Abstract

Methods and apparatus for treatment of patent foramen ovale (PFO) provide for applying energy to tissues adjacent the PFO with a catheter device to substantially close the PFO acutely. Apparatus generally includes a catheter device having at least one energy transmission member at or near its distal end configured to apply energy to PFO tissues to acutely, substantially close the PFO. Applied energy may be monopolar or bipolar radiofrequency energy or any other suitable energy, such as laser, microwave, ultrasound, resistive heating or the like. Some embodiments of a catheter device further include one or more tissue apposition members near the distal end for helping bring PFO tissues together, such as a PFO covering member, a vacuum applying member and/or the like. PFO closure via energy-based approaches of the invention may help prevent stroke, treat migraine headache, and possibly treat or prevent other medical conditions.

Claims (35)

1. A method of treating a patent foramen ovale (PFO) having a right side and a left side in a heart of a patient, the method comprising:

positioning a distal portion of an elongate catheter device proximate to tissues of a PFO as to bring the tissues together; and

applying about 0.5 Watts to about 100 Watts of energy to the tissues with the catheter device to substantially close the PFO,

wherein the energy is applied only from the right side of the heart.

2. The method of claim 1 , wherein the applying comprises applying about 25 Watts of energy.

3. The method of claim 1 , wherein applying the energy comprises an application time of at least 30 seconds.

4. The method of claim 1 , wherein applying the energy comprises an application time of about 15 minutes.

5. The method of claim 1 , wherein applying the energy comprises an application time of about 0.5 seconds to about 15 minutes.

6. The method of claim 1 , wherein applying the energy comprises an application time of about 30 seconds to about 5 minutes.

7. The method of claim 1 , wherein applying the energy comprises an application time of about 7 minutes.

8. The method of claim 1 , further comprising determining when a sufficient amount of energy has been applied to the tissues to close the PFO based upon temperature.

9. The method of claim 1 , further comprising determining when a sufficient amount of energy has been applied to the tissues to close the PFO based upon impedance of the tissues.

10. A method as in claim 1 , wherein discontinuing application of energy results in cooling of the tissues.

11. A method as in claim 1 , further comprising monitoring an amount of energy applied to the tissues.

12. A method as in claim 1 , further comprising monitoring the temperature of the tissues.

13. A method as in claim 1 , further comprising determining when a sufficient amount of energy has been applied to the tissues to close the patent foramen ovale.

14. A method as in claim 13 , further comprising determining when a sufficient amount of energy has been applied to the tissues to close the patent foramen ovale based the amount of energy applied to the tissues.

15. A method as in claim 13 , further comprising discontinuing the application of energy when the sufficient amount of energy has been applied.

16. A method of treating a patent foramen ovale (PFO) having a right side and a left side in a heart of a patient, the method comprising:

positioning a distal portion of an elongate catheter device proximate to tissues of a PFO as to bring the tissues together; and

applying about 2 Watts to about 20 Watts of energy to the tissues with the catheter device to substantially close the PFO,

wherein the energy is applied only from the right side of the heart.

17. The method of claim 16 , wherein applying the energy comprises an application time of at least 30 seconds.

18. The method of claim 16 , wherein applying the energy comprises an application time of about 15 minutes.

19. The method of claim 16 , wherein applying the energy comprises an application time of about 0.5 seconds to about 15 minutes.

20. The method of claim 16 , wherein applying the energy comprises an application time of about 30 seconds to about 5 minutes.

21. The method of claim 16 , wherein applying the energy comprises an application time of about 7 minutes.

22. The method of claim 16 , further comprising determining when a sufficient amount of energy has been applied to the tissues to close the PFO based upon temperature.

23. The method of claim 16 , further comprising determining when a sufficient amount of energy has been applied to the tissues to close the PFO based upon impedance of the tissues.

24. A method as in claim 16 , wherein discontinuing application of energy results in cooling of the tissues.

25. A method as in claim 16 , further comprising monitoring an amount of energy applied to the tissues.

26. A method as in claim 16 , further comprising monitoring the temperature of the tissues.

27. A method as in claim 16 , further comprising determining when a sufficient amount of energy has been applied to the tissues to close the patent foramen ovale.

28. A method as in claim 27 , further comprising determining when a sufficient amount of energy has been applied to the tissues to close the patent foramen ovale based the amount of energy applied to the tissues.

29. A method as in claim 27 , further comprising discontinuing the application of energy when the sufficient amount of energy has been applied.
