Abstract

According to some embodiments, a method of treating a subject having diabetes or symptoms associated with diabetes is provided. The method includes delivering a neuromodulation catheter within a vessel (e.g., hepatic artery) having surrounding nerves that innervate the liver (e.g., sympathetic nerves of the hepatic plexus). The method may also include modulating (e.g., disrupting, ablating, stimulating) the nerves by mechanical compression, energy delivery, or fluid delivery.

Claims (45)

1. A method of altering glucose levels of a subject, the method comprising:

intravascularly advancing a distal portion of an ablation device to a location within a portion of a common hepatic artery of the subject;

ablating nerves surrounding the common hepatic artery with the ablation device;

positioning the distal portion of the ablation device at a location within a portion of a splenic artery;

ablating nerves surrounding the splenic artery with the ablation device;

positioning the distal portion of the ablation device at a location within a portion of a superior mesenteric artery;

ablating nerves surrounding the superior mesenteric artery with the ablation device; and

removing the ablation device from the subject.

2. The method of claim 1 , wherein the ablation device comprises an ultrasound ablation catheter.

3. The method of claim 2 , wherein the ultrasound ablation catheter is configured to deliver ultrasound energy having a frequency between 20 kHz and 20 MHz.

4. The method of claim 1 , wherein the ablation device comprises a radiofrequency ablation catheter and wherein the distal portion of the ablation device comprises at least one electrode.

5. The method of claim 4 , wherein the radiofrequency ablation catheter is configured to deliver radiofrequency energy having a frequency between 100 kHz and 2.5 MHz.

6. The method of claim 1 , wherein the ablation device comprises a cryoablation catheter, and wherein ablating the nerves comprises using cryoblation to ablate the nerves.

7. The method of claim 1 , wherein ablating the nerves comprises delivering chemical agents to the nerves configured to cause chemoablation of the nerves.

8. The method of claim 1 , wherein the ablation device comprises a laser ablation catheter.

9. A method of altering glucose levels of a subject, the method comprising:

intravascularly advancing a distal portion of a neuromodulation device to a location within a portion of a hepatic artery of the subject;

modulating nerves surrounding the hepatic artery with the neuromodulation device;

positioning the distal portion of the neuromodulation device at a location within a portion of a splenic artery;

modulating nerves surrounding the splenic artery with the neuromodulation device;

positioning the distal portion of the neuromodulation device at a location within a portion of a superior mesenteric artery;

modulating nerves surrounding the superior mesenteric artery with the neuromodulation device; and

removing the neuromodulation device from the subject.

10. The method of claim 9 ,

wherein the hepatic artery is a common hepatic artery,

wherein the neuromodulation device comprises at least one ultrasound transducer,

wherein modulating the nerves comprises causing the at least one ultrasound transducer to deliver energy to the nerves at a frequency in a range of 20 kHz to 20 MHz, and

wherein the energy is sufficient to cause ablation of the nerves.

11. The method of claim 9 , wherein modulating the nerves comprises delivering energy in a circumferential pattern.

12. The method of claim 9 , wherein the neuromodulation device is selected from the group consisting of a radiofrequency energy delivery device and an ultrasound energy delivery device.

13. A method of altering glucose levels of a subject, the method comprising:

denervating sympathetic nerves of a hepatic plexus with an energy delivery device from a location within a common hepatic artery of the subject;

denervating sympathetic nerves of a splenic plexus with the energy delivery device;

denervating sympathetic nerves of a superior mesenteric plexus; and

removing the energy delivery device from the subject.

14. The method of claim 13 ,

wherein a distal portion of the energy delivery device comprises at least one ultrasound transducer,

wherein a frequency of the energy is between 20 kHz and 20 MHz,

wherein the energy delivery device comprises a balloon.

15. The method of claim 13 , further comprising denervating sympathetic nerves of the hepatic plexus from a location within a proper hepatic artery of the subject.

16. The method of claim 13 , wherein the energy delivery device comprises at least one ultrasound transducer.

17. The method of claim 13 , further comprising cooling the energy delivery device.

18. The method of claim 13 , wherein a distal portion of the energy delivery device comprises at least one electrode, and wherein a frequency of the energy is between 100 kHz and 2.5 MHz.

19. The method of claim 18 , wherein the at least one electrode is configured to maintain contact with the inner wall at a contact pressure of between 0.1 g/mm 2 and 10 g/mm 2 .

20. The method of claim 13 , wherein the energy delivery device comprises a microwave energy delivery device.
