Abstract

This disclosure provides mixtures of beta-cyclodextrin molecules substituted at one or more hydroxyl positions by hydroxypropyl groups, the mixture optionally including unsubstituted beta-cyclodextrin molecules, for use as a pharmaceutically active ingredient; methods of making such mixtures; methods of qualifying such mixtures for use in a pharmaceutical composition suitable for intrathecal or intracerebroventricular administration; pharmaceutical compositions suitable for intrathecal or intracerebroventricular administration comprising such mixtures; and methods of using the pharmaceutical compositions for treatment of Niemann-Pick disease Type C.

Claims (21)

1. A method of treating Niemann-Pick disease comprising intrathecally administering to a subject in need thereof a composition comprising an initial dose of 200 mg of a mixture of beta-cyclodextrin molecules substituted at one or more hydroxyl positions by hydroxypropyl groups, and the composition comprises less than 1 International Units (IU) of endotoxins per gram.

2. The method of claim 1 , wherein the administering occurs every week.

3. The method of claim 1 , wherein the administering occurs every two weeks.

4. The method of claim 1 , wherein the administering occurs every month.

5. The method of claim 1 , further comprising assessing safety with an audiologic evaluation of the subject to monitor for ototoxicity.

6. The method of claim 1 , further comprising administering the composition at an escalated dose from 200 mg.

7. The method of claim 1 , wherein the Niemann-Pick disease is early-infantile onset.

8. The method of claim 1 , wherein the Niemann-Pick disease is late-infantile onset.

9. The method of claim 1 , wherein the Niemann-Pick disease is juvenile onset.

10. The method of claim 1 , wherein the Niemann-Pick disease is adolescent/adult onset.

11. The method of claim 1 , further comprising administering the composition at a dose of 300 mg, 400 mg, 500 mg, 600 mg, 700 mg, 800 mg, or 900 mg.

12. The method of claim 1 , further comprising administering the composition at a dose of 250 mg, 350 mg, 450 mg, 550 mg, 650 mg, 750 mg, or 850 mg.

13. The method of claim 1 , wherein the subject is a drug-naïve patient.

14. The method of claim 1 , wherein the composition comprises less than 0.5 International Units (IU) of endotoxins per gram.

15. The method of claim 1 , wherein the composition comprises less than 0.3 International Units (IU) of endotoxins per gram.

16. The method of claim 1 , wherein the composition comprises less than 0.2 International Units (IU) of endotoxins per gram.

17. The method of claim 1 , wherein the composition comprises less than 0.1 International Units (IU) of endotoxins per gram.

18. The method of claim 1 , wherein the composition comprises less than 0.05 International Units (IU) of endotoxins per gram.

19. The method of claim 1 , wherein the composition comprises no more than 0.5% propylene glycol, and less than 1 ppm of propylene oxide.

20. The method of claim 1 , wherein the mixture comprises less than 1% unsubstituted beta-cyclodextrin (“DS-0”).

21. The method of claim 1 , wherein the mixture comprises an average molar substitution (“MS”) in the range of 0.58-0.68.
