Abstract

A method and apparatus for providing ventilatory assistance to a spontaneously breathing patient An error signal ( 56 ) is computed that is the difference between a function of respiratory airflow ( 54 ) over a period of time and a target value ( 52 ). Using a servo loop, air is delivered to the patient at a pressure that is a function of the error signal, the phase of the current breathing cycle, and a loop gain that varies depending on the magnitude of the error signal. The loop gain increases with the magnitude of the error signal, and the gain is greater for error signals below a ventilation target than for error signals above the ventilation target value. The target value ( 52 ) is an alveolar ventilation that takes into account the patient's physiologic dead space.

Claims (24)

1. Apparatus for providing ventilatory pressure support to a patient comprising

a control mechanism configured to:

obtain a first ventilation measure with a first speed of response and a second ventilation measure with a second speed of response, the first speed of response being faster than the second speed of response, the ventilation measures being derived from respiratory flow of the patient, the second ventilation measure derived by low pass filtering;

derive a first calculated error being a function of a target ventilation value and the first ventilation measure, and a second calculated error being a function of the target ventilation value and the second ventilation measure, and

calculate a control response based on the first calculated error and the second calculated error, wherein said control response is increasingly dependent on the first calculated error and less dependent on the second calculated error, as the first ventilation measure becomes increasingly less than said target ventilation value; and

a ventilator responsive to said control response for controlling the level of pressure of air delivered to said patient.

2. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 1 wherein the control exercised by said ventilator increases with magnitudes of said first and second calculated errors.

3. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 2 wherein for equal calculated errors below and above said target value, the control exercised by said ventilator is greater for calculated errors below said target value.

4. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 3 wherein said target value is an alveolar ventilation that takes into account the patient's anatomical or physiologic dead space.

Apparatus for providing ventilatory pressure support to a patient in accordance with claim 4 wherein said control mechanism further determines a phase of a breathing cycle and adjusts said control response to be a function of an amplitude of the determined phase of said breathing cycle of an amplitude-versus-phase template that is appropriate for a normal breathing cycle.

6. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 5 wherein said control mechanism determines the phase of a current breathing cycle by relating respiratory airflow and its rate of change to different phases of a normal breathing cycle.

7. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 6 wherein said control mechanism determines the phase of the current breathing cycle by applying a set of fuzzy logic rules.

8. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 7 wherein said control response is a clipped integral of a function of both of said calculated errors.

9. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 1 wherein said target value is an alveolar ventilation that takes into account the patient's physiologic dead space.

Apparatus for providing ventilatory pressure support to a patient in accordance with claim 1 wherein said control mechanism further determines a phase of a current breathing cycle and adjusts said control response to be a function of an amplitude at the determined phase of the current breathing cycle of an amplitude-versus-phase template that is appropriate for a normal breathing cycle.

11. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 10 wherein said control mechanism determines the phase of the current breathing cycle by relating respiratory airflow and its rate of change to different phases of a normal breathing cycle.

12. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 11 wherein said control mechanism determines the phase of the current breathing cycle by applying a set of fuzzy logic rules.

13. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 1 wherein each of said calculated errors is a clipped integral of a respective patient ventilation measure minus said target value.

14. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 1 wherein said ventilator includes a servo control mechanism whose gain is adjusted in accordance with magnitudes of said calculated errors.

15. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 14 wherein said gain increases with the magnitudes of said calculated errors.

16. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 15 wherein for equal calculated errors below and above said target value, said gain is greater for calculated errors below said target value.

17. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 15 wherein said gain is varied more aggressively for conditions of hypoventilation than for conditions of hyperventilation.

18. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 1 wherein said ventilator is flow-triggered and phase cycled.

19. Apparatus for providing ventilatory pressure support to a patient in accordance with claim 1 wherein said ventilator withdraws ventilation support more gradually when the patient is over-ventilated than when the patient is under-ventilated.
