Diagnosis and monitoring treatment of prostate cancer
Provided herein are assays and methods related to determining a ratio of expression levels of PSA/PSMA or determining the expression level of PSMA in circulating tumor cells for diagnosis and/or for the purpose of monitoring treatment efficacy for prostate cancers that are likely hormone resistant.
1. A method for treating prostate cancer in a patient, wherein said patient is currently being treated with first line androgen deprivation therapy (ADT), said method comprising:
(a) isolating circulating prostate tumor cells from said patient over a period of time during which said patient is being treated with said first line ADT;
(b) measuring the levels of expression of PSA and PSMA on the surfaces of single isolated circulating prostate tumor cells;
(c) determining if the ratio of the levels of expression of PSA to PSMA on isolated single circulating prostate tumor cells remains the same or increases over said period of time, wherein if said ratio remains the same or increases over said period of time there is an indication that castration-resistant prostate cancer (CRPC) is emerging in said patient; and
(d) if said ratio remains the same or increases over said period of time, then discontinuing treatment with said first line ADT and initiating treatment with a second line therapy effective to treat emerging CRPC; or
(e) if said ratio decreases over said period of time, then continuing treatment with said first line ADT.
2. The method of claim 1 , wherein said first line ADT comprises administering to the patient leuprolide.
3. The method of claim 1 , wherein said second line therapy comprises administering to the patient a therapeutically effective amount of abiraterone acetate.
4. The method of claim 1 , wherein said second line therapy comprises administering to the patient a therapeutically effective amount of an anti-cancer agent that has not been previously used to treat said prostate cancer in said patient.
5. The method of claim 1 , further comprising treating emerging CRPC with a therapeutically effective amount of one or more additional chemotherapeutic agents.
6. The method of claim 5 , wherein said one or more additional chemotherapeutic agents is selected from the group consisting of paclitaxel, docetaxel, and cabazitaxel.
7. The method of claim 1 , wherein said period of time ranges from 2 weeks to 20 years.
8. The method of claim 1 , wherein the steps (a)-(d) are repeated over said period of time every week, month, or year.
