Abstract

A dilator retractor and the dilators that are used for minimally invasive spinal surgery or other surgery are configured to accommodate the anatomical structure of the patient as by configuring the cross sectional area in an elliptical shape, or by forming a funnel configuration with the wider end at the proximate end. In some embodiments the distal end is contoured to also accommodate the anatomical structure of the patient so that a cylindrically shaped, funnel shaped, ovoid shaped dilator retractor can be sloped or tunneled to accommodate the bone structure of the patient or provide access for implants. The dilator retractor is made with different lengths to accommodate the depth of the cavity formed by the dilators.

Claims (21)

1. A method of positioning an implant relative to a bone anchor, comprising:

making a skin incision;

inserting a distal end of a guide wire into the skin incision;

advancing the distal end of the guide wire into proximity to a vertebra, the guide wire extending from proximate the vertebra to external to the skin incision;

inserting an instrument over the guide wire, the instrument having a cut-out formed in a sidewall of the instrument at a distal end of the instrument;

removing the guide wire, the instrument defining a working channel from the skin incision to proximate the vertebra; and

passing an implant through the working channel and through the cut-out to position the implant in a bone anchor.

2. The method of claim 1 , wherein the implant is a rod.

3. The method of claim 1 , wherein the bone anchor is a pedicle screw.

4. The method of claim 1 , further comprising advancing a plurality of cannulated dilators over the guide wire to sequentially dilate the skin incision.

5. The method of claim 1 , wherein the instrument is cylindrical.

6. The method of claim 1 , wherein the instrument is non-cylindrical.

7. A method of positioning an implant relative to a bone anchor, the method comprising:

forming a skin incision;

inserting a distal end of an instrument through the skin incision, the distal end having diametrically-opposed sidewall cut-outs formed therein;

positioning the instrument such that the distal end of the instrument is proximate to a vertebra to define a working channel from the skin incision to a bone anchor coupled to the vertebra; and

passing an implant through the cut-outs of the instrument to position the implant relative to the bone anchor.

8. The method of claim 7 , wherein the implant is a spinal rod.

9. The method of claim 7 , wherein the bone anchor is a pedicle screw.

10. The method of claim 7 , further comprising sequentially dilating the skin incision prior to inserting the instrument through the skin incision.

11. The method of claim 10 , wherein sequentially dilating the skin incision comprises inserting a non-cannulated dilator through the skin incision and inserting one or more cannulated dilators over the non-cannulated dilator.
