Automated inspiratory muscle training for patients receiving mechanical ventilation
A system and method for automated inspiratory muscle strength exercise includes software for setting a mechanical ventilator for a pressure regulated breath with an initial pressure target that is at the highest pressure setting a patient can tolerate and increasing the pressure target as tolerated.
1. A system for automated inspiratory muscle strength training of a patient during mechanical ventilation comprising:
a gas delivery device, wherein said gas delivery device comprises an inhalation assembly that provides breathing gas for inspiratory air flow during patient spontaneous inhalation and includes a valve that controls the breathing gas during a pre-determined number of the spontaneous inhalations;
a control module configured to instruct the gas delivery device to control the breathing gas to maintain a pressure adapted to be below a baseline pressure of a mechanical ventilator during the pre-determined number of spontaneous inhalations; and
a sensing assembly, operably connected to the control module, wherein the sensing assembly, through the operable connection to the control module, is configured to monitor the pressure and facilitate control of the gas delivery device to provide breathing gas for inspiratory air flow.
2. The system of claim 1 , wherein the gas delivery device is removably integrated with the mechanical ventilator.
3. The system of claim 1 , wherein the valve is a positive end-expiratory pressure (PEEP) valve that is used in a reverse direction.
4. The system of claim 1 , further comprising a display.
5. The system of claim 1 , further comprising gas delivery settings for the control module to adjust the inspiratory air flow.
6. The system of claim 1 , wherein the gas delivery device delivers the breathing gas at a pressure between 1 cm H 2 O to 80 cm of H 2 O below the baseline pressure of the mechanical ventilator.
7. The system of claim 6 , wherein the gas delivery device delivers a breathing gas at a pressure between 1 cm of H 2 O to 20 cm of H 2 O below the baseline pressure of the mechanical ventilator.
8. The system of claim 1 , wherein the pressure of the breathing gas controlled by the gas delivery device is determined by a target power of breathing.
9. The system of claim 8 , wherein the target power of breathing is determined by the mechanical ventilator based upon one or more patient-derived parameters.
10. The system of claim 1 , wherein the control module further instructs the gas delivery device to restrict the breathing gas for between twelve to forty spontaneous inhalations over a twenty-four hour period.
11. The system of claim 1 , wherein the inspiratory training pressure comprises 30% to 60% of a maximal inhalation strength.
12. The system of claim 1 , wherein the control module further instructs the gas delivery device to restrict the breathing gas for between two to six sets of six to ten spontaneous inhalations over a twenty-four hour period.
13. The system of claim 1 , wherein the inspiratory training pressure comprises 60% to 80% of a maximal inhalation strength.
14. The system of claim 1 , wherein the control module instructs the gas delivery device to restrict the breathing gas for inspiratory air flow for two to five sets of six to ten spontaneous inhalations in a twenty-four hour period, and wherein the inspiratory air flow is restricted to 70% to 80% of a maximal inhalation strength.
