Abstract

Disclosed is a method for enhancing tumor response to chemotherapy, the method comprising administering a short-acting anti-angiogenic agent (AAA) capable of activating ASMase to a subject afflicted with a solid tumor, and thereby creating a time interval of increased susceptibility of said tumor to one or more chemotherapeutic agents, followed by administration of at least one chemotherapeutic agent within the interval. The interval can be defined in terms of a short-duration activation of ASMase signaling by the AAA. Disclosed are also methods for predicting the tumor response in a patient afflicted with a solid tumor to a chemotherapeutic agent, using as an indicator of the response ASMase level or activity (or ceramide level) in the patient following the administration of the chemotherapeutic agent to the patient, or dynamic IVIM based DW-MRI to measure perfusion alterations following administration of the chemotherapeutic agent.

Claims (16)

1. A method for enhancing tumor response to chemotherapy in a subject having a solid tumor comprising:

(a) administering a short-acting anti-angiogenic agent (AAA) to the subject under conditions that result in the tumor having an increased susceptibility to at least one chemotherapeutic agent that is capable of activating ASMase/ceramide signaling pathway;

(b) administering the at least one chemotherapeutic agent to the subject at a time point at which the tumor has increased susceptibility to the at least one chemotherapeutic agent thereby enhancing the effect of the at least one chemotherapeutic agent against the tumor compared to that observed with the chemotherapeutic agent being used (i) without the AAA or (ii) at a time point outside the time point of step (b),

wherein the AAA is a small molecule inhibitor of receptor tyrosine kinase;

wherein the chemotherapeutic agent is selected from the group consisting of taxanes, alkylating agents, topoisomerase inhibitors, antimetabolites, and any combination thereof,

wherein the administration of the chemotherapeutic agent is commenced within about 0.5 to about 2 hours after administration of the AAA, and

wherein an enhanced tumor response comprises a reduction in tumor growth relative to that observed with the chemotherapeutic agent in the absence of the AAA.

2. The method of claim 1 further comprising: repeating steps (a) and (b) after a time period that is sufficient for the AAA to decay.

The method of claim 2 wherein decay of the AAA is assessed by measuring one or more of: serum levels of the AAA, restoration of biologic output of ASMase such that ASMase activity increases upon a subsequent administration of the AAA, restoration of biologic output of ceramide, such that ceramide increases upon a subsequent administration of the AAA, and perfusion alteration upon a subsequent administration of the AAA.

4. The method of claim 1 wherein the AAA has an average plasma half-life of less than about 120 hours.

5. The method of claim 1 wherein the AAA is one or more agents selected from the group consisting of cediranib, axitinib, sunitinib, sorafenib, pazopanib, vatalanib, cabozantinib, ponatinib, lenvatinib, and SU6668.

The method of claim 1 wherein the chemotherapeutic agent is at least one selected from the group consisting of chlorambucil, cyclophosphamide, ifosfamide, melphalan, streptozocin, carmustine, lomustine, busulfan, dacarbazine, temozolomide, thiotepa, altretamine, 5-fluorouracil (5-FU), 6-mercaptopurine (6-MP), capecitabine, cytarabine, floxuridine, fludarabine, gemcitabine, hydroxyurea, methotrexate, pemetrexed, SN-38, ARC, NPC, topotecan, 9-nitrocamptothecin, 9-aminocamptothecin, gimatecan, diflomotecan, BN80927, MAG-CPT, etoposide, etoposide phosphate, teniposide, paclitaxel, docetaxel, baccatin III, 10-deacetyltaxol, 7-xylosyl-10-deacetyltaxol, cephalomannine, 10-deacetyl-7-epitaxol, 7-epitaxol, 10-deacetylbaccatin III, 10-deacetyl cephalomannine, nimustine, ranimustine, bendamustine, uramustine, estramustine, mannosulfan, camptothecin, exatecan, DX-8951f, lurtotecan, lamellarin, amsacrine, ellipticines, aurintricarboxylic acid, HU-331 and mixtures thereof.

The method of claim 1 wherein the tumor is selected from the group consisting of adrenal, anal, bile duct, bladder, bone, brain/CNS, breast, cervical, colon/rectum, endometrial, esophageal, eye, gallbladder, gastrointestinal, kidney, heart, head and neck, laryngeal and hypopharyngeal, liver, lung, oral mesothelioma, nasopharyngeal, neuroblastoma, ovarian, pancreatic, peritoneal, pituitary, prostate, retinoblastoma, rhabdomyosarcoma, salivary gland, sarcoma, skin, small intestine, stomach, soft tissue sarcoma, testicular, thymus, thyroid, parathyroid, uterine, and vaginal tumors and metastases thereof.

8. The method of claim 1 wherein the administration of the chemotherapeutic agent is commenced within about 1 hour after administration of the AAA.

9. The method of claim 1 wherein the chemotherapeutic agent is paclitaxel, etoposide, gemcitabine, and 5-fluorouracil (5-FU).

10. The method of claim 1 wherein the AAA is sorafenib, or lenvatinib.
