MANAGING THE CARE OF A CLIENT IN A CARE MANAGEMENT SYSTEM
A care management system for improving the care of a client who is supported by a caregiver is described. The caregiver may participate in regular activities with the client and record these activities in the care management system. The care management system may present the caregiver with questions on a regular basis. Information may also be input into the system by a care management team made up of professionals responsible for the client's overall care. A risk analyzer may determine the client's risk for different risk conditions, and may calculate domain-specific and overall risk scores for the client. Furthermore, the risk analyzer may make recommendations for actions to be taken by the client, the caregiver, or the care management team in order to improve the client's care. The information in the care management system may be used by the care management team to direct aspects of the client's care.
1 . A non-transitory computer-readable medium storing instructions that, when executed by a processor, cause the processor to:
receiving first authentication credentials for a first user;
displaying a first interface for the first user, the first interface accepting first input data in a first domain that describes an aspect of a client's overall health;
receiving the first input data from the first user through the first interface;
storing the first input data in a data storage mechanism;
calculating a first risk score based on the first input data;
receiving second authentication credentials for a second user;
displaying a second interface for the second user, the second interface accepting second input data in a second domain, wherein:
the second interface differs from the first interface, and
the second domain differs from the first domain;
receiving the second input data from the second user through the second interface;
storing the second input data in the data storage mechanism;
calculating a second risk score based on the second input data;
aggregating the first input data and the second input data to generate aggregated input data; and
calculating an overall risk score based on the aggregated input data.
2 . The medium of claim 1 , wherein the first domain and the second domain are each selected from a group consisting of:
a functional domain relating to an ability of the client to function in daily life;
a medical domain relating to a medical status of the client;
a psychosocial/behavioral domain relating to a mental status of the client;
a family/community domain relating to a family and community support network of the client; and
an environmental domain relating to a client's immediately daily environment and physical security.
3 . The medium of claim 1 , wherein the first input data is collected during an assessment of the first user, and the first risk score is further calculated using input data from one or more previous assessments.
4 . The medium of claim 1 , wherein the first user is a caregiver for the client, and the first input data entered on substantially a daily basis.
5 . The medium of claim 1 , wherein the second user is a member of a care management team having expertise in the second domain, and the second input data is entered on substantially a monthly basis.
6 . The medium of claim 1 , wherein the aggregated data further comprises input data entered on an ad hoc basis outside of a normal data input schedule of the first user or the second user.
7 . The medium of claim 1 , further storing a database of intervention information, and further storing instructions for:
searching the database of intervention information based on known information about one or more conditions of the client and correlating data stored with interventions in the database, and
recommending an intervention from the database of intervention information.
8 . The medium of claim 7 , wherein the correlating data comprises success rate information related to the interventions.
9 . The medium of claim 1 , further storing instructions for:
receiving a selection of a subdomain;
in response to the selection of the subdomain, displaying risk data related to one or more portions of the subdomain, the one or more portions comprising a subset of information used to calculate a risk score for the subdomain.
10 . The medium of claim 1 , wherein the first input data is provided in response to questions selected from a questions database and presented on an interface, wherein the questions are presented iteratively over a period of time in order to build a representation of the client's risk factors.
11 . A computer-implemented method comprising:
receiving first authentication credentials for a first user;
displaying a first interface for the first user, the first interface accepting first input data in a first domain that describes an aspect of a client's overall health;
receiving the first input data from the first user through the first interface;
storing the first input data in a data storage mechanism;
calculating a first risk score based on the first input data;
receiving second authentication credentials for a second user;
displaying a second interface for the second user, the second interface accepting second input data in a second domain, wherein:
the second interface differs from the first interface, and
the second domain differs from the first domain;
receiving the second input data from the second user through the second interface;
storing the second input data in the data storage mechanism;
calculating a second risk score based on the second input data;
aggregating the first input data and the second input data to generate aggregated input data; and
calculating, using a processor of a computer, an overall risk score based on the aggregated input data.
12 . The method of claim 11 , wherein the first user is a caregiver for the client, and the first input data entered on substantially a daily basis.
13 . The method of claim 11 , wherein the second user is a member of a care management team having expertise in the second domain, and the second input data is entered on substantially a monthly basis.
14 . The method of claim 11 , comprising:
searching a database of intervention information based on known information about one or more conditions of the client and correlating data stored with interventions in the database, and
recommending an intervention from the database of intervention information.
15 . The method of claim 11 , wherein the first input data is provided in response to questions selected from a questions database and presented on an interface, wherein the questions are presented iteratively over a period of time in order to build a representation of the client's risk factors.
16 . A system comprising:
a non-transitory storage medium storing receiving first authentication credentials for a first user;
displaying a first interface for the first user, the first interface accepting first input data in a first domain that describes an aspect of a client's overall health;
receiving the first input data from the first user through the first interface;
storing the first input data in a data storage mechanism;
calculating a first risk score based on the first input data;
receiving second authentication credentials for a second user;
displaying a second interface for the second user, the second interface accepting second input data in a second domain, wherein:
the second interface differs from the first interface, and
the second domain differs from the first domain;
receiving the second input data from the second user through the second interface;
storing the second input data in the data storage mechanism;
calculating a second risk score based on the second input data;
aggregating the first input data and the second input data to generate aggregated input data;
calculating an overall risk score based on the aggregated input data; and
storing the first risk score, the second risk score, and the overall risk score in the storage.
17 . The system of claim 16 , wherein the first user is a caregiver for the client, and the first input data entered on substantially a daily basis.
18 . The system of claim 16 , wherein the second user is a member of a care management team having expertise in the second domain, and the second input data is entered on substantially a monthly basis.
19 . The system of claim 16 , wherein the storage medium further stores a database of intervention information, and
the processor is further configured to:
search the database of intervention information based on known information about one or more conditions of the client and correlating data stored with interventions in the database, and
recommending an intervention from the database of intervention information.
20 . The system of claim 16 , wherein the first input data is provided in response to questions selected from a questions database and presented on an interface, wherein the questions are presented iteratively over a period of time in order to build a representation of the client's risk factors.
