Abstract

A dilation introducer for orthopedic surgery is provided for minimally invasive access for insertion of an intervertebral implant. The dilation introducer may be used to provide an access position through Kambin's triangle from a posterolateral approach. A first dilator tube with a first longitudinal axis is provided. An access cannula may be introduced over the first dilator tube. A drill may be inserted through the access cannula and used to perform a foraminoplasty. Surgical instruments may pass through the access cannula to operate on an intervertebral disc and/or insert an intervertebral implant.

Claims (31)

1. A method for accessing a patient's intervertebral disc to be treated in orthopedic surgery, comprising the steps of:

introducing a guiding element along a posterolateral trajectory towards the intervertebral disc;

passing at least one dilator over the guiding element;

passing an outer cannula over the guiding element, wherein a distal end of the outer cannula is positioned to allow direct visualization of an exiting nerve;

performing a foraminoplasty through the outer cannula; and

after performing the foraminoplasty, inserting an inner access cannula through the outer cannula.

2. The method of claim 1 , wherein inserting the inner access cannula comprises inserting the inner access cannula into or up to the intervertebral disc.

3. The method of claim 1 , wherein the inner access cannula is inserted through the outer cannula along a trajectory so as to avoid damage to the exiting nerve.

4. The method of claim 1 , wherein the posterolateral trajectory is between about 45 and 55 degrees with respect to the sagittal plane.

5. The method of claim 1 , further comprising accessing the patient's intervertebral disc by passing surgical tools through the inner access cannula.

6. The method of claim 1 , wherein the guiding element comprises a trocar or a K-wire.

7. The method of claim 1 , wherein inserting the inner access cannula through the outer cannula further comprises direct visualization of the inner access cannula.

8. The method of claim 1 , wherein the increased access space created by removal of bone aids in positioning the inner access cannula.

9. The method of claim 1 , further comprising passing a surgical tool through the inner access cannula.

10. The method of claim 1 , further comprising performing a nucleotomy through the inner access cannula.

11. The method of claim 1 , further comprising performing a discectomy through the inner access cannula.

12. The method of claim 1 , further comprising inserting an intervertebral implant through the inner access cannula.

13. A method for accessing a patient's intervertebral disc to be treated in orthopedic surgery, comprising the steps of:

introducing a first guiding element along a posterolateral trajectory towards the intervertebral disc;

passing at least one dilator over the first guiding element;

passing an outer cannula over the first guiding element, wherein a distal end of the outer cannula is positioned to allow direct visualization of an exiting nerve;

performing a foraminoplasty through the outer cannula;

inserting a second guiding element through the outer cannula; and

after performing the foraminoplasty, guiding an inner access cannula along the second guiding element in the outer cannula.

14. The method of claim 13 , further comprising removing the outer cannula after inserting the second guiding element.

15. The method of claim 13 , wherein guiding the inner access cannula along the second guiding element further comprises direct visualization of the inner access cannula.

16. The method of claim 13 , wherein the increased access space created by removal of bone aids in positioning the inner access cannula.

17. The method of claim 13 , further comprising passing a surgical tool through the inner access cannula.

18. The method of claim 13 , further comprising performing a nucleotomy through the inner access cannula.

19. The method of claim 13 , further comprising performing a discectomy through the inner access cannula.

20. The method of claim 13 , further comprising inserting an intervertebral implant through the inner access cannula.
