Abstract

The present disclosure relates to methods of treating a patient that has a low coronary ostia height. The methods can involve advancing a heart valve prosthesis to a native valve structure having an ostium and positioning the prosthesis to align a gap between major peaks of the prosthesis with the ostium to avoid obstruction of the ostium. The presently disclosed methods and prostheses can be effective at treating patients with a coronary ostia height of as low as 10 mm, 8 mm, 6 mm, or less.

Claims (20)

1 . A transcatheter aortic valve replacement procedure comprising inserting a heart valve prosthesis into an aortic root such that a circumferential gap between major peaks of the prosthesis is rotationally aligned relative to the aortic root to avoid obstruction of a coronary ostium.

2 . The procedure of claim 1 , wherein a minor peak of the prosthesis is located longitudinally intermediate a bottom edge and the major peaks.

3 . The procedure of claim 1 , wherein a minor peak of the prosthesis is located at a lower axial distance relative to the coronary ostium.

4 . The procedure of claim 1 , wherein the coronary ostium has a height less than 10 mm.

5 . The procedure of claim 1 , wherein the coronary ostium has a height less than 8 mm.

6 . The procedure of claim 1 , wherein the coronary ostium has a height less than 6 mm.

7 . The procedure of claim 1 , wherein the circumferential gap is formed along an upper edge of a frame of the heart valve prosthesis.

8 . The procedure of claim 7 , wherein the circumferential gap is formed by opposing struts of the frame.

9 . The procedure of claim 7 , wherein the circumferential gap comprises a minor peak formed along the upper edge of the frame between the major peaks.

9 . procedure of claim 9 , wherein the minor peak is formed by an apex of a diamond shape.

11 . The procedure of claim 1 , wherein inserting the heart valve prosthesis into the aortic root comprises inserting the heart valve prosthesis within an existing prosthesis for a valve- in-valve procedure.

12 . The procedure of claim 1 , wherein inserting the heart valve prosthesis into the aortic root comprises transfemorally inserting the heart valve prosthesis into the aortic root.

13 . The procedure of claim 1 , wherein inserting the heart valve prosthesis into the aortic root comprises apposing the prosthesis against native tissue of the aortic root.

14 . The procedure of claim 1 , further comprising positioning at least a portion of the heart valve prosthesis below a valve annulus.

15 . A heart valve replacement procedure comprising rotating at least a portion of a heart valve prosthesis within an aortic root to position the prosthesis within the aortic root while rotationally aligning a circumferential window of the prosthesis with a coronary ostium to permit blood flow to the coronary ostium.

16 . The procedure of claim 15 , wherein rotationally aligning the circumferential window of the prosthesis with the coronary ostium comprises positioning the circumferential window at a lower height relative to the coronary ostium.

17 . The procedure of claim 15 , wherein rotationally aligning the circumferential window of the prosthesis with the coronary ostium comprises permitting blood flow to the coronary ostium at a height of less than 10 mm.

18 . The procedure of claim 15 , wherein rotationally aligning the circumferential window of the prosthesis with the coronary ostium comprises permitting blood flow to the coronary ostium at a height of less than 8 mm.

19 . The procedure of claim 15 , wherein rotationally aligning the circumferential window of the prosthesis with the coronary ostium comprises permitting blood flow to the coronary ostium at a height of less than 6 mm.

20 . The procedure of claim 15 , wherein rotating at least a portion of the heart valve prosthesis within the aortic root comprises rotating at least a portion of the heart valve prosthesis within an existing prosthesis for a valve-in-valve procedure.
