Abstract

Systems and methods for joint preparation and fusion are disclosed. The system includes a cutting device having a handle, and rigid blade member. The blade member may include cutting edges on first and second sides, and may be curved in one or more planes. The blade member may be deployed to project outside of an outer tube, and the cutting device may be rotated to create a circular cavity. In a method of use, a cannula provides access to a procedure site such as a joint. A pathway to the joint is created, and the cutting device is inserted and deployed to undercut a cavity in the joint. A fusion device may be implanted across the joint to provide compression and fuse the joint. The system may also include instrumentation for creating access to a joint, bone graft insertion and implant insertion.

Claims (31)

1. An instrument for cutting bone, comprising:

a longitudinal axis; and

a blade assembly comprising a blade body and a blade holder;

wherein the blade body comprises a proximal end, a distal end, first and second opposing sides extending between the proximal and distal ends, and a void, wherein the blade body is curved in a first plane between the proximal and distal ends, wherein the first side has a first curved cutting edge, wherein the second side has a second curved cutting edge opposite the first curved cutting edge, wherein the void is open to the first and second sides;

wherein the blade holder comprises a distal blade end that comprises a ramped surface that extends distally and laterally away from the longitudinal axis, wherein the proximal end of the blade body is hinged to the distal blade end so that the blade body is free to rotate relative to the blade holder about a hinge axis;

wherein the blade assembly is movable between a retracted position, in which the blade body extends along the longitudinal axis, and an extended position, in which the blade body protrudes away from the longitudinal axis and the ramped surface functions as a physical stop to rotation of the blade body relative to the blade holder about the hinge axis.

2. The instrument of claim 1 , wherein the blade assembly comprises a blade shaft, wherein the blade shaft extends along the longitudinal axis and comprises a distal attachment feature, wherein the blade body is removably connectable to the distal attachment feature of the blade shaft.

3. The instrument of claim 1 , comprising:

an actuation mechanism, wherein actuation of the actuation mechanism moves the blade assembly between the retracted position and the extended position.

4. The instrument of claim 3 , comprising:

a handle, wherein the blade assembly extends distally from the handle.

5. An instrument for cutting bone, comprising:

a longitudinal axis; and

a blade assembly comprising a blade body and a blade holder;

wherein the blade body comprises a proximal end, a distal end, opposing first and second sides extending between the proximal and distal ends, and a void, wherein the blade body is curved in a first plane between the proximal and distal ends, wherein the first plane is between the first and second sides, wherein the first side has a first curved cutting edge, wherein the second side has a second curved cutting edge opposite the first curved cutting edge, wherein the void extends through the blade body through the first and second sides;

wherein the blade holder comprises a distal blade end that comprises a ramped surface that extends distally and laterally away from the longitudinal axis, wherein the proximal end of the blade body is hinged to the distal blade end so that the blade body is free to rotate relative to the blade holder about a hinge axis;

wherein the blade assembly is movable between a retracted position, in which the blade body extends along the longitudinal axis, and an extended position, in which the blade body protrudes away from the longitudinal axis and the ramped surface functions as a physical stop to rotation of the blade body relative to the blade holder about the hinge axis.

6. The instrument of claim 5 , wherein the blade assembly comprises a blade shaft, wherein the blade shaft extends along the longitudinal axis and comprises a distal attachment feature, wherein the blade body is removably connectable to the distal attachment feature of the blade shaft.

7. The instrument of claim 5 , comprising:

an actuation mechanism, wherein actuation of the actuation mechanism moves the blade assembly between the retracted position and the extended position.

8. The instrument of claim 7 , comprising:

a handle, wherein the blade assembly extends distally from the handle.

9. An instrument for cutting bone, comprising:

a longitudinal axis; and

a blade assembly comprising a blade body and a blade holder;

wherein the blade body comprises a proximal end, an opposite distal end, a front side extending between the proximal and distal ends, an opposite back side extending between the proximal and distal ends, opposing first and second sides extending between the proximal and distal ends, and a void, wherein the blade body is curved between the proximal and distal ends in a first plane so that the front side is concave in the first plane and the back side is convex in the first plane, wherein the first plane is between the first and second sides, wherein the first side has a first curved cutting edge that extends along the front side, around the distal end, and along the back side, wherein the second side has a second curved cutting edge opposite the first curved cutting edge, wherein the second cutting edge extends along the front side, around the distal end, and along the back side, wherein the void extends through the first and second sides of the blade body;

wherein the blade holder comprises a distal blade end that comprises a ramped surface that extends distally and laterally away from the longitudinal axis, wherein the proximal end of the blade body is hinged to the distal blade end so that the blade body is free to rotate relative to the blade holder about a hinge axis;

wherein the blade assembly is movable between a retracted position, in which the blade body extends along the longitudinal axis, and an extended position, in which the blade body protrudes away from the longitudinal axis and the ramped surface functions as a physical stop to rotation of the blade body relative to the blade holder about the hinge axis.

10. The instrument of claim 9 , wherein the blade assembly comprises a blade shaft, wherein the blade shaft extends along the longitudinal axis and comprises a distal attachment feature, wherein the blade body is removably connectable to the distal attachment feature of the blade shaft.

11. The instrument of claim 9 , comprising:

an actuation mechanism, wherein actuation of the actuation mechanism moves the blade assembly between the retracted position and the extended position.
