Abstract

Instruments and surgical techniques may be used to correct a bone deformity, such as a bunion in which a metatarsal is misaligned with respect to a medial cuneiform. In some examples, a bone cutting guide is utilized to cut an end of the metatarsal and/or an end of the medial cuneiform to facilitate realignment of the bones. The cutting guide can have a pivotable cut guide surface along which a cutting instrument is translated to provide a precise bone cut. In some applications, after suitably preparing and aligning the bones, the bones are compressed together using a fixation pin. The fixation pin can be driven through one of the bones, into the second bone, and then further driven to compress the bones together.

Claims (65)

1. A method of comprising:

preparing an end of a first metatarsal;

preparing an end of a medial cuneiform opposing the end of the first metatarsal;

moving the first metatarsal from an anatomically misaligned position with respect to a second metatarsal to establish a moved position of the first metatarsal;

driving a threaded end portion of a bone fixation pin into one of the first metatarsal and the medial cuneiform;

driving the threaded end portion of the bone fixation pin through the one of the first metatarsal and the medial cuneiform;

driving the threaded end portion of the bone fixation pin into an other of the first metatarsal and the medial cuneiform until a collar of the bone fixation pin is in apposition to the one of the first metatarsal and the medial cuneiform; and

further driving the threaded end portion of the bone fixation pin into the other of the first metatarsal and the medial cuneiform to compress together the first metatarsal and the medial cuneiform.

The method of claim 1 , wherein, moving the first metatarsal comprises moving the first metatarsal after driving the threaded end portion into the one of the first metatarsal and the medial cuneiform and prior to driving the threaded end portion into the other of the first metatarsal and the medial cuneiform.

3. The method of claim 1 , wherein the one of the first metatarsal and the medial cuneiform is the first metatarsal and the other of the first metatarsal and the medial cuneiform is the medial cuneiform.

4. The method of claim 3 , wherein

driving the threaded end portion into the medial cuneiform until the collar is in apposition to the first metatarsal comprises driving the threaded end portion into the medial cuneiform until the collar contacts the first metatarsal, and

further driving the threaded end portion into the medial cuneiform to compress together the first metatarsal and the medial cuneiform comprises driving the collar against the first metatarsal and advancing the first metatarsal into contact with the medial cuneiform.

5. The method of claim 1 , wherein the bone fixation pin comprises a shaft, the threaded end portion is on a leading end of the shaft, and the collar comprises a region of the shaft having a greater cross-sectional dimension than a remainder of the shaft.

6. The method of claim 5 ,

wherein the shaft is configured to be detached proximally of the collar, thereby providing an implantable portion and a removable portion, and

further comprising, subsequent to further driving the threaded end portion into the other of the first metatarsal and the medial cuneiform, detaching the removable portion of the shaft proximally of the collar.

The method of claim 6 , wherein the shaft comprises a main drive engagement surface on a proximal end of the shaft and a secondary drive engagement surface on the collar, the secondary drive engagement surface being configured to drive the implantable portion of the shaft after detaching the removable portion.

8. The method of claim 1 , wherein preparing the end of the first metatarsal and the end of the medial cuneiform comprises:

positioning a projection of a block of a cutting guide at least partially within a tarsal-metatarsal joint between the first metatarsal and medial cuneiform, the block being pivotally connected to a first guide member;

aligning the first guide member at a location to be cut, wherein a first guide surface of the first guide member is positioned at the location to be cut;

fixing the first guide member to a bone; and

making a first cut at the location to be cut by placing a cutting member in apposition to the first guide surface.

9. The method of claim 1 , further comprising, subsequent to compressing the first metatarsal and the medial cuneiform together, applying one or more bone plates to the first metatarsal and the medial cuneiform.

The method of claim 1 , wherein the collar is positioned at a location along a length of the bone fixation pin effective to allow the bone fixation pin to be driven through the one of the first metatarsal and the medial cuneiform but not through the other of the first metatarsal and the medial cuneiform.

11. The method of claim 1 , wherein the bone fixation pin comprises a shaft and the collar is positioned along a length of the shaft such that a distal edge of the collar ranges from about 20 mm to about 60 mm from a distal-most tip of the bone fixation pin.

12. The method of claim 1 , wherein bone fixation pin comprises a shaft and the collar is an enlarged region of the shaft, the shaft has a diameter ranging from 1.2 mm to 3.5 mm, and the collar has a diameter ranging from 0.1 mm to 10 mm larger than the diameter of the shaft.

13. The method of claim 1 , wherein the bone fixation pin comprises a shaft, and the shaft is threaded along at least 10% of its length between a distal tip of the shaft and a distal edge of the collar.

14. The method of claim 13 , wherein the shaft is threaded from 10% to 65% of its length between the distal tip of the shaft and the distal edge of the collar.

15. The method of claim 1 , wherein moving the first metatarsal from the anatomically misaligned position with respect to the second metatarsal to establish the moved position of the first metatarsal comprises moving the first metatarsal to an anatomically aligned position.

16. The method of claim 1 , wherein moving the first metatarsal from the anatomically misaligned position with respect to the second metatarsal to establish the moved position of the first metatarsal comprises moving the first metatarsal after preparing the end of the first metatarsal and after preparing the end of the medial cuneiform opposing the end of the first metatarsal.

17. The method of claim 1 , wherein moving the first metatarsal from the anatomically misaligned position with respect to the second metatarsal to establish the moved position of the first metatarsal comprises moving the first metatarsal before preparing the end of the first metatarsal and before preparing the end of the medial cuneiform opposing the end of the first metatarsal.

18. The method of claim 1 , wherein:

preparing the end of the first metatarsal comprises cutting the end of the first metatarsal, and

preparing the end of the medial cuneiform comprises cutting the end of the medial cuneiform.

19. The method of claim 1 , wherein:

preparing the end of the first metatarsal comprises morselizing the end of the first metatarsal, and

preparing the end of the medial cuneiform comprises morselizing the end of the medial cuneiform.

20. The method of claim 1 , wherein:

preparing the end of the first metatarsal comprises cutting and morselizing the end of the first metatarsal, and

preparing the end of the medial cuneiform comprises cutting and morselizing the end of the medial cuneiform.

21. The method of claim 1 , wherein the bone fixation pin comprises a shaft, the collar comprises a region of the shaft having a greater cross-sectional dimension than a remainder of the shaft, and a position of the collar along a length of the shaft is fixed.

22. A method comprising:

preparing an end of a first metatarsal;

preparing an end of a medial cuneiform opposing the end of the first metatarsal across a tarsal-metatarsal joint;

moving the first metatarsal from an anatomically misaligned position with respect to a second metatarsal to establish a moved position of the first metatarsal;

compressing the end of the first metatarsal against the end of the medial cuneiform across the tarsal-metatarsal joint by driving a bone fixation pin with a collar through one of the first metatarsal and the medial cuneiform and into an other of the first metatarsal and the medial cuneiform, wherein driving the bone fixation pin comprises pressing the collar against a surface of the one of the first metatarsal and the medial cuneiform to apply compression; and

applying a permanent fixation device across the tarsal-metatarsal joint.

23. The method of claim 22 , wherein the permanent fixation device comprises a bone plate.

24. The method of claim 22 , further comprising, after applying the permanent fixation device across the tarsal-metatarsal joint, removing the bone fixation pin from the first metatarsal and from the medial cuneiform.

25. The method of claim 22 , wherein moving the first metatarsal comprises moving the first metatarsal after driving the bone fixation pin into the one of the first metatarsal and the medial cuneiform and prior to driving the bone fixation pin into the other of the first metatarsal and the medial cuneiform.

26. The method of claim 22 , wherein the one of the first metatarsal and the medial cuneiform is the first metatarsal and the other of the first metatarsal and the medial cuneiform is the medial cuneiform.

27. The method of claim 22 , wherein the bone fixation pin comprises a shaft with a threaded end portion on a leading end of the shaft, and the collar comprises a region of the shaft having a greater cross-sectional dimension than a remainder of the shaft.

28. The method of claim 22 , wherein the collar is positioned at a location along a length of the bone fixation pin effective to allow the bone fixation pin to be driven through the one of the first metatarsal and the medial cuneiform but not through the other of the first metatarsal and the medial cuneiform.

29. The method of claim 22 , wherein:

the bone fixation pin comprises a shaft and the collar is positioned along a length of the shaft such that a distal edge of the collar ranges from about 20 mm to about 60 mm from a distal-most tip of the bone fixation pin,

the shaft has a diameter ranging from 1.2 mm to 3.5 mm, and

the collar has a diameter ranging from 0.1 mm to 10 mm larger than the diameter of the shaft.

30. The method of claim 22 , wherein the bone fixation pin comprises a shaft, and the shaft is threaded along at least 10% of its length between a distal tip of the shaft and a distal edge of the collar.

31. The method of claim 22 , wherein moving the first metatarsal from the anatomically misaligned position with respect to the second metatarsal to establish the moved position of the first metatarsal comprises moving the first metatarsal to an anatomically aligned position.

32. The method of claim 22 , wherein moving the first metatarsal from the anatomically misaligned position with respect to the second metatarsal to establish the moved position of the first metatarsal comprises moving the first metatarsal after preparing the end of the first metatarsal and after preparing the end of the medial cuneiform opposing the end of the first metatarsal.

33. The method of claim 22 , wherein moving the first metatarsal from the anatomically misaligned position with respect to the second metatarsal to establish the moved position of the first metatarsal comprises moving the first metatarsal before preparing the end of the first metatarsal and before preparing the end of the medial cuneiform opposing the end of the first metatarsal.

34. The method of claim 22 , wherein:

preparing the end of the first metatarsal comprises at least one of cutting and morselizing the end of the first metatarsal, and

preparing the end of the medial cuneiform comprises at least one of cutting and morselizing the end of the medial cuneiform.
