Abstract

Devices and techniques for adjusting an alignment for a first metatarsal may include making a plurality of cuts along the length of the first metatarsal. In different applications of the technique, a plurality of transverse cuts may or may not be made intersecting the longitudinal cuts. In either case, the cuts can separate the first metatarsal into two individual portions and release a removable wedge of bone. The different portions of the metatarsal can be moved relative to each other to adjust an anatomical alignment of one portion relative to another portion, for example in multiple planes. After suitably adjusting the alignment of the different bone portions, the portions can be fixed together.

Claims (33)

1. A bone cutting guide comprising:

a body configured to be positioned against a bone to be cut, wherein the body includes a longitudinal cutting slot having a first end and a second end, a first transverse cutting slot extending downwardly from the first end of the longitudinal cutting slot, and a second transverse cutting slot extending upwardly from the second end of the longitudinal cutting slot; wherein the body is sized to be positioned against a first metatarsal; and wherein the body is configured such medial side of the first metatarsal, the first transverse cutting slot is position on the medial side of the first metatarsal, and the second transverse cutting slot is positioned extending from the medial side of the first metatarsal to a dorsal side of the first metatarsal.

2. The guide of claim 1 , wherein the body defines a curvature configured to conform to the bone to be cut.

3. The guide of claim 1 , wherein the first transverse cutting slot intersects the longitudinal cutting slot and the second transverse cutting slot also intersects the longitudinal cutting slot.

4. The guide of claim 1 , wherein the body further comprises a second longitudinal cutting slot extending parallel to the first longitudinal cutting slot.

5. The guide of claim 4 , wherein the second longitudinal cutting slot has a first end and a second end, and further comprising a third transverse cutting slot extending upwardly from the second end of the second longitudinal cutting slot.

6. The guide of claim 4 , wherein the first longitudinal cutting slot and the second longitudinal cutting slot are offset from each other a distance ranging from 2 mm to 35 mm.

7. The guide of claim 6 , wherein the first longitudinal cutting slot and the second longitudinal cutting slot are angled relative to each other to form cutting lines that converge at an angle ranging from 5 degrees to 45 degrees.

8. The guide of claim 1 , further comprising a depth limiter along which a cutting instrument is configured to be translated when cutting through the longitudinal cutting slot.

9. The guide of claim 8 , wherein the depth limiter is part of the body.

10. The guide of claim 8 , wherein the depth limiter comprises a surface projecting outwardly from the body.

11. The guide of claim 10 , wherein the surface projecting outwardly comprises a first surface projecting outwardly from the body on one side of the longitudinal cutting slot and a second surface projecting outwardly from the body on an opposite side of the longitudinal cutting slot.

12. The guide of claim 10 , wherein the surface projects outwardly from the body a distance ranging from 0.25 mm to 10 mm.

13. The guide of claim 1 , wherein the body further comprises at least two fixation apertures each configured to receive a fixation element to secure the body to the bone.

14. The guide of claim 13 , wherein the at least two fixation apertures each comprise slots adjacent to the first transverse cutting slot and the second transverse cutting slot.

The guide of claim 13 , wherein the body has a first longitudinal side edge and a second longitudinal side edge, one of the at least two fixation apertures is located between the first longitudinal side edge of the body and the first end of the longitudinal cutting slot, and another of the at least two fixation apertures is located between the second longitudinal side edge of the body and the second end of the longitudinal cutting slot.

16. The guide of claim 1 , wherein

the first transverse cutting slot extends downwardly from the first end of the longitudinal cutting slot to form a first intersection angle ranging from 90 degrees to 135 degrees; and

the second transverse cutting slot extends upwardly from the second end of the longitudinal cutting slot to form a second intersection angle ranging from 90 degrees to 135 degrees.

The guide of claim 16 , wherein the first intersection angle and the second intersection angle are each greater than 90 degrees and oriented such that, when the body is positioned against the bone, the first transverse cutting slot projects in a proximal direction along the bone moving away from the longitudinal cutting slot and the second transverse cutting slot projects in a distal direction along the bone moving away from the longitudinal cutting slot.

18. The guide of claim 1 , wherein the longitudinal cutting slot, the first transverse cutting slot, and the second transverse cutting slot each have a width ranging from 0.5 mm to 3 mm.

19. A bone cutting guide comprising:

a body having a length extending from a first end to a second end and being sized to be positioned against a first metatarsal,

the body including a longitudinal cutting slot extending between the first end and the second end, a first securing projection extending at an angle relative to the length in one direction and containing at least one fixation aperture, and a second securing projection extending at an angle to the length in an opposite direction and containing at least one fixation aperture.

20. The guide of claim 19 , wherein the first securing projection and the second securing projection each comprise a plurality of fixation apertures.

21. The guide of claim 20 , wherein the plurality of fixation apertures are linearly and/or radially aligned, thereby configuring the bone cutting guide to be positioned at different degrees of rotation about the first metatarsal.

22. The guide of claim 21 , wherein longitudinal cutting slot has a first longitudinal end and a second longitudinal end, and the first longitudinal end and the second longitudinal end each define a fixation aperture.

23. The guide of claim 19 , wherein the first securing projection and the second securing projection each extend substantially orthogonally relative to the length.

24. The guide of claim 19 , wherein the first securing projection and the second securing projection are each located adjacent the first end of the body, and further comprising a third securing projection and a fourth securing projection each located adjacent the second end of the body, the third securing projection extending at an angle relative to the length in the one direction and containing at least one fixation aperture, and the fourth securing projection extending at an angle to the length in the opposite direction and containing at least one fixation aperture.

25. The guide of claim 19 , wherein the longitudinal cutting slot has a width ranging from 0.1 mm to 3 mm.

26. The guide of claim 19 , wherein the body defines a base at the first end and further comprising a handle attached to the base.

27. The guide of claim 26 , wherein the handle extends from the body at an angle ranging from 10 degrees to 90 degrees relative to the length of the body.

28. The guide of claim 26 , wherein the handle is threadingly coupled to the body.
