Abstract

Implants and methods for augmentation, preferably by minimally invasive procedures and means, of body tissue, including in some embodiments repositioning of body tissue, for example, bone and, preferably vertebrae are described. The implant may comprise one or more chain linked bodies inserted into the interior of body tissue. As linked bodies are inserted into body tissue, they may fill a central portion thereof and for example in bone can push against the inner sides of the cortical exterior surface layer, for example the end plates of a vertebral body, thereby providing structural support and tending to restore the body tissue to its original or desired treatment height. A bone cement or other filler can be added to further augment and stabilize the body tissue. The preferred implant comprises a single flexible monolithic chain formed of allograft cortical bone having a plurality of substantially non-flexible bodies connected by substantially flexible links.

Claims (36)

1. A spinal fusion method, comprising:

inserting a cannula into a disc space located between two adjacent vertebral bodies in a patient's spinal column, wherein the disc space has a transverse plane;

inserting a guide through the cannula to position a terminal distal end portion of the guide in the disc space;

inserting a chain of biocompatible material through the cannula and into the disc space, the guide engaging the chain during insertion to guide the chain in a desired direction, the chain comprising a plurality of bodies joined by a plurality of cylindrical links such that at least one of said bodies is attached at one end of the body to a first adjacent body by a first link and attached at another, opposite end of the body to a second adjacent body by a second link, the first and second links being separate from one another, the chain having a height and a length configured to reside within the disc space; and

during insertion into the disc space, angulating the plurality of bodies with respect to each other about the links to position the chain such that the chain curves in the transverse plane and such that a terminal distal end of the chain approaches a proximal end portion of the chain,

wherein, during said insertion of the chain into the disc space, one or more of the bodies are disposed in the disc space and are not in contact with the guide.

2. The method of claim 1 , wherein the heights of each of the plurality of bodies are substantially equal and at least two of the plurality of bodies have different lengths.

3. The method of claim 1 , wherein each of the bodies is formed from a polymer or a metal.

4. The method of claim 1 , wherein the bodies are non-flexible.

5. The method of claim 1 , wherein the chain comprises titanium.

6. The method of claim 1 , wherein one or more of the bodies includes a through bore extending from one outer surface of the body to another outer surface of the body.

7. The method of claim 1 , wherein one or more of the bodies is kidney shaped.

8. The method of claim 1 , wherein one or more of the bodies has a rectangular cross-section.

9. The method of claim 1 , further comprising delivering graft material to the disc space.

10. The method of claim 1 , further comprising inserting a pedicle screw into one of the vertebral bodies.

11. The method of claim 1 , further comprising using a fixation apparatus to fix the vertebral bodies in a desired position, the fixation apparatus comprising a plurality of anchors and a fixation member.

12. The method of claim 1 , further comprising delivering an osteoinductive agent to the disc space.

13. The method of claim 1 , further comprising delivering graft material to the disc space after inserting the chain.

14. A spinal fusion method, comprising:

inserting a cannula having a longitudinal axis into a disc space located between two adjacent vertebral bodies in a patient's spinal column, wherein the disc space has a transverse plane;

inserting a guide having a proximal portion and a distal end portion through the cannula such that the distal end portion of the guide is positioned in the disc space, the proximal portion of the guide is parallel to the longitudinal axis of the cannula, and the distal end portion of the guide is curved in the transverse plane;

inserting a chain of biocompatible material through the cannula and into the disc space, the distal end portion of the guide contacting the chain during insertion to guide the chain in a desired direction, the chain comprising a plurality of bodies joined by a plurality of links and having a height and a length configured to reside within the disc space; and

during insertion into the disc space, angulating the plurality of bodies with respect to each other about the links to position the chain such that the chain curves in the transverse plane and such that a terminal distal end of the chain approaches a proximal end portion of the chain,

wherein, during said insertion of the chain into the disc space, one or more of the bodies are disposed in the disc space and are not in contact with the guide.

15. The method of claim 14 , wherein the heights of each of the plurality of bodies are substantially equal and at least two of the plurality of bodies have different lengths.

16. The method of claim 14 , wherein each of the bodies is formed from a polymer or a metal.

17. The method of claim 14 , wherein the bodies are non-flexible.

18. The method of claim 14 , wherein the chain comprises titanium.

19. The method of claim 14 , wherein one or more of the bodies includes a through bore extending from one outer surface of the body to another outer surface of the body.

20. The method of claim 14 , wherein one or more of the bodies is kidney shaped.

21. The method of claim 14 , wherein one or more of the bodies has a rectangular cross-section.

22. The method of claim 14 , further comprising delivering graft material to the disc space.

23. The method of claim 14 , further comprising inserting a pedicle screw into one of the vertebral bodies.

24. The method of claim 14 , further comprising using a fixation apparatus to fix the vertebral bodies in a desired position, the fixation apparatus comprising a plurality of anchors and a fixation member.

25. The method of claim 14 , further comprising delivering an osteoinductive agent to the disc space.

26. The method of claim 14 , further comprising delivering graft material to the disc space after inserting the chain.
