Abstract

Endolumenal devices and methods can be used for the treatment of health conditions including obesity and diabetes. In some embodiments, the methods and systems provided herein can cause weight loss or control diabetes by reducing the caloric absorption of an individual. For example, this document provides several devices and methods for treating obesity and diabetes by using electroporation to modulate the duodenal mucosa. In addition, this document provides devices and methods for bypassing portions of the gastrointestinal tract to reduce nutritional uptake.

Claims (42)

1 . A method of treating diabetes and/or obesity in a patient with non-thermal electroporation, comprising:

advancing an electroporation device comprising an expandable member comprising one or more electrodes into a duodenum of the patient, the duodenum comprising a target tissue including at least a mucosa layer and a non-target muscularis layer;

expanding the expandable member to place the one or more electrodes in contact with the target tissue;

energizing the one or more electrodes with a therapeutically beneficial amount of non-thermal electroporation energy; and

delivering the therapeutically beneficial amount of non-thermal electroporation energy from the one or more electrodes to the target tissue; and

treating cells within the target tissue with the delivered non-thermal electroporation energy without permanently damaging cells in the non-target muscularis layer.

2 . The method of claim 1 further comprising transitioning the expandable member from a

contracted configuration to an expanded configuration.

3 . The method of claim 2 , wherein transitioning the expandable member to the expanded configuration stretches at least a portion of the duodenum.

4 . The method of claim 1 , wherein the non-thermal electroporation energy comprises pulses of current.

5 . The method of claim 1 , wherein the one or more electrodes comprise bipolar and/or monopolar electrodes.

6 . The method of claim 1 , wherein the one or more electrodes comprise a plurality of spaced-apart electrodes.

7 . The method of claim 1 , wherein the expandable member comprises an inflatable balloon.

8 . The method of claim 7 , wherein the inflatable balloon comprises one or more apertures in fluid communication with an interior of the inflatable balloon, and a fluid within the interior of the inflatable balloon; and

delivering the fluid into an interior space of the duodenum through the one or more apertures.

9 . The method of claim 8 , wherein the fluid is circulated in an interior space of the duodenum while with the non-thermal electroporation energy is delivered, and wherein the fluid is selected from one or more of the group consisting of: a hydrogel; an electrically conductive liquid; and a drug.

10 . The method of claim 19 , wherein delivering the fluid comprises pheresing the drug to cells within the target tissue of the duodenum.

11 . The method of claim 1 , wherein the non-thermal electroporation energy treats at least a portion of the entirety of a circumferential portion of the target tissue of the duodenum.

12 . The method of claim 1 , wherein the non-thermal electroporation energy treats cells in diseased target tissue.

13 . The method of claim 1 , wherein the non-thermal electroporation energy increases a level of a gut hormone important in appetite regulation, and/or increases a level of a gut hormone important in insulin secretion.

14 . The method of claim 1 , wherein the non-thermal electroporation energy reshapes the mucosa of the duodenum.

15 . The method of claim 1 , wherein at least a portion of the non-thermal electroporation energy is delivered to crypts of the mucosa layer.

16 . The method of claim 1 , wherein the one or more electrodes comprise DC electrodes.

17 . The method of claim 1 , wherein the non-thermal electroporation energy comprises AC energy.

18 . The method of claim 1 , wherein the non-thermal electroporation energy comprises irreversible electroporation energy.

19 . An electroporation device configured to treat diabetes and/or obesity in at least a target tissue of a patient's duodenum including at least a mucosa layer while not treating or damaging a non-target tissue including the duodenal muscularis layer, comprising:

a shaft;

an expandable member surrounding a distal portion of the shaft;

one or more electrodes located on the expandable member;

an electroporation energy source in operative communication and connection with the one or more electrodes, configured to deliver pulsed non-thermal electroporation energy to the one or more electrodes and to the target tissue,

wherein the pulsed non-thermal electroporation energy is delivered from the one or more electrodes to the target tissue to treat cells within the target tissue,

wherein the delivered pulsed non-thermal electroporation energy is configured to not permanently damage the non-target tissue, and

wherein the delivered pulsed non-thermal electroporation energy is configured to treat the target tissue without heat.

20 . The electroporation device of claim 19 , wherein the one or more electrodes comprise bipolar electrodes.

21 . The electroporation device of claim 19 , wherein the one or more electrodes comprise a plurality of spaced-apart electrodes.

22 . The electroporation device of claim 19 , wherein the expandable member comprises an inflatable balloon.

23 . The electroporation device of claim 22 , wherein the expandable member is configured to transition from a first configuration to a second configuration, wherein transitioning the expandable member to the second configuration stretches at least a portion of the target tissue.

24 . The electroporation device of claim 1 , wherein the pulsed non-thermal electroporation comprises pulses of current.

25 . The electroporation device of claim 1 , wherein the delivered pulsed non-thermal electroporation energy is configured to treat cells within the target tissue, wherein the target tissue is diseased.

26 . The electroporation device of claim 1 , wherein the delivered pulsed non-thermal electroporation energy is configured to reshape the target tissue.

27 . The electroporation device of claim 1 , wherein the delivered pulsed non-thermal electroporation energy is configured to increase a level of a gut hormone important in insulin secretion, and/or increase a level of a gut hormone important in appetite regulation.

28 . The electroporation device of claim 1 , wherein the pulsed non-thermal electroporation energy comprises irreversible electroporation energy.
