Abstract

Method are provided for treating a patient having bladder pain and/or irritative voiding symptoms. The method includes administering to the patient's bladder lidocaine or another anesthetic agent continuously over a treatment period of 24 hours or more in an amount effective to achieve a therapeutic effect which is sustained beyond the end of the treatment period.

Claims (35)

1. A method of treating a patient having bladder pain and/or irritative voiding symptoms, the method comprising:

administering to the patient's bladder an agent, which comprises lidocaine, continuously over a treatment period of 24 hours or more in an amount effective to achieve a therapeutic effect which is sustained beyond an end of the treatment period,

wherein the lidocaine is released into the bladder at a rate of from 15 mg to 30 mg lidocaine (FBE) per day over a first 1 to 4 days of the treatment period and then tapers to a rate of from 15 mg to 3 mg lidocaine (FBE) per day over a remainder of the treatment period.

2. The method of claim 1 , wherein the step of administering comprises releasing the lidocaine from a drug delivery device inserted into the bladder.

3. The method of claim 2 , wherein the device releases the lidocaine continuously from 2 to 28 days.

4. The method of claim 2 , wherein the device releases the lidocaine continuously from 3 to 21 days.

5. The method of claim 2 , wherein the device releases the lidocaine continuously from 10 to 14 days.

6. The method of claim 1 , wherein the lidocaine is administered continuously from 10 to 14 days.

7. The method of claim 1 , wherein a cumulative amount of the lidocaine (FBE) administered over the treatment period is from about 20 mg to about 400 mg.

8. The method of claim 1 , wherein the patient has a measurable plasma concentration of lidocaine which does not exceed 65 ng/ml at a time of peak lidocaine exposure in the bladder.

9. The method of claim 1 , wherein the therapeutic effect is sustained more than 48 hours beyond the end of the treatment period.

10. The method of claim 1 , wherein the agent administered to the bladder is released from a delivery device which is wholly deployed into bladder during the treatment period.

11. The method of claim 1 , wherein the agent administered to the bladder is pumped into bladder through a urethral catheter which is deployed into the bladder during the treatment period.

The method of claim 1 , wherein the agent administered to the bladder is released from a coating substance that is transurethrally applied to the bladder wall, the coating substance comprising the lidocaine and one or more excipient materials that promote adherance of the coating substance to the wall of the bladder and provides continuous controlled release of the lidocaine over the treatment period.

13. The method of claim 1 , wherein a cumulative amount of the lidocaine (FBE) administered over the treatment period is between 150 mg and 1000 mg.

14. A method of treating a patient having bladder pain and/or irritative voiding symptoms, the method comprising:

administering lidocaine to the bladder of the patient in an amount effective to achieve a therapeutic effect which is sustained beyond an end of the treatment period,

wherein the lidocaine is administered continuously over a treatment period of from 2 to 28 days,

wherein a cumulative amount of the lidocaine (FBE) administered over the treatment period is between 100 mg and 600 mg,

wherein the lidocaine is administered at an average amount of from about 10 mg to about 30 mg lidocaine (FBE) per day.

15. The method of claim 14 , wherein the measurable plasma concentration of lidocaine does not exceed 65 ng/ml at a time of peak lidocaine exposure in the bladder.

16. The method of claim 14 , wherein the step of administering comprises releasing the lidocaine from a drug delivery device inserted into the bladder.

17. A method of treating a patient having bladder pain and/or irritative voiding symptoms, the method comprising:

administering lidocaine into the patient's bladder continuously over a treatment period at an average amount of from about 5 mg to about 30 mg lidocaine (FBE) per day, to achieve a therapeutic effect which is sustained beyond an end of the treatment period,

wherein the treatment period is from 2 to 28 days,

wherein the lidocaine is continuously released from a drug delivery device inserted into the bladder, and

wherein a cumulative amount of lidocaine (FBE) released over the treatment period is between 150 mg and 1000 mg.

18. The method of claim 17 , wherein the lidocaine is released continuously from 10 to 14 days.

19. The method of claim 17 , wherein the step of administering comprises releasing the lidocaine from a drug delivery device inserted into the bladder and the device releases the lidocaine continuously from 2 to 4 weeks.

20. A method of treating a patient having bladder pain and/or irritative voiding symptoms, the method comprising:

administering to the patient's bladder an agent, which comprises lidocaine, continuously over a treatment period of 24 hours or more in an amount effective to achieve a therapeutic effect which is sustained beyond an end of the treatment period,

wherein the lidocaine is released into the bladder at a rate from 20 mg to 70 mg lidocaine (FBE) per day over a first 1 to 6 days of the treatment period and then tapers to a rate of from 25 mg to 10 mg lidocaine (FBE) per day over a remainder of the treatment period.

21. The method of claim 20 , wherein the patient has a measurable plasma concentration of lidocaine which does not exceed 65 ng/ml at a time of peak lidocaine exposure in the bladder.

22. The method of claim 20 , wherein a cumulative amount of the lidocaine (FBE) administered over the treatment period is between 150 mg and 1000 mg.

23. The method of claim 20 , wherein the agent administered to the bladder is released from a delivery device which is wholly deployed into bladder during the treatment period.
