Abstract

According to some embodiments, a method of treating a subject having diabetes or symptoms associated with diabetes is provided. The method includes delivering a neuromodulation catheter within a vessel (e.g., hepatic artery) having surrounding nerves that innervate the liver (e.g., sympathetic nerves of the hepatic plexus). The method may also include modulating (e.g., disrupting, ablating, stimulating) the nerves by mechanical compression, energy delivery, or fluid deliver.

Claims (36)

1. A method of facilitating modulation of nerves surrounding a gastroduodenal artery of a subject, comprising:

intravascularly positioning a distal end of a radiofrequency (RF) energy delivery catheter within a portion of a gastroduodenal artery of a vasculature of a subject; and

using the RF catheter to deliver RF energy to a wall of the gastroduodenal artery sufficient to denervate one or more sympathetic nerves surrounding the gastroduodenal artery,

wherein the RF catheter comprises at least one electrode, and

wherein the at least one electrode is configured to contact the wall of the gastroduodenal artery while the RF energy is being delivered.

2. The method of claim 1 , wherein the at least one electrode comprises a bipolar pair of electrodes.

3. The method of claim 1 , wherein the at least one electrode is configured to penetrate through the wall of the gastroduodenal artery.

4. The method of claim 1 , wherein a contact pressure between 0.1 g/mm 2 and 100 g/mm 2 is exerted on the wall.

5. The method of claim 1 , wherein the RF energy delivered by the at least one electrode to the wall is in the range of between 100 J and 1 kJ.

6. The method of claim 1 ,

wherein the RF energy is delivered at a frequency in the range of 50 kHz to 5 MHz; and

wherein the RF energy delivered is sufficient to cause permanent ablation of the one or more sympathetic nerves.

7. The method of claim 1 , wherein the denervation is effective to alter signaling of at least one of GLP-1, GIP or PYY hormones.

8. The method of claim 1 , wherein the denervation is effective to reduce a norepinephrine level in the subject.

9. The method of claim 1 , wherein the denervation is effective to improve beta cell function in the subject.

10. A method of facilitating modulation of nerves surrounding a gastroduodenal artery of a subject, comprising:

delivering an energy delivery catheter within a gastroduodenal artery of a subject; and

using the energy delivery catheter to deliver energy to a wall of the gastroduodenal artery sufficient to modulate one or more nerves surrounding the gastroduodenal artery to treat a metabolic disorder.

11. The method of claim 10 , wherein the energy delivery catheter is delivered through a guide catheter and a distal end of the energy delivery catheter is positioned at a location within the gastroduodenal artery.

12. The method of claim 10 , wherein the energy delivery catheter comprises a radiofrequency energy delivery catheter having at least one electrode at a distal end portion of the energy delivery catheter.

13. The method of claim 10 , wherein the energy delivery catheter comprises an ultrasound energy delivery catheter having at least one transducer at a distal end portion of the energy delivery catheter.

14. The method of claim 10 , further comprising positioning the energy delivery catheter in the vicinity of sympathetic nerve fibers that innervate the duodenum.

15. The method of claim 10 ,

wherein the energy is delivered at a frequency in the range of 50 kHz to 5 MHz, and

wherein the energy is delivered to cause permanent ablation of the one or more nerves.

16. The method of claim 10 , wherein the energy is delivered at a frequency in the range of 20 kHz to 20 MHz.

17. The method of claim 10 , wherein the modulation comprises temporary disruption of nerve conduction of the one or more nerves.

18. A method of facilitating neuromodulation in a subject, comprising:

providing an energy delivery catheter configured for:

accessing a portion of a gastroduodenal artery; and

delivering energy through a wall of the gastroduodenal artery sufficient to disrupt neural communication along one or more nerves associated with the gastroduodenal artery to treat one or more symptoms of a metabolic disorder.

19. The method of claim 18 ,

wherein the energy comprises radiofrequency (RF) energy at a frequency in the range of 50 kHz to 5 MHz,

wherein the one or more nerves comprises sympathetic nerves, and

wherein the RF energy delivered is sufficient to cause permanent ablation of the one or more sympathetic nerves.

20. The method of claim 18 , wherein the disruption of neural communication comprises temporary stopping or terminating nerve conduction.
