Methods of distracting tissue layers of the human spine
Methods of separating and/or supporting layers of tissue in the human spine. Such methods generally comprise inserting at least a distal end portion of a guide member between layers of tissue in the human spine wherein the distal end portion defines a predetermined shape. A distraction device is advanced over at least the distal end portion of the guide member so that the distraction device substantially assumes the predetermined shape of the distal end portion of the guide member to form a support structure between the tissue layers that separates and/or supports the tissue layers.
1. A method of treating a human vertebral body having an interior region defined by a superior endplate, an inferior endplate and a cortical rim, and containing cancellous bone therewithin, the method comprising:
inserting a distal end portion of a guide member into an interior region of a vertebral body;
forming the distal end portion of the guide member into a configuration that circumferentially surrounds a volume of cancellous bone within the vertebral body;
advancing an elongated intravertebral implant distally along the distal end portion of the guide member in the interior region of the vertebral body, the implant conforming to the shape of the distal end portion of the guide member to form a structure that surrounds the volume of cancellous bone within the vertebral body and that separates and/or supports the superior and inferior endplates.
2. The method of claim 1 further including deforming the distal end portion of the guide member from an initial shape to a deformed shape and the distal end portion of the guide member is inserted into the interior region of the vertebral body in the deformed shape; and
wherein forming the distal end portion of the guide member into the configuration that surrounds a volume of cancellous bone comprises forming the distal end portion into the initial shape.
3. The method of claim 2 in which the deformed shape of the distal end portion of the guide member is substantially linear.
4. The method of claim 2 in which the initial configuration of the distal end portion of the guide member comprises a plurality of adjacent windings.
5. The method of claim 1 in which forming the distal end portion of the guide member into the configuration that surrounds a volume of cancellous bone comprises self-forming of the distal end portion into the configuration.
6. The method of claim 1 in which at least the distal end portion of the guide member is comprised of shape memory material.
7. The method of claim 1 further including removing the elongated guide member from the structure.
8. The method of claim 1 in which the elongated implant has a proximal end, a distal end and a longitudinal passageway therethrough, wherein the passageway is configured to receive the guide member for advancement of the elongated implant along the guide member.
9. A method of treating a human vertebral body having an interior region defined by a superior endplate, an inferior endplate and a cortical rim, and containing cancellous bone therewithin, the method comprising:
inserting a distal end portion of a delivery device into an interior region of a vertebral body;
advancing an elongated guide member through the delivery device so that a distal end portion of the guide member extends out of an aperture in the distal end portion of the delivery device and into the interior region of the vertebral body while a more proximal portion of the guide member resides within the delivery device;
forming the distal end portion of the guide member, in the interior region of the vertebral body, into an arcuate configuration that extends at least 360 degrees;
advancing an elongated intravertebral implant along the proximal end portion of the guide member within the delivery device; and
advancing the intravertebral implant out of the distal end portion of the delivery device into the interior region of the vertebral body and along the distal end portion of the guide member, the intravertebral implant conforming to the arcuate configuration of the distal end portion of the guide member to form an arcuate structure that extends at least 360 degrees and separates and/or supports the endplates of the vertebral body.
10. The method of claim 9 in which the distal end portion of the guide member has an initial shape wherein the distal end portion of the guide member is in the arcuate configuration;
the method further comprising:
deforming the distal end portion of the guide member from the initial shape to a deformed shape;
advancing the guide member through the delivery device in the deformed shape; and
wherein forming the distal end portion of the guide member into the arcuate configuration comprises forming the distal end portion of the guide member from the deformed shape back into the initial shape.
11. The method of claim 10 in which the deformed shape of the distal end portion of the guide member is substantially linear.
12. The method of claim 10 in which the distal end portion of the guide member is deformed by the delivery device constraining the distal end portion of the guide member into the deformed shape.
13. The method of claim 9 in which forming the distal end portion into the arcuate configuration, in the interior region of the vertebral body, comprises the distal end portion self-forming into the arcuate configuration.
14. The method of claim 9 further including removing the elongated guide member from the structure.
15. The method of claim 9 in which the elongated implant has a proximal end, a distal end and a longitudinal passageway therethrough, wherein the passageway is configured to receive the guide member for advancement of the elongated implant along the guide member.
16. A method of treating a human vertebral body having an interior region defined by a superior endplate, an inferior endplate and a cortical rim, and containing cancellous bone therewithin, the method comprising:
inserting at least a distal end portion of an elongated guide member in an insertion configuration into an interior region of a vertebral body;
changing the configuration of the distal end portion of the guide member into an inserted configuration comprising a plurality of generally horizontally-disposed windings within the interior region of the vertebral body; and
advancing an elongated intravertebral implant distally along the distal end portion of the guide member, the implant being independently movable longitudinally relative to the guide member and sufficiently flexible to conform to the inserted configuration of the distal end portion of the guide member so that the implant forms a structure comprising a plurality of generally horizontally-disposed windings that extends vertically within the interior region of the vertebral body.
17. The method of claim 16 wherein the distal end portion of the elongated guide member is substantially linear in the insertion configuration.
18. The method of claim 16 in which changing the configuration of the distal end portion of the guide member into the inserted configuration results from a self-forming of the distal end portion of the guide member into the inserted configuration.
19. The method of claim 16 in which at least one winding of the support structure has at least one adjacent winding thereabove or therebelow and the at least one winding being in contact with the adjacent winding.
20. The method of claim 16 in which the windings of the structure extend in a generally circular configuration.
