Abstract

A fracture mobility testing system is provided for use in surgical procedures for augmenting vertebral bodies having collapsed due to compression fractures. The testing system is utilized to determine if the cortical shell of the vertebral body has begun to heal over the fracture lines to the point at which height restoration is not possible. Depending on the feedback provided by the testing system, the surgeon may elect to proceed with any of a variety of known height restoration techniques if the fractured portions of the vertebral body are still mobile, or may elect for a simple vertebroplasty procedure without height restoration if the feedback from the testing system determines that the cortical outer portions of the fractured vertebral body have fused to one another to the point at which height restoration is no longer an option.

Claims (56)

1. A method of diagnostic testing for determining fracture mobility of a bone, comprising:

preparing an expandable balloon for insertion into a bone;

capturing and storing a first image of the bone and displaying the first image;

determining a location of a superior endplate and an inferior endplate of a vertebral body of the bone;

prior to performing a repair procedure, inserting the expandable balloon into the bone;

inflating the balloon with a solution having radiopaque properties;

capturing and storing a second image including at least one of the bone and the inflated balloon while inflating the expandable balloon and displaying the second image;

comparing the first image and the second image to determine an amount of fracture height restoration between the superior endplate and the inferior endplate with the balloon inflated;

deflating the balloon;

capturing and storing a third image of the bone and displaying the third image;

comparing the third image to at least one of the first image and the second image to determine an amount of fracture height restoration between the superior endplate and the inferior endplate after the balloon has deflated; and

performing the repair procedure by inserting a non-fluid implant within the bone, the repair procedure determined based on at least one of the amount of fracture height restoration of the bone with the balloon inflated and the amount of fracture height restoration after the balloon has deflated.

2. The method of claim 1 , further comprising:

adjusting a position of the expandable balloon within the bone; and

re-inflating the expandable balloon to determine where a fault line is located on the bone.

3. The method of claim 1 , wherein determining the amount of fracture height restoration of the bone comprises determining the amount of fracture height restoration of a fractured portion of the vertebral body.

4. The method of claim 3 , wherein determining the amount of fracture height restoration with the balloon inflated comprises comparing the first image and the second image to determine a change in separation between the superior and inferior endplates of the vertebral body.

5. The method of claim 4 , wherein determining the amount of fracture height restoration after the balloon has deflated comprises comparing the first image and the third image to determine a change in separation between the superior and inferior endplates of the vertebral body.

6. The method of claim 3 , further comprising:

determining that a fracture of the vertebral body is mobile based on a determination of a positive change in separation between the superior and inferior endplates indicating a corresponding positive amount of fracture height restoration;

creating a cavity within the vertebral body.

7. The method of claim 6 , wherein the non-fluid implant is inserted into the cavity created in the vertebral body.

8. The method of claim 3 , further comprising performing the repair procedure by injecting cement into the vertebral body.

9. The method of claim 3 , further comprising:

preparing a second expandable balloon for insertion into the vertebral body;

inserting the second balloon into the vertebral body;

inflating the second balloon with a solution having radiopaque properties;

capturing and storing a fourth image including at least one of the bone, the inflated balloon, and the inflated second balloon and displaying the fourth image;

comparing the first image with the fourth image to determine an amount of fracture height restoration between the superior endplate and the inferior endplate with the balloon and the second balloon inflated;

deflating the second balloon;

capturing and storing a fifth image of the bone and displaying the fifth image;

comparing the first image and the fifth image to determine an amount of fracture height restoration between the superior endplate and the inferior endplate after the balloon and the second balloon have been deflated.

10. The method of claim 9 , wherein the balloon in inserted through a first bipedicular access corridor provided in a first pedicle of the vertebral body and the second balloon is inserted through second bipedicular access corridor provided in a second pedicle of the vertebral body.

11. The method of claim 3 , wherein inserting the balloon into the bone includes inserting the balloon through a first bipedicular access corridor provided in a first pedicle of the vertebral body.

12. The method of claim 11 , further comprises:

removing the balloon from the bone through the first bipedicular access corridor;

re-inserting balloon into the bone through a second bipedicular access corridor provided in a second pedicle of the vertebral body;

re-inflating the expandable balloon;

determining an amount of fracture height restoration between the superior endplate and the inferior endplate with the balloon re-inflated;

deflating the balloon; and

determining an amount of fracture height restoration between the superior endplate and the inferior endplate after the balloon has deflated.

13. The method of claim 1 , further comprising:

creating an access corridor to a treatment area of the bone, wherein inserting the balloon into the bone includes inserting the balloon through the access corridor.

14. The method of claim 13 , wherein the access corridor is created by performing a sequence of cannula-trocar-drill-plunger access steps.

15. The method of claim 1 , wherein capturing an image is capturing the first image, second image and the third image comprises capturing first, second, and third x-ray image, respectively.

16. The method of claim 1 , further comprising:

sensing at least one of a volume and pressure of an inflatable medium used during inflation of the balloon within the bone, and

tracking a change in the at least one of a volume and pressure data to determine a difference the mobility of bone within the treatment area in response to inflation of the balloon.

17. The method of claim 1 , wherein capturing the first, second and third images comprises positioning an imaging device in an anterior-posterior position.

18. The method of claim 17 , further comprising:

positioning a second imaging device in a lateral position;

capturing and storing a first, second, and third lateral image of the bone, corresponding to the first, second and third image;

combining the first, second, and third images with the corresponding first, second, and third lateral images to provide a bi-planar projection of the bone,

wherein the imaging device is position in the anterior-posterior position such that pedicles of the vertebral body appear symmetric.

19. The method of claim 1 , wherein determining the amount of fracture height restoration with the balloon inflated comprises comparing the first image and the second image to determine if the superior endplate and the inferior endplate have moved with respect to each other,

wherein determining the amount of fracture height restoration after the balloon has deflated comprises comparing the first image and the third image to determine if the superior endplate and the inferior endplate have moved with respect to each other.
