Abstract

Various system embodiments comprise a stimulator adapted to deliver a stimulation signal for a heart failure therapy, a number of sensors adapted to provide at least a first measurement of a heart failure status and a second measurement of the heart failure status, and a controller. The controller is connected to the stimulator and to the number of sensors. The controller is adapted to use the first and second measurements to create a heart failure status index, and control the stimulator to modulate the signal using the index. Other aspects and embodiments are provided herein.

Claims (41)

1. A system, comprising:

a stimulator configured to deliver a stimulation signal for a heart failure therapy;

a plurality of data sources of heart failure parameters; and

a controller connected to the stimulator and to the plurality of data sources, wherein the controller is configured to:

use a dual feedback response to account for both long-term heart failure status changes and short-term heart failure status changes;

use the data sources to create heart failure status indices, the heart failure status indices including a short term heart failure status index for detecting inadequate cardiac output and a long term heart failure status index for detecting worsening or improving heart failure; and

control the stimulation to deliver the heart failure therapy using the short-term heart failure status index and the long-term heart failure index.

2. The system of claim 1 , further comprising a pacemaker configured to pace a heart to enforce a heart rate to meet metabolic demand.

3. The system of claim 1 , wherein the short term heart failure status index for detecting inadequate cardiac output includes a short term status for detecting a decompensation event.

4. The system of claim 1 , wherein the short term heart failure status index for detecting inadequate cardiac output includes a short term status for detecting an approaching decompensation event.

5. The system of claim 4 , wherein the controller is configured to apply neural stimulation in response to detecting the approaching decompensation event.

6. The system of claim 1 , wherein the controller is configured to use the long term heart failure status index to detect trends in the long term heart failure status index.

7. The system of claim 1 , wherein the controller is configured to create heart failure status indices using a composite index of two or more heart failure parameters.

8. The system of claim 1 , wherein the controller is configured to use the heart failure status indices to manage battery power by reducing false positives.

9. The system of claim 1 , wherein the controller is configured to use the heart failures status indices to reduce patient over stimulation.

10. The system of claim 1 , wherein the controller is configured to use the dual feedback response, which accounts for both long-term heart failure status changes and short-term heart failure status changes, to manage battery power.

11. The system of claim 1 , wherein the controller is configured to use the dual feedback response, which accounts for both long-term heart failure status changes and short-term heart failure status changes, to avoid false positives, wherein the system is configured with a programmable false alarm rate used by the controller to automatically set a threshold to yield false alarms at the programmable false alarm rate.

12. The system of claim 11 , wherein the system is configured to allow a clinician to program the programmable false alarm rate.

13. The system of claim 1 , wherein the stimulator includes a neural stimulator adapted to deliver a neural stimulation signal to an autonomic neural target for the heart failure therapy.

14. The system of claim 13 , wherein the controller is adapted to control the stimulator to increase a parasympathetic response when the long-term index is less than a first threshold and to decrease the parasympathetic response when the long-term index exceeds a second threshold.

15. The system of claim 14 , wherein circuitry in a single implantable device is a neural stimulator and a myocardial stimulator.

16. The system of claim 1 , wherein the stimulation signal for the heart failure therapy includes a myocardial stimulation signal.

17. The system of claim 1 , wherein the plurality of data sources of parameters related to heart failure include data sources of externally sensed data.

18. The system of claim 17 , wherein the externally sensed data includes externally sensed weight or externally sensed blood pressure.

19. The system of claim 1 , wherein the plurality of data sources of parameters related to heart failure include data input by a physician or by a patient.

20. The system of claim 1 , wherein the plurality of data sources of parameters related to heart failure include pharmaceutical data.

21. The system of claim 1 , wherein the plurality of data sources of parameters related to heart failure include data from a population/clinical study.

22. The system of claim 1 , wherein the plurality of data sources of parameters related to heart failure include measurements selected from a heart rate variability (HRV) measurement, a heart rate turbulence (HRT) measurement, a respiration measurement, a heart sound measurement, a blood pressure measurement, or a thoracic impedance measurement.

23. The system of claim 1 , wherein the controller is configured to use an inference engine to create heart failure status indices from the data sources.

24. The system of claim 1 , wherein the controller is configured to use a probability function to create heart failure status indices from the data sources.

25. The system of claim 1 , wherein the plurality of data sources of parameters related to heart failure include data input by a physician or by a patient, pharmaceutical data, data from a population/clinical study, a heart rate variability (HRV) measurement, a heart rate turbulence (HRT) measurement, a respiration measurement, a heart sound measurement, or a thoracic impedance measurement.

26. A system, comprising:

a neural stimulator configured to deliver a stimulation signal to stimulate an autonomic neural target for a heart failure therapy;

a myocardial stimulator configured to deliver a stimulation signal to stimulate myocardia, the myocardial stimulator includes a pacemaker configured to pace a heart to enforce a heart rate to meet metabolic demand;

a plurality of data sources of heart failure parameters, wherein the data sources include an externally sensed measure of weight; and

a controller connected to the stimulator and to the plurality of data sources, wherein the controller is configured to:

use a dual feedback response to account for both long-term heart failure status changes and short-term heart failure status changes;

use the data sources to create heart failure status indices using a composite index of two or more heart failure parameters, the heart failure status indices including a short term heart failure status index for detecting a decompensation event and a long term heart failure status index for detecting worsening or improving heart failure; and

control the stimulator to deliver the heart failure therapy using the short-term heart failure status index and the long-term heart failure index, including controlling the stimulator in response to detecting the decompensation event to reduce a parasympathetic response.

27. The system of claim 26 , wherein the controller is configured to use the dual feedback response, which accounts for both long-term heart failure status changes and short-term heart failure status changes, to manage battery power.

28. The system of claim 26 , wherein the controller is configured to use the dual feedback response, which accounts for both long-term heart failure status changes and short-term heart failure status changes, to avoid false positives, wherein the system is configured with a programmable false alarm rate used by the controller to automatically set a threshold to yield false alarms at the programmable false alarm rate.
