Abstract

A heart monitor is disclosed. The monitor computes ST segment deviations and stores the results in heart rate based histograms. Periodically, the monitor analyzes the histogram data to determine heart rate dependent acute ischemia detection thresholds. If the statistical distribution associated with a heart rate range is insufficient, the threshold for that heart rate range is set as a function of the threshold for a neighboring heart rate range. Thresholds are also increased for heart rate ranges associated with statistical distributions that are sufficient but that have a relatively small number of entries.

Claims (33)

1. A cardiac monitor for detecting a cardiac event, the monitor comprising:

(a) a sensor for sensing an analog signal from a human heart;

(b) a device coupled to the sensor, the device having an analog-to-digital circuit system contained therein for digitizing the analog signal to produce a digitized waveform;

(c) a processor electrically coupled to the analog-to-digital circuit system, said processor configured to:

(i) compute a plurality of values of a heart signal parameter from the digitized waveform, the plurality of values of the heart signal parameter being characterized by a plurality of statistical distributions corresponding to a plurality of heart rates, wherein each statistical distribution is characterized by an upper boundary and a lower boundary and a dispersion;

(ii) store data pertaining to the plurality of values of the heart signal parameter in a form that enables characteristics of first and second statistical distributions to be computed for first and second heart rates;

(iii) determine a boundary value of the heart signal parameter associated with the second heart rate;

(iv) determine a measure of the number of data points of the first statistical distribution;

(v) operative when the number of data points indicates the first statistical distribution is insufficient, compute a detection threshold associated with the first heart rate, wherein the detection threshold is based on the boundary value of the heart signal parameter associated with the second heart rate;

(vi) apply a test to detect a cardiac event, wherein the test is based on the detection threshold.

2. The cardiac monitor of claim 1 wherein the data pertaining to the plurality of values of the heart signal parameter is stored according to a plurality of heart rate ranges, so that the first and second heart rates correspond to first and second heart rate ranges.

3. The cardiac monitor of claim 2 wherein the data pertaining to the plurality of values of the heart signal parameter is stored in a plurality of histograms corresponding to the plurality of heart rate ranges.

4. The cardiac monitor of claim 1 wherein the boundary value is determined by searching for heart signal parameter value beyond which there are less than a specified number of elements.

The cardiac monitor of claim 1 wherein the processor is further configured to compute a detection threshold associated with the second heart rate, and wherein the detection threshold associated with the first heart rate is set equal to a function of the detection threshold associated with the second heart rate.

6. The cardiac monitor of claim 5 wherein the detection threshold associated with the second heart rate is determined by adding a distance to the boundary value.

7. The cardiac monitor of claim 6 wherein the distance is a function of the dispersion of the second statistical distribution.

8. The cardiac monitor of claim 5 wherein the function is linear.

9. The cardiac monitor of claim 8 wherein the detection threshold associated with the first heart rate is set equal to the detection threshold associated with the second heart rate plus an offset value.

10. The cardiac monitor of claim 1 wherein the device is configured to process the analog signal in segments, and wherein each element in the plurality of statistical distributions corresponds to a single segment.

11. The cardiac monitor of claim 1 wherein each element in the plurality of statistical distributions has a value that is based on an average of a plurality of values of the heart signal parameter.

12. The cardiac monitor of claim 1 wherein each element in the plurality of statistical distributions has a value that is based on the output of a recursive low pass filter.

13. The cardiac monitor of claim 1 wherein the heart signal parameter is ST segment deviation.

14. A cardiac monitor for detecting a cardiac event, the monitor comprising:

(a) a sensor for sensing an analog signal from a human heart;

(b) a device coupled to the sensor, the device having an analog-to-digital circuit system contained therein for digitizing the analog signal to produce a digitized waveform;

(c) a processor electrically coupled to the analog-to-digital circuit system, said processor configured to:

(i) compute a plurality of values of a heart signal parameter from the digitized waveform, the plurality of values of the heart signal parameter being characterized by a statistical distribution characterized by an upper boundary and a lower boundary and a dispersion;

(ii) store data pertaining to the plurality of values of the heart signal parameter in a form that enables characteristics of the statistical distribution to be computed;

(iii) determine the number of elements in the statistical distribution;

(iv) determine a characteristic of the statistical distribution based on the plurality of values of the heart signal parameter;

(v) compute a detection threshold based on the characteristic and the number of elements;

(vi) apply a test to detect a cardiac event, wherein the test is based on the detection threshold.

15. The cardiac monitor of claim 14 wherein the characteristic of the statistical distribution is an upper or lower boundary.
