Abstract

Methods and kits for predicting the efficacy of antifolate (e.g., methotrexate) treatment of rheumatoid arthritis by detecting polymorphisms, particularly single nucleotide polymorphisms, in adenosine pathway genes.

Claims (12)

1. A method for predicting clinical responsiveness to methotrexate therapy in a human afflicted with, or at risk of developing, rheumatoid arthritis comprising:

detecting the presence of a 34C>T polymorphism in the adenosine monophosphate deaminase (AMPD1) gene, and

determining the patient's genotype at position 347 of the aminoimidazole carboxamide ribonucleotide transformylase (ATIC) gene and at position 94 of the inosine triphosphate pyrophosphatase (ITPA) gene,

wherein the presence of said polymorphism, a CC genotype at position 347 of the ATIC gene, and a CC genotype at position 94 of the ITPA gene is indicative of clinical responsiveness to said methotrexate therapy.

2. The method according to claim 1 , wherein said methotrexate responsiveness is measured as a disease activity score (DAS) less than or equal to 2.4.

3. The method according to claim 1 , wherein the polymorphism is detected by microarray analysis, DNA sequencing or allele specific PCR techniques.

4. The method of claim 1 , further comprising selecting or adjusting said methotrexate therapy depending upon the results of said detecting.

5. The method of claim 4 , wherein said selected or adjusted therapy is methotrexate monotherapy, methotrexate combination therapy, or methotrexate biologic therapy.

6. The method of claim 1 , wherein said human is afflicted with recent-onset rheumatoid arthritis.

7. The method of claim 1 , wherein said human's rheumatoid arthritis is active.

8. The method of claim 7 , wherein active rheumatoid arthritis is defined as greater than or equal to 6/66 swollen joints and greater than or equal to 6/68 tender joints and either an ESR greater than or equal to 28 mm/hr or a Visual Analogue Score for global health less than or equal to 20 mm.

9. The method of claim 1 , further comprising detecting a rheumatoid arthritis diagnostic marker selected from the group consisting of rheumatoid factor, C-reactive protein, and a citrullinated antigen.
