit is configured to generate the diagnostic imaging result from a first image obtained from the diagnostic radiation beam detector.

79 . The system of claim 78 , wherein the control unit is configured to generate the diagnostic imaging result by comparing and combining first and second images obtained from the diagnostic radiation beam detector, the first image obtained while the diagnostic radiation beam emitter is at a first position and the second image obtained while the diagnostic radiation beam emitter is at a second position.

80 . The system of claim 79 , wherein the first and second images are sequential views from different perspectives obtained by movement of the diagnostic radiation beam emitter.

81 . The system of claim 80 , wherein the control unit is further configured to obtain and utilize sequential views for initial patient alignment.

82 . The system of claim 81 , wherein the control unit is further configured to obtain and utilize additional sequential views for tracking of the target during therapeutic radiation delivery.

83 . The system of claim 78 , wherein the control unit is configured to generate the diagnostic imaging result by comparing and combining the first image with a digital reconstructed radiograph (DRR) image.

84 . The system of claim 83 , wherein the control unit is further configured to:

obtain the first image at a first position of the first shield component and correlate the first image with the DRR image to generate a 2D translation;

obtain a second image at a second position of the first shield component and correlate the second image with the DRR image to generate a second 2D translation;

generate a 3D translation result based on the 2D translations and compare with the second image; and

control delivery of the radiation therapy beam to the target based on the comparison of the 3D translation result with the second image.

85 . The system of claim 84 , wherein the control unit is configured to adjust a patient table supporting the patient based on the comparison of the 3D translation result with the second image to align the target with the isocenter.

86 . The system of claim 84 , wherein the control unit is configured to deliver the treatment radiation beam at the second position of the first shield component when the comparison of the 3D translation result indicates alignment of the target with the isocenter.

87 . The system of claim 84 , wherein comparing the 3D translation result with the second image includes generating 3D rotations when the first image corresponds to near alignment.

88 . The system of claim 73 , wherein the control unit is configured to generate the diagnostic image result by:

performing in-plane image enhancement and optimization transformation on each of a plurality of 2D images until near alignment is indicated; and

performing out-of-plane image enhancement and optimization transformation on a combined image when near alignment is indicated to generate the diagnostic imaging result.

89 . The system of claim 72 , wherein the first and second axes are transverse.

90 . A method of treating a patient, the method comprising:

rotating a first shield component of a radiation shield along a first axis that extends within an interior treatment space within the radiation shield and intersects at an isocenter, the first shield component having a diagnostic radiation beam emitter mounted within;

obtaining one or more diagnostic images from a diagnostic radiation detector mounted within a second shield component opposite the diagnostic radiation emitter, the second shield component being rotatable about a second axis that intersects at the isocenter;

generating a first diagnostic image result from one or more images obtained from the diagnostic radiation detector for patient alignment of the target with the isocenter and/or tracking of the target; and

delivering a therapeutic radiation beam from a therapeutic radiation emitter mounted within the second shield component to the isocenter based on the first diagnostic image.

91 . The method of claim 90 wherein the therapeutic radiation beam is delivered from a first direction corresponding to a first position when the first diagnostic image result corresponds to alignment of the target at the isocenter.

92 . The method of claim 91 further comprising:

rotating the second shield component of the radiation shield about the second axis as to position a therapeutic radiation emitter mounted within the second shield component to the first position.

93 . The method of claim 90 , wherein the diagnostic imaging result is generated from a first image obtained from the diagnostic radiation beam detector.

94 . The method of claim 90 , further comprising:

tracking the target during therapy by:

obtaining one or more additional images from the diagnostic radiation detector mounted within the second shield component and generating a second diagnostic image result, wherein the one or more additional images are sequential views;

rotating the second shield component about the second axis to position the therapeutic radiation emitter at a second position; and

delivering a therapeutic radiation beam from the therapeutic radiation emitter to the isocenter from a second direction corresponding to the second position when the second diagnostic image result corresponds to alignment of the target and the isocenter.

95 . The method of claim 94 , wherein the one or more additional images are sequential views obtained from the therapeutic radiation emitter.

96 . The method of claim 95 , wherein the sequential views are obtained at differing positions by moving the diagnostic radiation beam emitter between the differing positions by movement of the first shield component.

97 . The method of claim 96 , wherein the sequential views are utilized for initial patient alignment.

98 . The method of claim 97 , wherein additional sequential views are obtained and used for tracking of the target during therapeutic radiation delivery.

99 . The method of claim 90 , further comprising:

generating the diagnostic imaging result by comparing and combining a first image from the diagnostic radiation detector with a digital reconstructed radiograph (DRR) image.

100 . The method of claim 90 , wherein the diagnostic radiation beam emitter is the sole imaging source of the system and is configured for transmitting kV radiation of diagnostic intensity.

101 . The method of claim 100 , wherein the diagnostic radiation beam detector is the sole imaging detector of the system.

102 . The method of claim 90 , further comprising:

obtaining a first image at a first position of the first shield component and correlate the first image with the DRR image to generate a 2D translation;

obtaining a second image at a second position of the first shield component and correlate the second image with the DRR image to generate a second 2D translation;

generating a 3D translation result based on the 2D translations and compare with the second image; and

controlling delivery of the radiation therapy beam to the target based on the comparison of the 3D translation result with the second image.

103 . The method of claim 90 , wherein generating the first diagnostic image

result comprises:

performing in-plane image enhancement and optimization transformation on each of a plurality of 2D images until near alignment is indicated; and

performing out-of-plane image enhancement and optimization transformation on a combined image when near alignment is indicated to generate the diagnostic imaging result.

104 . The method of claim 90 , wherein the first and second axes are transverse.
