Abstract

A minimal incision maximal access system allows for maximum desirable work space exposure within the incision along with maximum access to the operative field utilizing a minimum incision as small as the width of the working tube. Instead of multiple insertions of dilating tubes the design is a streamlined single entry device to avoid repetitive skin surface entry. The system offers the capability to expand to optimum exposure size for the surgery utilizing retractors depending from a bi-hinged frame having internal or external controls to control the angle and separation of the retractors. Interchangeable retractor blades can be attached to the frame. Both retractor blades are independently controlled as to displacement and angle.

Claims (11)

1. A method of performing spine surgery comprising: providing: a surgical access device including: first and second frame members; a rotatable member operatively coupled with the first frame member; first and second guide members having the first and second frame members mounted thereon, each of the first and second guide member including two arms that are coplanar, wherein the two arms of the first guide member are pivotably coupled to the two arms of the second guide member, such that the first frame member is disposed between the two arms of the first guide member and the second frame member is disposed between the two arms of the second guide member, the first frame member translatable relative to the second frame member in response to rotation of the rotatable member; and first and second retractor blades, each of the first and second retractor blades defining at least one longitudinal groove and releasably attached to the first and second frame members, respectively, the first and second retractor blades movable within the first and second guide members; and a guide pin; advancing the guide pin into tissue; mounting the surgical access device over the guide pin; retracting open the surgical access device by translating the first frame member away from the second frame member; and removing the guide pin.

2. The method of performing spine surgery according to claim 1 , wherein retracting open the surgical access device includes aligning the guide pin through the at least one longitudinal groove.

3. The method of performing spine surgery according to claim 1 , wherein retracting open the surgical access device includes positioning the first and second retractor blades adjacent respective vertebral bodies.

4. The method of performing spine surgery according to claim 1 , further including verifying correct placement of the surgical access device with fluoroscopy.

5. The method of performing spine surgery according to claim 1 , further including connecting the surgical access device to an operating table.

6. The method of performing spine surgery according to claim 1 , further including enlarging a disc space.

7. The method of performing spine surgery according to claim 1 , further including placing an implant in a disc space.

8. The method of performing spine surgery according to claim 1 , wherein retracting open the surgical access device includes rotating the rotatable member.

9. The method of performing spine surgery according to claim 1 , wherein the rotatable member is threadably engageable with the first frame member.

10. The method of performing spine surgery according to claim 1 , wherein the first frame member is disposed between the two arms of the first guide member, and the second frame member is disposed between the two arms of the second guide member.

11. The method of performing spine surgery according to claim 1 , wherein each of the first and second guide members include a base, the arms of each of the first and second guide members being coplanar with their respective base.
