Abstract

A catheter may include electrodes for transvascular nerve stimulation. The electrodes may be positioned within lumens of the catheter and aligned with apertures in the outer wall of the catheter. The electrodes may produce focused electrical fields for stimulation of one or more nerves. In one embodiment, the catheter may include a set of proximal electrodes and a set of distal electrodes, and the proximal electrodes may stimulate a patient's left phrenic nerve and the distal electrodes may stimulate a patient's right phrenic nerve.

21 . A method for operating a medical device, the method comprising:

positioning the medical device within a subject, the medical device comprising:

a body comprising a longitudinal axis;

a plurality of electrodes positioned radially outward of the longitudinal axis;

a flexible member including a plurality of leads separated by insulating material;

an aperture corresponding to at least one electrode of the plurality of electrodes; and

a connection disposed in the aperture, wherein a lead of the plurality of leads is electrically connected to the at least one electrode of the plurality of electrodes through the connection; and

delivering a stimulation signal from the at least one electrode to a nerve.

22 . The method of claim 21 , wherein delivering the stimulation signal from the at least one electrode to the nerve activates a respiratory muscle.

23 . The method of claim 22 , wherein the respiratory muscle is a diaphragm.

24 . The method of claim 21 , wherein the at least one electrode is a first electrode, the nerve is a left phrenic nerve, and the method further comprises delivering a stimulation signal from a second electrode of the plurality of electrodes to a right phrenic nerve.

25 . The method of claim 21 , wherein positioning the medical device within the subject includes positioning the medical device within a blood vessel of the subject.

26 . The method of claim 21 , wherein each electrode of the plurality of electrodes is aligned with a corresponding aperture in a nonconductive material.

27 . The method of claim 21 , wherein the flexible member is affixed to the body.

28 . A method for operating a medical device, the method comprising:

positioning the medical device within a blood vessel of a subject, the medical device comprising:

a body defining a longitudinal lumen therein;

an electrode positioned radially outward of the longitudinal lumen;

a lead structure including a plurality of leads separated by insulating material;

an aperture in the insulating material corresponding to the electrode; and

a connection disposed in the aperture, wherein a lead of the plurality of leads is electrically connected to the electrode through the connection;

delivering a stimulation signal from the electrode to a nerve; and

transporting a fluid from the longitudinal lumen to tissue.

29 . The method of claim 28 , wherein transporting the fluid through the longitudinal lumen to tissue includes transporting the fluid to the blood vessel.

30 . The method of claim 28 , wherein a portion of the connection is radially outward of the lead and radially inward of the electrode.

31 . The method of claim 30 , wherein the portion is transverse to the longitudinal lumen

32 . The method of claim 28 , wherein the electrode is positioned on an exterior surface of the medical device.

33 . The method of claim 28 , wherein the longitudinal lumen is a first lumen, and the body further defines a second lumen comprising a pressure sensor.

34 . The method of claim 28 , wherein positioning the device within the blood vessel includes positioning the device such that the electrode is in a jugular vein, a subclavian vein, or a superior vena cava.

35 . The method of claim 28 , further comprising transporting a fluid from the blood vessel to the longitudinal lumen.

36 . A method for operating a medical device, the method comprising:

inserting the medical device into a subject, the medical device comprising:

a body defining a longitudinal lumen therein;

a first electrode positioned radially outward of the lumen;

a second electrode positioned radially outward of the lumen;

a flexible member including a plurality of leads separated by insulating material;

a first aperture corresponding to the first electrode;

a second aperture corresponding to the second electrode;

a first connection disposed in the first aperture, wherein a first lead of the plurality of leads is electrically connected to the first electrode through the first connection; and

a second connection disposed in the second aperture, wherein a second lead of the plurality of leads is electrically connected to the second electrode through the second connection;

delivering a first stimulation signal from the first electrode to a left phrenic nerve or a right phrenic nerve;

delivering a second stimulation signal from the second electrode to the left phrenic nerve or the right phrenic nerve.

37 . The method of claim 36 , wherein the first stimulation signal is delivered to the left phrenic nerve, and the second stimulation signal is delivered to the right phrenic nerve.

38 . The method of claim 36 , wherein the first electrode is at a radial position that is different than a radial position of the second electrode.

39 . The method of claim 36 , wherein the first stimulation signal is delivered from only a portion of a circumference of the medical device.

40 . The method of claim 39 , wherein the portion is a first portion, the circumference is a first circumference, and the second stimulation signal is delivered from only a second portion of a second circumference of the medical device, and where the first portion is at a radial position different that a radial position of the second portion.
