Abstract

The invention comprises a method of operating a breathing apparatus comprising measuring a baseline breath flow parameter being respiratory rate and/or tidal volume or a parameter derived therefrom, varying the flow rate provided by the breathing apparatus, measuring a current breath flow parameter being respiratory rate and/or tidal volume or a parameter derived therefrom, comparing the baseline and current breath flow parameters, and altering operation of the breathing apparatus based on the comparison. The invention also comprises a breathing apparatus that implements the above method.

Claims (79)

1. A method of operating a breathing apparatus configured to provide nasal high flow therapy, the method comprising the steps of:

providing fluid to a patient, using the breathing apparatus, at a baseline flow rate, wherein the fluid is provided via an unsealed interface;

measuring, using the breathing apparatus, a baseline breath flow parameter of the patient while providing fluid to the patient at the baseline flow rate, the baseline breath flow parameter comprising a parameter indicative of a baseline respiratory rate and a parameter indicative of a baseline tidal volume;

providing, using the breathing apparatus, fluid to the patient at a first flow rate, the first flow rate being greater than the baseline flow rate;

measuring, using the breathing apparatus, a first breath flow parameter of the patient while providing fluid to the patient at the first flow rate, the first breath flow parameter comprising a parameter indicative of a first respiratory rate and a parameter indicative of a first tidal volume;

comparing, using the breathing apparatus, the first breath flow parameter to the baseline breath flow parameter, wherein the breathing apparatus executes software instructions programmed on the breathing apparatus to cause the breathing apparatus to:

in response to the first respiratory rate being lower than the baseline respiratory rate and the first tidal volume being higher than the baseline tidal volume, determining, using the breathing apparatus, that a sleep state of the patient is awake, and

in response to the first respiratory rate being the same or lower than the baseline respiratory rate and the first tidal volume being lower than the baseline tidal volume, determining, using the breathing apparatus, that the sleep state of the patient is asleep; and

altering operation of the breathing apparatus based on the comparison between the baseline breath flow parameter and the first breath flow parameter, and the sleep state of the patient, wherein operation of the breathing apparatus is altered to change a fluid flow rate provided to the patient.

2. The method according to claim 1 further comprising, in response to determining that the sleep state of the patient is not asleep and the sleep state of the patient is not awake, setting the baseline breath flow parameter to the first breath flow parameter.

3. The method according to claim 2 further comprising after the first breath flow parameter becomes the baseline breath flow parameter,

providing fluid, using the breathing apparatus, to the patient at a second flow rate, the second flow rate being different than the first flow rate,

measuring, using the breathing apparatus, a second breath flow parameter of the patient, the second breath flow parameter comprising a parameter indicative of a second respiratory rate and a parameter indicative of a second tidal volume,

comparing, using the breathing apparatus, the second breath flow parameter to the baseline breath flow parameter of the patient measured previously at the first flow rate, and

altering operation of the breathing apparatus based on the comparison.

4. The method according to claim 1 comprising:

increasing, using the breathing apparatus, the first flow rate provided to the patient in response to the determination that the sleep state of the patient is asleep or

decreasing, using the breathing apparatus, the first flow rate provided to the patient in response the determination that the sleep state of the patient is awake.

5. The method according to claim 1 wherein measuring using the breathing apparatus, the first breath flow parameter to compare the first breath flow parameter against the baseline breath flow parameter is triggered by one or more of:

time or

patient physiology.

6. The method according to claim 1 wherein operation of the breathing apparatus is altered by changing a fan speed to alter the fluid flow rate generated by a blower of the breathing apparatus.

7. A breathing apparatus to provide nasal high flow therapy to a patient, the apparatus comprising:

a blower,

at least one sensor, and

a controller configured to:

operate the blower to generate a fluid flow at a baseline flow rate to a patient,

measure using the sensor a baseline breath flow parameter of the patient at the baseline flow rate, the baseline breath flow parameter comprising a parameter indicative of a baseline respiratory rate and a parameter indicative of a baseline tidal volume,

operate the blower to generate fluid flow at a first flow rate to the patient, the first flow rate being greater than the baseline flow rate,

measure using the sensor a first breath flow parameter of the patient at the first flow rate, the first breath flow parameter comprising a parameter indicative of a first respiratory rate and a parameter indicative of a first tidal volume,

compare the first breath flow parameter to the corresponding baseline breath flow parameter of the patient, and

alter operation of the breathing apparatus based on the comparison,

wherein the controller is further configured to:

in response to the first respiratory rate being lower than the baseline respiratory rate and the first tidal volume being higher than the baseline tidal volume, determine that a sleep state of the patient is awake, and

in response to the first respiratory rate being the same or lower than the baseline respiratory rate and the first tidal volume being lower than the baseline tidal volume, determine that the sleep state of the patient is asleep, and

wherein altering operation of the breathing apparatus based on the comparison between the baseline breath flow parameter and the first breath flow parameter, utilises the determination of the sleep state of the patient, wherein operation of the breathing apparatus is altered to change a fluid flow rate provided to the patient.

8. The apparatus according to claim 7 wherein the controller is further configured to:

in response to a determination that the sleep state of the patient is not asleep and the sleep state of the patient is not awake, set the baseline breath flow parameter to the first breath flow parameter.

9. The apparatus according to claim 8 wherein, after the first breath flow parameter becomes the baseline breath flow parameter, the controller is further configured to:

provide fluid to the patient at a second flow rate, the second flow rate being different to the first flow rate,

measure a second breath flow parameter of the patient, the second breath flow parameter comprising a parameter indicative of a second respiratory rate and a parameter indicative of a second tidal volume,

compare the second breath flow parameter to the baseline breath flow parameter of the patient measured previously at the first flow rate, and

alter operation of the breathing apparatus based on the comparison.

10. An apparatus according to claim 7 wherein the controller is further configured to operate the blower to:

increase the first flow rate provided to the patient in response to the determination that the sleep state of the patient is asleep, or

decrease the first flow rate provided to the patient in response to the determination that the sleep state of the patient is asleep.

11. An apparatus according to claim 7 wherein measuring using the breathing apparatus, the first breath flow parameter to compare the first breath flow parameter against the baseline breath flow parameter is triggered by one or more of:

time or

patient physiology.

12. The breathing apparatus according claim 7 wherein operation of the breathing apparatus is altered by changing a fan speed to alter the fluid flow rate generated by the blower.

13. A method of operating a breathing apparatus to provide nasal high flow therapy to a patient comprising the steps of:

providing fluid, using the breathing apparatus, to a patient at a baseline flow rate, wherein the baseline flow rate is a nominal flow rate, wherein the fluid is provided via an unsealed interface;

measuring, using the breathing apparatus, a baseline breath flow parameter of the patient while providing fluid to the patient at the baseline flow rate, the baseline flow parameter comprising a parameter indicative of a baseline respiratory rate and a parameter indicative of a baseline tidal volume;

increasing, using the breathing apparatus, a fluid flow rate provided to the patient above the baseline flow rate to a first flow rate;

measuring, using the breathing apparatus, a first breath flow parameter of the patient while providing fluid to the patient at the first flow rate, the first breath flow parameter comprising a parameter indicative of a first respiratory rate and a parameter indicative of a first tidal volume, wherein the breathing apparatus executes software instructions programmed on the breathing apparatus to cause the breathing apparatus to:

in response to the first respiratory rate being lower than the baseline respiratory rate and the first tidal volume being higher than the baseline tidal volume, determining, using the breathing apparatus, that the sleep state of the patient is awake;

in response to the first respiratory rate being the same or lower than the baseline respiratory rate and the first tidal volume being lower than the baseline tidal volume, determining, using the breathing apparatus, that the sleep state of the patient is asleep; and

in response to the determination that the sleep state of the patient is asleep, providing the fluid flow rate to the patient at a flow rate which is higher than the baseline flow rate.

14. The method according to claim 13 wherein increasing using the breathing apparatus, the fluid flow rate provided to the patient above the baseline flow rate to the first flow rate comprises:

increasing the fluid flow rate to a fixed higher flow rate higher than the nominal flow rate, or

ramping the fluid flow rate at a first ramp rate.

15. The method according to claim 14 wherein providing, using the breathing apparatus, the fluid flow rate to the patient higher than the baseline flow rate being the nominal flow rate comprises:

increasing the fluid flow rate to a fixed higher flow rate higher than the nominal flow rate, or

ramping the fluid flow rate at a second ramp rate higher than the first ramp rate.

16. A breathing apparatus to provide nasal high flow therapy to a patient comprising:

a flow generator operated by a controller for delivering fluid flow to a patient, the controller for operating the apparatus configured to:

provide fluid to the patient at a baseline flow rate, wherein the baseline flow rate is a nominal flow rate;

measure a baseline breath flow parameter of the patient while providing fluid to the patient at the baseline flow rate, the baseline flow parameter comprising a parameter indicative of a baseline respiratory rate and a parameter indicative of a baseline tidal volume;

increase a fluid flow rate provided to the patient above the baseline flow rate to a first flow rate;

measure a first breath flow parameter of the patient while providing fluid to the patient at the first flow rate, the first breath flow parameter comprising a parameter indicative of a first respiratory rate and a parameter indicative of a first tidal volume;

in response to the first respiratory rate being lower than the baseline respiratory rate and the first tidal volume being higher than the baseline tidal volume, determine that the sleep state of the patient is awake;

in response to the first respiratory rate being the same or lower than the baseline respiratory rate and the first tidal volume being lower than the baseline tidal volume, determine that the sleep state of the patient is asleep; and

in response to the sleep state of the patient being asleep, providing the fluid flow rate to the patient higher than the baseline flow rate.

17. The breathing apparatus according to claim 16 wherein increasing the fluid flow rate provided to the patient above the baseline flow rate to the first flow rate further comprises:

increasing the fluid flow rate to a fixed higher flow rate higher than the nominal flow rate, or

ramping the fluid flow rate at a first ramp rate.

18. The breathing apparatus according claim 17 wherein providing the fluid flow rate to the patient higher than the nominal flow rate comprises:

increasing the fluid flow rate to a fixed higher flow rate higher than the nominal flow rate, or

ramping the fluid flow rate at a second ramp rate higher than the first ramp rate.
