Abstract

A method of using CPAP equipment to sense cardiogenic oscillations in a patient's airflow, and to monitor and treat the patient's cardiac condition. The apparatus diagnoses cardiac morbidity conditions, such as the existence of arrhythmias or other cardiac abnormalities, and influences and optimizes cardiac stroke volume. The apparatus further monitors pulse-transit time, changes in the heart pre-ejection period, and the duration of the cardiac cycle.

Claims (25)

1. A method for determining a cardiac condition with a pressure apparatus for treating breathing and controlling delivery of a pressurized gas by the pressure apparatus, comprising steps of:

sensing, with a sensor, cardiogenic pressure oscillations or cardiogenic flow oscillations of a patient;

determining, with a processor, the patient's cardiac condition with the sensed cardiogenic oscillations; and

determining, with the processor and based on one or more sensed cardiogenic oscillations, an adjustment for a pressure delivery device of the pressure apparatus to control delivery of a pressurized gas by the pressure apparatus that would adjust to optimize a stroke volume of the patient.

2. The method of claim 1 , the method further comprising adjusting a pressure of the delivery device of the pressure apparatus, based on the determined adjustment.

3. The method of claim 2 wherein the adjustment for the pressure delivery device comprises titrating positive airway pressure treatment using the pressure apparatus in accordance with one or more values of the sensed cardiogenic oscillations.

4. The method of claim 3 wherein the step of monitoring comprises discontinuous monitoring of heartbeats during a portion of a respiratory cycle.

5. The method of claim 4 wherein the portion comprises a middle to end expiration portion.

6. The method of claim 5 wherein the pressure apparatus for treating breathing comprises a sleep disordered breathing apparatus.

The method of claim 2 wherein the step of determining the patient's cardiac condition comprises examining one or more amplitudes of the sensed cardiogenic oscillations, and wherein determining the adjustment for the pressure delivery device is based on the one or more examined amplitudes of the sensed cardiogenic oscillations.

8. The method of claim 7 wherein the adjustment for the pressure delivery device comprises servo-controlling pressure of the pressure apparatus in accordance with one or more amplitude values of the sensed cardiogenic oscillations.

9. The method of claim 1 wherein the adjustment for the pressure delivery device is determined so as to increase the stroke volume of the patient based on the one or more of the sensed cardiogenic oscillations.

10. The method of claim 1 wherein the adjustment for the pressure delivery device is determined so as to maximize the stroke volume of the patient based on one or more of the sensed cardiogenic oscillations.

11. An apparatus for determining a cardiac condition and providing a pressure for treating breathing, comprising:

at least one sensor to sense cardiogenic pressure oscillations or cardiogenic flow oscillations of a patient; and

a controller, the controller coupled with the sensor and a pressure delivery device, the controller including a processor configured to determine the patient's cardiac condition with the sensed cardiogenic oscillations and to determine, based on one or more sensed cardiogenic oscillations, an adjustment to delivery of pressurized gas by the pressure delivery device that would adjust to optimize a stroke volume of the patient.

12. The apparatus of claim 11 wherein the pressure delivery device is further configured to adjust a pressure of the pressurized gas, based on the determined adjustment.

13. The apparatus of claim 12 wherein the adjustment to the delivery of pressurized gas comprises titrating positive airway pressure treatment using the pressure delivery device of the apparatus based on the sensed cardiogenic oscillations.

14. The apparatus of claim 13 wherein the processor is further configured to perform discontinuous monitoring of heartbeats during a portion of a respiratory cycle.

15. The apparatus of claim 14 wherein the portion comprises a middle to end expiration portion.

16. The apparatus of claim 15 wherein the pressure delivery device comprises a sleep disordered breathing apparatus.

17. The apparatus of claim 12 wherein the cardiac condition determination comprises the processor being further configured to examine one or more amplitude values of the sensed cardiogenic oscillations, and wherein the adjustment to the delivery of pressurized gas is further based on the one or more amplitude values.

18. The apparatus of claim 17 wherein the adjustment to delivery of pressurized gas comprises a servo-controller configured to control a pressure of the pressure delivery device in accordance with the one or more amplitude values.

19. The apparatus of claim 11 wherein the adjustment for the pressure delivery device is determined so as to increase the stroke volume of the patient based on one or more of the sensed cardiogenic oscillations.

20. The apparatus of claim 11 wherein the adjustment for the pressure delivery device is determined so as to maximize the stroke volume of the patient based on one or more of the sensed cardiogenic oscillations.
