Abstract

A care management system for improving the care of a client who is supported by a caregiver is described. The caregiver may participate in regular activities with the client and record these activities in the care management system. The care management system may present the caregiver with questions on a regular basis. Information may also be input into the system by a care management team made up of professionals responsible for the client's overall care. A risk analyzer may determine the client's risk for different risk conditions, and may calculate domain-specific and overall risk scores for the client. Furthermore, the risk analyzer may make recommendations for actions to be taken by the client, the caregiver, or the care management team in order to improve the client's care. The information in the care management system may be used by the care management team to direct aspects of the client's care.

Claims (54)

1 . A non-transitory electronic device readable medium storing instructions that, when executed by one or more processors, cause the one or more processors to:

identify a profile of a client, the profile specifying demographic or historical information about a status of a client;

present one or more structured questions on an interface relating to a status of the client;

accept structured inputs from a caregiver relating to the status of the client;

analyze the structured inputs based on information in the profile to determine a risk value for the client in one or more domains; and

store the risk value.

2 . The medium of claim 1 , further storing instructions for:

presenting an activity log relating to the regular activities of the client on an interface; and

accepting activity data through the interface, the activity data comprising a level of support provided to the client by the caregiver in performing the regular activities, wherein the risk value is further determined based on the activity data.

3 . The medium of claim 1 , further storing instructions for:

presenting a freeform interface for accepting unstructured freeform notes, wherein the risk value is further determined based on a lexical analysis of the unstructured freeform notes.

4 . The medium of claim 3 , wherein the freeform notes are associated with either one of the one or more structured questions, or an activity in an activity log.

5 . The medium of claim 4 , wherein the one of the one or more structured questions or the activity is associated with a domain from among the one or more domains, and results of the lexical analysis are given increased weight in calculating the risk score for the associated domain.

6 . The medium of claim 1 , further storing instructions for:

receiving a selection of a domain;

in response to the selection of the domain, displaying risk data related to one or more subdomains of the domain, the one or more subdomains comprising a subset of information used to calculate a risk score for the domain.

7 . The medium of claim 1 , further storing instructions for reporting the risk value or a change in the risk value to one or more members of a care management team responsible for the well-being of the client.

8 . The medium of claim 1 , wherein a risk value is calculated for each of a plurality of domains, and further storing instructions for:

calculating an overall risk value based on the risk value for each domain.

9 . The medium of claim 1 , further storing instructions for:

receiving an input from one or more members of a care management team responsible for the well-being of the client; and

using the input to determine the risk value.

10 . The medium of claim 9 , wherein the input from the caregiver is provided on a regular basis that is more frequent than the input from the one or more members of the care management team.

11 . An electronic-device implemented method comprising:

identifying a profile of a client, the profile specifying demographic or historical information about a status of a client;

presenting one or more structured questions on an interface relating to a status of the client;

accepting structured inputs from a caregiver relating to the status of the client;

analyzing, using a processor of an electronic device, the structured inputs based on information in the profile to determine a risk value for the client in one or more domains; and

storing the risk value.

12 . The method of claim 11 , further comprising:

presenting an activity log relating to the regular activities of the client on an interface; and

accepting activity data through the interface, the activity data comprising a level of support provided to the client by the caregiver in performing the regular activities, wherein the risk value is further determined based on the activity data.

13 . The method of claim 11 , further comprising:

presenting a freeform interface for accepting unstructured freeform notes, wherein the risk value is further determined based on a lexical analysis of the unstructured freeform notes.

14 . The method of claim 11 , further comprising reporting the risk value or a change in the risk value to one or more members of a care management team responsible for the well-being of the client.

15 . The method of claim 11 , further comprising:

receiving an input from one or more members of a care management team responsible for the well-being of the client; and

using the input to determine the risk value.

16 . A system comprising:

a non-transitory storage device storing a profile of a client, the profile specifying demographic or historical information about a status of a client; and

one or more processors configured to:

present one or more structured questions on an interface relating to a status of the client;

accept structured inputs from a caregiver relating to the status of the client;

analyze the structured inputs based on information in the profile to determine a risk value for the client in one or more domains; and

store the risk value in the storage device.

17 . The system of claim 16 , wherein the processor is further configured to:

present an activity log relating to the regular activities of the client on an interface; and

accept activity data through the interface, the activity data comprising a level of support provided to the client by the caregiver in performing the regular activities, wherein the risk value is further determined based on the activity data.

18 . The system of claim 16 , wherein the processor is further configured to:

present a freeform interface for accepting unstructured freeform notes, wherein the risk value is further determined based on a lexical analysis of the unstructured freeform notes.

19 . The system of claim 16 , wherein the processor is further configured to report the risk value or a change in the risk value to one or more members of a care management team responsible for the well-being of the client.

20 . The system of claim 16 , wherein the processor is further configured to:

receive an input from one or more members of a care management team responsible for the well-being of the client; and

use the input to determine the risk value.
