Abstract

The present invention relates generally to improved systems and methods for removing undesirable material residing in vessels. More specifically, the present invention relates to systems and methods for using at least one cannula to remove substantially en bloc, from a site of obstruction or interest, undesirable material without fragmentation and without excessive fluid loss.

21 . A method for removing an undesirable material from a treatment site within a vasculature, the method comprising:

placing an aspiration cannula and an outer sheath near the treatment site, wherein the aspiration cannula comprises an expandable funnel coupled to a distal end of the aspiration cannula;

expanding the funnel within the vasculature, wherein the funnel comprises a unitary metallic reinforcement structure and a polymer wall, the unitary metallic reinforcement structure comprising a metallic proximal ring configured to be operably coupled to an outer surface of the cannula distal end, and a metallic expansion section extending from the metallic proximal ring, the metallic expansion section comprising a shape memory material and is configured to automatically expand the funnel when released from constraint of the outer sheath;

activating a pump configured to be in operable fluid communication with the aspiration cannula to generate a suction force through a lumen of the aspiration cannula, wherein the suction force is sufficient to aspirate at least a part of the undesirable material and blood from the vasculature, wherein the metallic expansion section is configured to resist collapse from the suction force; and

retracting the funnel into the outer sheath, wherein the metallic expansion section is configured to permit radial compression of the funnel when the funnel is retracted into the outer sheath.

The method of claim 21 , wherein the undesirable material comprises a pulmonary embolism and the vasculature comprises a pulmonary artery; and wherein the placing the aspiration cannula and the outer sheath near the treatment site further comprises navigating the aspiration cannula and the outer sheath through at least one heart chamber.

23 . The method of claim 22 , wherein the funnel further comprises a radiopaque marker, and the method further comprises:

imaging the aspiration cannula; and

confirming the aspiration cannula is properly placed near the treatment site by visualizing the radiopaque marker on the imaging before activating the pump.

24 . The method of claim 23 , further comprising:

driving the aspirated pulmonary embolism and the aspirated blood through a filter assembly, thereby producing filtered blood; and

visually confirming that the filtered pulmonary embolism is captured in a reservoir.

25 . The method of claim 21 , further comprising:

activating a secondary device to aid in the removal of the undesirable material.

26 . The method of claim 25 , wherein the secondary device comprises a mechanical thrombectomy device, a fluid thrombectomy device, an expandable compliant balloon catheter, or a non-complaint balloon catheter.

27 . The method of claim 26 , wherein the secondary device is configured to dissolve, dislodge, or macerate the undesirable material, and wherein the activation of the secondary device and the activation of the pump are configured to be a combination therapy.

28 . The method of claim 21 , further comprising:

repositioning the aspiration cannula and the outer sheath to a second treatment site within the vasculature;

expanding the funnel near the second treatment site;

activating the pump to generate the suction force when the funnel is near the second treatment site; and

retracting the funnel into the outer sheath near the second treatment site.

29 . The method of claim 24 , further comprising:

placing a reinfusion cannula in the vasculature;

reinfusing the filtered blood through the reinfusion cannula into the vasculature.

30 . A method for removal of a pulmonary embolism from within a pulmonary artery, the method comprising:

placing an aspiration cannula and an outer sheath in the pulmonary artery, wherein the aspiration cannula comprises an expandable funnel;

advancing the expandable funnel beyond a distal most end of the outer sheath, wherein the expandable funnel comprises a unitary metallic reinforcement structure and a polymer jacket, the unitary metallic reinforcement structure comprising a metallic proximal ring configured to couple to a cannula distal end, and a metallic expansion section extending from the metallic proximal ring, the metallic expansion section comprising a nitinol material pre-shaped to automatically expand the expandable funnel when the expandable funnel is advanced beyond the distal most end of the outer sheath;

activating a pump configured to be in operable fluid communication with the aspiration cannula to apply a suction force through a lumen of the aspiration cannula, wherein the suction force is sufficient to aspirate at least a part of the pulmonary embolism and blood from the pulmonary artery, and wherein the metallic expansion part is configured to resist collapse from the applied suction force;

activating a secondary device configured to aid in the removal of the pulmonary embolism from the pulmonary artery; and

retracting the expandable funnel into the outer sheath, wherein the metallic expansion part is configured to permit the expandable funnel to transition to a collapsed state when the expandable funnel is retracted into the outer sheath.

31 . The method of claim 30 , further comprising:

flowing the aspirated pulmonary embolism and the aspirated blood through a filter assembly to produce filtered blood;

placing a reinfusion cannula in a vasculature; and

reinfusing the filtered blood through the reinfusion cannula.

32 . The method of claim 30 , wherein at least one of the aspiration cannula or the outer sheath comprises a reinforcement member configured to provide sufficient torque to steer or navigate the aspiration cannula through a heart chamber and the pulmonary artery.

33 . The method of claim 30 , wherein the secondary device comprises a mechanical thrombectomy device, a fluid thrombectomy device, an expandable compliant balloon catheter, or a non-complaint expandable balloon catheter.

34 . The method of claim 30 , wherein the metallic proximal ring is configured to be coupled to the aspiration cannula distal end by a lap joint covered by a polymer material.

35 . A method for removal of an undesirable material from a treatment site within a vasculature, the method comprising:

placing an aspiration cannula and an outer sheath near the treatment site, wherein the aspiration cannula is operably coupled to an expandable funnel comprising a radiopaque marker;

imaging the expandable funnel in the expanded state within the vasculature;

confirming the image shows the radiopaque marker of the expandable funnel is near the treatment site;

expanding the expandable funnel from a collapsed state to an expanded state near the treatment site, wherein the expandable funnel comprises a unitary metallic reinforcement member and a polymer wall, the unitary metallic reinforcement member comprising a metallic proximal collar configured to couple to a distal end of the aspiration cannula, and an expandable metallic member extending from the metallic proximal collar, the expandable metallic member being pre-shaped in an expanded configuration to automatically expand the expandable funnel when released from constraint of the outer sheath;

activating the pump to generate a suction force through a lumen of the aspiration cannula, wherein the suction force is sufficient to aspirate at least a part of the undesirable material and blood from the vasculature, wherein the expandable metallic member is configured to resist collapse from the suction force;

retracting the aspiration cannula into the outer sheath, wherein the expandable metallic member is configured to permit radial compression of the expandable funnel when the aspiration cannula is retracted into the outer sheath.

36 . The method of claim 35 , further comprising:

activating a mechanical thrombectomy device; or

activating a fluid thrombectomy device.

37 . The method of claim 35 , wherein the aspiration cannula lumen comprises a first coefficient of friction, and an inner surface of the expandable funnel comprises a second coefficient of friction that is different from the first coefficient of friction.

38 . The method of claim 35 , further comprising:

filtering the aspirated undesirable material from the aspirated blood;

placing a reinfusion cannula in the vasculature; and

reinfusing the filtered blood to the vasculature through the reinfusion cannula.

39 . The method of claim 35 , wherein the imaging comprises imaging via fluoroscopy or echocardiography.

40 . The method of claim 35 , wherein the expandable metallic member is configured to be stackable during the radial compression of the expandable funnel.
