Abstract

Implantable devices and methods for delivery of lidocaine or other drugs to a patient are provided. In one embodiment, the device includes a first drug portion which has a first drug housing which contains a first drug formulation in a solid form which includes a pharmaceutically acceptable salt of lidocaine; and a second drug portion which includes a second drug housing which contains a second drug formulation which includes lidocaine base. In another embodiment, the device includes a drug reservoir component which has an elastic tube having at least one lumen bounded by a porous sidewall having an open-cell structure, a closed-cell structure, or a combination thereof; and a drug formulation contained within the at least one lumen, wherein the device is deformable between a low-profile deployment shape and a relatively expanded retention shape.

Claims (32)

1. An implantable medical device for controlled drug delivery device comprising:

a first drug portion that comprises a first drug housing loaded with a first drug formulation in a solid or semi-solid form which comprises a first drug which has a high solubility, wherein the first drug housing comprises a first wall that is permeable to water but is substantially impermeable to the first drug and has at least one aperture through the first wall, the first drug portion being configured to release the first drug from the first drug housing through the at least one aperture primarily via osmotic pressure in vivo according to a first release profile; and

a second drug portion that comprises a second drug housing loaded with a second drug formulation in a solid or semi-solid form which comprises a second drug which has a low solubility, wherein the second drug housing comprises a second wall that is permeable to water and to the second drug, the second drug portion being configured to release the second drug from the second drug housing by diffusion through the second wall in vivo according to a second release profile; and

a retention portion operably associated with both the first and second drug portions.

2. The device of claim 1 , wherein:

the first drug is in a salt form with high solubility in water; and

the second drug is the same drug as the first drug but is in non-salt form with low solubility in water.

3. The device of claim 1 , wherein the first and second drug housings are partitioned regions of a single tubular body.

4. The device of claim 3 , wherein at least one of the drug formulations comprises a plurality of solid drug tablets aligned within the single tubular body in a row.

5. The device of claim 1 , wherein the retention portion comprises a retention frame and the device is deformable between a retention shape and a deployment shape.

6. The device of claim 1 , wherein the second drug formulation comprises lidocaine base.

7. The device of claim 1 , wherein the first drug formulation comprises a lidocaine salt.

8. The device of claim 1 , wherein the first drug formulation comprises a lidocaine salt and the second drug formulation comprises lidocaine base.

9. The device of claim 1 , wherein the device is elastically deformable between a deployment shape permitting deployment of the device through a patient's urethra and a retention shape for retaining the deployed device within the patient's bladder.

10. The device of claim 1 , wherein the second release profile differs from the first release profile.

11. An implantable drug delivery device comprising:

a first drug portion that comprises a first drug housing loaded with a first drug formulation which comprises a first drug, wherein the first drug housing comprises a first wall that is permeable to water but is substantially impermeable to the first drug and has at least one aperture through the first wall, the first drug portion being configured to release the first drug from the first drug housing through the at least one aperture primarily via osmotic pressure in vivo according to a first release profile; and

a second drug portion that comprises a second drug housing loaded with a second drug formulation which comprises a second drug, wherein the second drug portion is configured to release the second drug from the second drug housing by diffusion according to a second release profile, the second release profile differing from the first release profile; and

a retention portion operably associated with both the first and second drug portions,

wherein the first drug formulation is in the form of a plurality of solid drug tablets.

12. The implantable drug delivery device of claim 11 , wherein the first and second drug housings are segregated portions within a polymeric tube.

13. The implantable drug delivery device of claim 11 , which is dimensioned and flexible to permit passage of the device through the urethra and into the bladder of a patient for retention and release of the first and second drugs into the bladder of the patient.

14. The implantable drug delivery device of claim 11 , wherein first drug has a high solubility.

15. The implantable drug delivery device of claim 11 , wherein second drug has a low solubility.

16. The implantable drug delivery device of claim 11 , wherein:

the first drug is in a salt form with high solubility in water; and

the second drug is the same drug as the first drug but is in non-salt form with low solubility in water.

17. The implantable drug delivery device of claim 11 , wherein the retention portion comprises a retention frame having one or more windings, coils, or curves.

18. The implantable drug delivery device of claim 11 , wherein:

the second drug housing comprises a second wall that is permeable to water and to the second drug, and

the second drug portion is configured to release the second drug from the second drug housing by diffusion through the second wall in vivo.

19. The implantable drug delivery device of claim 11 , wherein the second drug formulation is in the form of a plurality of solid drug tablets.
