Abstract

Devices, systems, and methods for visually depicting a vessel and evaluating treatment options are disclosed. The methods can include introducing instruments into the vessel of a patient and obtaining proximal and distal pressure measurements of a stenosis of the vessel, calculating a pressure ratio based on the obtained proximal and distal pressure measurements, applying a correlation factor to the calculated pressure ratio to produce a predicted diagnostic pressure ratio, and displaying the predicted diagnostic pressure ratio to a user.

Claims (39)

1. A method of evaluating a vessel of a patient, comprising:

obtaining, from a pressure sensor or transducer of a first intravascular pressure-sensing catheter or guidewire positioned within the vessel, distal pressure measurements within the vessel at a position distal of a stenosis of the vessel without using a hyperemic agent, the first intravascular pressure-sensing catheter or guidewire comprising a proximal portion, a distal portion, and the pressure sensor or transducer coupled to the distal portion;

obtaining, from a different, second intravascular pressure-sensing catheter or guidewire positioned within the vessel, proximal pressure measurements within the vessel at a position proximal of the stenosis of the vessel without using the hyperemic agent;

calculating a non-hyperemic, non-diagnostic pressure ratio for each cardiac cycle based on the distal pressure measurements and the proximal pressure measurements in the vessel of the patient during each cardiac cycle of the patient;

applying a correlation factor to the calculated non-hyperemic, non-diagnostic pressure ratio to produce a predicted diagnostic pressure ratio, wherein the predicted diagnostic pressure ratio correlates to at least one of fractional flow reserve (FFR) or instantaneous wave-free ratio (iFR); and

displaying the predicted diagnostic pressure ratio to a user.

2. The method of claim 1 , wherein the correlation factor has a fixed value.

3. The method of claim 1 , wherein the correlation factor is variable.

The method of claim 3 , wherein the correlation factor has a first correlation value for calculated non-hyperemic, non-diagnostic pressure ratios below a first threshold, a second correlation value for calculated non-hyperemic, non-diagnostic pressure ratios between the first threshold and a second threshold, and a third correlation value for calculated non-hyperemic, non-diagnostic pressure ratios above the second threshold.

5. The method of claim 1 , wherein the correlation factor is defined based on a relationship of a slope of a plot of calculated non-hyperemic, non-diagnostic pressure ratios relative to accepted diagnostic pressure ratios.

6. The method of claim 1 , wherein the correlation factor is defined based on a relationship of a measure of curvature of a plot of calculated non-hyperemic, non-diagnostic pressure ratios relative to accepted diagnostic pressure ratios.

7. The method of claim 1 , wherein the correlation factor is defined based on a relationship of an approximated polynomial function of a plot of calculated non-hyperemic, non-diagnostic pressure ratios relative to accepted diagnostic pressure ratios.

8. The method of claim 1 , wherein the correlation factor is defined based on a relationship of an area below a curve of a plot of calculated non-hyperemic, non-diagnostic pressure ratios to accepted diagnostic pressure ratios.

9. The method of claim 1 , further comprising:

introducing the first intravascular pressure-sensing catheter or guidewire into the vessel of the patient; and

introducing the second intravascular pressure-sensing catheter or guidewire into the vessel of the patient.

10. The method of claim 1 , wherein the calculated non-hyperemic, non-diagnostic pressure ratio is not FFR or iFR.

11. The method of claim 1 , wherein the correlation factor is based on at least one characteristic of the patient.

12. The method of claim 1 , wherein the calculating the non-hyperemic, non-diagnostic pressure ratio is based on an average distal pressure measurement and an average proximal pressure measurement.

13. A system for evaluating a vessel of a patient, comprising:

a first intravascular pressure-sensing catheter or guidewire configured to be positioned within the vessel, the first intravascular pressure-sensing catheter or guidewire comprising a proximal portion, a distal portion, and a pressure sensor or transducer coupled to the distal portion;

a processing system comprising a processor in communication with the first intravascular pressure-sensing catheter or guidewire and a different, second intravascular pressure-sensing catheter or guidewire, the processor of the processing system configured to:

obtain, from the pressure sensor or transducer of the first intravascular pressure-sensing catheter or guidewire, distal pressure measurements measured within the vessel at a position distal of a stenosis of the vessel without using a hyperemic agent;

obtain, from the second intravascular pressure-sensing catheter or guidewire, proximal pressure measurements measured within the vessel at a position proximal of the stenosis of the vessel without using a hyperemic agent;

calculate a non-hyperemic, non-diagnostic pressure ratio based on the obtained proximal and distal pressure measurements;

apply a correlation factor to the calculated non-hyperemic, non-diagnostic pressure ratio to produce a predicted diagnostic pressure ratio, wherein the predicted diagnostic pressure ratio correlates to at least one of fractional flow reserve (FFR) or instantaneous wave-free ratio (iFR); and

cause a display to display the predicted diagnostic pressure ratio.

14. The system of claim 13 , wherein the correlation factor has a fixed value.

15. The system of claim 13 , wherein the correlation factor is variable.

16. The system of claim 15 , wherein the correlation factor has a first correlation value for calculated non-hyperemic, non-diagnostic pressure ratios below a first threshold, a second correlation value for calculated non-hyperemic, non-diagnostic pressure ratios between the first threshold and a second threshold, and a third correlation value for calculated non-hyperemic, non-diagnostic pressure ratios above the second threshold.

17. The system of claim 14 , wherein the correlation factor is defined based on a relationship of a slope of a plot of calculated non-hyperemic, non-diagnostic pressure ratios relative to accepted diagnostic pressure ratios.

18. The system of claim 13 , wherein the correlation factor is defined based on a relationship of a measure of curvature of a plot of calculated non-hyperemic, non-diagnostic pressure ratios relative to accepted diagnostic pressure ratios.

19. The system of claim 13 , wherein the correlation factor is defined based on a relationship of an approximated polynomial function of a plot of calculated non-hyperemic, non-diagnostic pressure ratios relative to accepted diagnostic pressure ratios.

20. The system of claim 13 , wherein the correlation factor is defined based on a relationship of an area below a curve of a plot of calculated non-hyperemic, non-diagnostic pressure ratios relative to accepted diagnostic pressure ratios.

21. The system of claim 13 , wherein the processor of the processing system is configured to calculate the non-hyperemic, non-diagnostic pressure ratio based on an average distal pressure measurement and an average proximal pressure measurement.

22. The system of claim 13 , wherein the correlation factor is defined based on previously collected pressure measurements.

23. The system of claim 13 , wherein the relationship between the non-hyperemic, non-diagnostic pressure ratio and the diagnostic pressure ratio is determined using the same pressure data.

24. The system of claim 21 , wherein the average distal pressure measurement and the average proximal pressure measurement are calculated over an entire heartbeat cycle.

25. The system of claim 13 , wherein the correlation factor is determined based on a relationship between non-hyperemic, non-diagnostic pressure ratio values and diagnostic pressure ratio values.
