Abstract

An ablation catheter system and method of use is provided to endoscopically access portions of the human esophagus experiencing undesired growth of columnar epithelium. The ablation catheter system and method includes controlled depth of ablation features and use of either radio frequency spectrum, non-ionizing ultraviolet radiation, warm fluid or microwave radiation, which may also be accompanied by improved sensitizer agents.

Claims (65)

1. A method of ablating mucosal tissue in an esophagus, comprising:

providing an energy delivery device;

positioning at least a portion of the energy delivery device at a mucosal tissue surface of the esophagus;

delivering sufficient energy from the energy delivery device to the mucosal tissue surface to create a lesion in the mucosal tissue, while controlling a depth of the lesion.

2. The method of claim 1 , wherein the energy delivery device further comprises an expandable member.

3. The method of claim 2 , further comprising;

expanding the expandable member in the esophagus.

4. The method of claim 3 , wherein expanding the expandable member comprises inflating a balloon.

5. The method of claim 4 , wherein the balloon comprises an elastomer.

6. The method of claim 4 , wherein the balloon comprises a non-extensible bladder.

7. The method of claim 3 , wherein the expandable member is expandable sufficient to cause at least a portion of the energy delivery device to be in contact with the esophagus mucosal tissue.

8. The method of claim 3 , wherein the expandable member is expanded sufficiently to reduce blood flow of at least a portion of the mucosal tissue in the esophagus.

9. The method of claim 1 , further comprising:

identifying an existence of abnormal tissue in the mucosal tissue.

10. The method of claim 9 , wherein the abnormal tissue is visually identified.

11. The method of claim 9 , wherein the abnormal tissue is selected from Barrett's epithelium, variants of Barrett's epithelium, dysplastic tissue and malignant tissue.

12. The method of claim 1 , wherein the energy is delivered from an RF source.

13. The method of claim 12 , wherein the energy is delivered at a dosage in the range of 15 watts to 40 watts.

14. The method of claim 13 , wherein the total energy dosage is delivered over a time period from 1 second to 120 seconds.

15. The method of claim 12 , wherein the energy is delivered under conditions which substantially necrose the mucosal tissue.

16. The method of claim 1 , wherein the energy is delivered from a microwave source.

17. The method of claim 1 , wherein the energy is delivered from a light source.

18. The method of claim 1 , wherein the energy is delivered from a thermal source.

19. The method of claim 1 , wherein the energy is delivered from a resistive heating source.

20. The method of claim 1 , wherein the energy is delivered at a dosage in the range of 15 watts to 40 watts.

21. A method of treating a tissue site in an esophagus, comprising:

providing a dilation and ablation catheter having a balloon member;

providing an energy delivery device having a plurality of RF electrodes positioned on an outside surface of the balloon member;

positioning at least a portion of the energy delivery device at a mucosal surface of the tissue site in the esophagus;

expanding the balloon member in the esophagus;

delivering sufficient energy from the energy delivery device to create a lesion with a controlled depth in the mucosal tissue.

22. The method of claim 21 , wherein delivering sufficient energy comprises delivery energy through the plurality of RF electrodes in a seciuential manner.

23. The method of claim 22 , wherein the balloon member is expanded sufficiently to cause at least a portion of the energy delivery device to be in contact with a portion of mucosal layer in the esophagus.

24. The method of claim 23 , wherein the balloon member is expanded sufficiently to reduce blood flow of at least a portion of the tissue site in the esophagus.

25. The method of claim 23 , wherein the energy is delivered over a treatment band of the mucosal layer.

26. The method of claim 25 , wherein the energy is delivered to one or more segments of the treatment band.

27. The method of claim 21 , further comprising: identifying an existence of abnormal tissue in the mucosal layer.

28. The method of claim 27 , wherein the abnormal tissue is visually identified.

29. The method of claim 27 , wherein the abnormal tissue is selected from Barrett's epithelium, variant's of Barrett's epithelium, dysplastic tissue and malignant tissue.

30. The method of claim 21 , wherein the plurality of RF electrodes are arranged in a pattern.

31. The method of 30 , where at least a portion of the plurality of RF electrodes are bi-polar RF electrode pairs.

32. The method of 31 , wherein adjacent RF electrodes are spaced apart by a distance in the range of 0.3mm to 2mm.

33. The method of claim 21 , wherein energy is delivered under conditions which substantially necrose the mucosal layer.

34. The method of claim 21 , wherein energy in the range of 15 to 40 watts is supplied to the plurality of RF electrodes.

35. The method of claim 21 , wherein energy in the range of 15 to 40 watts is supplied to the plurality of RF electrodes for a period of time in the range of 1 to 120 seconds.

36. A method for treating a tissue site in the esophagus, comprising:

providing an energy delivery device that includes a plurality of RF electrodes, a width of each RF electrode and a spacing between adjacent RF electrodes being selected to control a depth of ablation in mucosal layer of the esophagus;

positioning at least a portion of the energy delivery device at the tissue site in the esophagus;

delivering sufficient energy from the energy delivery device to create a desired ablation depth in the mucosal layer.

37. The method of claim 36 , wherein the energy delivery device further comprises an expandable member.

38. The method of claim 37 , further comprising;

expanding the expandable member in the esophagus.

39. The method of claim 38 , wherein the expandable member is expanded sufficient to cause at least a portion of the energy delivery device to be in contact with the esophagus mucosal layer.

40. The method of claim 39 , wherein the expandable member is expanded sufficiently to reduce blood flow of at least a portion of the tissue site in the esophagus.

41. The method of claim 38 , wherein the energy is delivered over a treatment band of the mucosal layer.

42. The method of claim 41 , wherein the energy is delivered to one or more segments of the treatment band.

43. The method of claim 36 , further comprising: identifying an existence of abnormal tissue in the mucosal layer.

44. The method of claim 43 , wherein the abnormal tissue is visually identified.

45. The method of claim 43 , wherein the abnormal tissue is selected from Barrett's epithelium, variant's of Barrett's epithelium, dysplastic tissue and malignant tissue.

46. The method of claim 36 , wherein the plurality of RF electrodes are arranged in a pattern.

47. The method of claim 46 , where at least a portion of the plurality of RF electrodes are bi-polar RF electrode pairs.

48. The method of claim 46 , wherein adjacent RF electrodes are spaced apart by a distance in the range of 0.3mm to 2mm.

49. The method of claim 36 , wherein energy is delivered under conditions which substantially necrose the mucosal layer.

50. The method of claim 36 , wherein energy in the range of 15 to 40 watts is supplied to the plurality of RF electrodes.

51. The method of claim 50 , wherein energy in the range of 15 to 40 watts is supplied to the plurality of RF electrodes for a period of time in the range of 1 to 120 seconds.
