Abstract

An illuminated suction apparatus including a hand-held surgical device combining a high-performance non-fiber optic optical waveguide with suction. This device is useful in a wide array of surgical procedures including open and minimally invasive orthopedics.

Claims (50)

1. An illuminated medical device comprising:

a non-fiber optic optical waveguide having a first index of refraction;

an optical cladding disposed on at least a portion of the non-fiber optic optical waveguide, the optical cladding having a second index of refraction lower than the first to preserve total internal reflection of light passing through the non-fiber optic optical waveguide; and

a medical or surgical instrument coupled to the waveguide, and

wherein the medical or surgical instrument is coupled to the waveguide such that an air gap is maintained between the medical or surgical instrument and the non-fiber optic optical waveguide, and wherein the air gap facilitates total internal reflection of light passing through the non-fiber optic optical waveguide.

2. The device of claim 1 , wherein the second index of refraction is coordinated to the first to preserve total internal reflection.

3. The device of claim 1 , wherein the first index of refraction is in a range from 1.46 to 1.70.

4. The device of claim 1 , wherein the second index of refraction is in a range from 1 to 1.67.

5. The device of claim 1 , wherein the non-fiber optic optical waveguide comprises a proximal portion and a distal portion, and wherein light is projected from the proximal portion to the distal portion by total internal reflection.

6. The device of claim 1 , wherein the non-fiber optic optical waveguide has an outer surface and the optical cladding is disposed at least partially over the outer surface.

7. The device of claim 6 , further comprising an outer protective layer disposed over the optical cladding to minimize damage to the waveguide.

8. The device of claim 1 , wherein the non-fiber optic optical waveguide has an inner bore with an inner surface.

9. The device of claim 8 , wherein the optical cladding is disposed against the inner surface.

10. The device of claim 8 , wherein the medical or surgical instrument is disposed within the inner bore.

11. The device of claim 1 , wherein the optical cladding comprises an elongate molded polymer element.

12. The device of claim 1 , wherein the optical cladding comprises a concave region configured to receive the waveguide.

13. The device of claim 1 , wherein the optical cladding is rigid.

14. The device of claim 1 , wherein the optical cladding is flexible.

15. The device of claim 1 , wherein light projected through the non-fiber optic optical waveguide is internally reflected with nearly 100% efficiency.

16. The device of claim 1 , wherein the optical cladding comprises a cap element placed over the optical waveguide to prevent blood or other fluid and debris from contacting the non-fiber optic optical waveguide.

17. The device of claim 16 , wherein the cap element is placed over the non-fiber optic optical waveguide such that an air gap is maintained between the cap element and the non-fiber optic optical waveguide, and wherein the air gap facilitates total internal reflection of light passing through the non-fiber optic optical waveguide.

18. The device of claim 1 , further comprising a flexible optical input coupled to a proximal end of the non-fiber optic optical waveguide.

19. The device of claim 18 , wherein the flexible optical input flares outward to match the width of the proximal end of the non-fiber optic optical waveguide.

20. The device of claim 18 , wherein the flexible optical input comprises a bundle of optical fibers or a single flexible light pipe.

21. A method of illuminating a surgical field with a surgical instrument comprising:

projecting light through a non-fiber optic optical waveguide having a first index of refraction, wherein the projected light passes through the non-fiber optic optical waveguide by total internal reflection;

preserving the total internal reflection by providing an optical cladding disposed on at least a portion of the non-fiber optic optical waveguide and having a second index of refraction lower than the first; and

maintaining an air gap between the non-fiber optic optical waveguide and a medical or surgical instrument coupled to the non-fiber optic optical waveguide to preserve the total internal reflection.

22. The method of claim 21 , further comprising applying suction through a medical or surgical instrument coupled to the non-fiber optic optical waveguide.

23. The method of claim 21 , further comprising applying electric current to tissue from a medical or surgical instrument coupled to the non-fiber optic optical wave guide to stimulate tissue.

24. An illuminated medical device comprising:

a non-fiber optic optical waveguide having a first index of refraction;

an optical cladding disposed on at least a portion of the non-fiber optic optical waveguide, the optical cladding having a second index of refraction lower than the first to preserve total internal reflection of light passing through the non-fiber optic optical waveguide,

wherein the optical cladding comprises a cap element placed over the optical waveguide to prevent blood or other fluid and debris from contacting the non-fiber optic optical waveguide,

wherein the cap element is placed over the non-fiber optic optical waveguide such that an air gap is maintained between the cap element and the non-fiber optic optical waveguide, and wherein the air gap facilitates total internal reflection of light passing through the non-fiber optic optical waveguide; and

a medical or surgical instrument coupled to the waveguide.

25. The device of claim 24 , wherein the second index of refraction is coordinated to the first to preserve total internal reflection.

26. The device of claim 24 , wherein the first index of refraction is in a range from 1.46 to 1.70.

27. The device of claim 24 , wherein the second index of refraction is in a range from 1 to 1.67.

28. The device of claim 24 , wherein the non-fiber optic optical waveguide comprises a proximal portion and a distal portion, and wherein light is projected from the proximal portion to the distal portion by total internal reflection.

29. The device of claim 24 , wherein the non-fiber optic optical waveguide has an outer surface and the optical cladding is disposed at least partially over the outer surface.

30. The device of claim 29 , further comprising an outer protective layer disposed over the optical cladding to minimize damage to the waveguide.

31. The device of claim 24 , wherein the non-fiber optic optical waveguide has an inner bore with an inner surface.

32. The device of claim 31 , wherein the optical cladding is disposed against the inner surface.

33. The device of claim 31 , wherein the medical or surgical instrument is disposed within the inner bore.

34. The device of claim 24 , wherein the optical cladding comprises a concave region configured to receive the waveguide.

35. The device of claim 24 , wherein light projected through the non-fiber optic optical waveguide is internally reflected with nearly 100% efficiency.

36. The device of claim 24 , further comprising a flexible optical input coupled to a proximal end of the non-fiber optic optical waveguide.

37. The device of claim 36 , wherein the flexible optical input flares outward to match the width of the proximal end of the non-fiber optic optical waveguide.

38. The device of claim 36 , wherein the flexible optical input comprises a bundle of optical fibers or a single flexible light pipe.
